Abstract

Background: HIV is a retrovirus targeting the immune system, requiring continuous antiretro-viral therapy (ARVs) for effective management. In Indonesia, 329,581 HIV cases were documented from 1987 to 2022, with only 51% receiving ARVs. Adherence is crucial for treatment success, but prolonged therapy may impact medication compliance. This study investigates the correlation between ARV treatment duration and adherence levels at Dr. Moewardi Hospital. Subjects and Method: A cross-sectional observational study was conducted at the Voluntary Counseling and Testing (VCT) clinic, involving 60 HIV-diagnosed individuals selected through purposive sampling. Therapy duration was extracted from medical records, and adherence was assessed using the MMAS-8 questionnaire during June and July 2024. Bivariate analysis employed the Spearman correlation coefficient test. Results: The study sample predominantly consisted of individuals aged 36-45 years, with the majority having completed high school education. The mean therapy duration was 5.60 ± 3.64 years, ranging from 1 to 15 years. Adherence levels varied significantly, with 48.3% of patients demonstrating high adherence, characterized by a mean therapy duration (Mean= 6.86 years; SD= 3.62). Moderate adherence was observed in 16.7% of participants, with a mean therapy duration (Mean= 6.60 years; SD= 2.55). Low adherence was found in 35.0% of patients, associated with a shorter mean therapy duration (Mean= 3.38 years; SD= 3.14). There was a positive and sig¬nificant association between therapy duration and adherence level (r= 0.415; p= 0.001). Conclusion: The length of antiretroviral therapy (ART) exhibits a favorable correlation with adherence levels among individuals living with HIV. An extended duration of the therapeutic regimen is correlated with an enhanced adherence rate. This observation underscores the significance of sustained support, particularly during the initial years of the treatment course.

Abstract

Background: Conjunctival tumor patients tend to exhibit a compromised health-related quality of life (HR-QoL) as a consequence of their condition.Objectives: To explore the quality-of-life (QoL) of conjunctival tumor patients and its association with psychological distress in Indonesia.Methods: A cross-sectional study with simple random sampling was used. Data collection occurred from May 9 to August 31, 2024. The health-related dimensions of quality of life were measured utilizing the reliable RAND SF-36, while psychological distress was assessed with the Hospital Anxiety and Depression Scale (HADS). We performed statistical analyses using version 26 of IBM SPSS, with normality assessed via skewness.Results: A total of 260 conjunctival tumors completed health assessments, revealing significant psychological distress and the RAND SF 36 questionnaire. The mean age was 51, 52.3% male, and 86.2% married. Participants had an average HADS anxiety score of 9.52, with 60.8% exhibiting anxiety, notably higher in those with malignant tumors (mean score: 10.64) compared to benign tumors (mean score: 8.57, p<0.001). Depression scores averaged 6.97, with 37.7% showing potential mild disorders. Health-related quality of life (HR-\QoL) was appraised across various domains, alongside overall physical functioning, averaging 60.76; benign patients scored significantly higher (71.95) than malignant patients (47.50). Correlations indicated that higher psychological distress negatively impacted QoL, with anxiety showing a strong correlation coefficient of -0.433 and depression at -0.306 (both p<0.001).Conclusion: Psychological distress is prevalent in conjunctival tumor patients, adversely affecting their QoL. These findings highlight the necessity of targeted interventions to enhance both physical fitness and mental wellness—challenges in this population. Further research is essential to develop effective support strategies tailored to their needs.

Abstract

BACKGROUND As quality of life (QoL) research continues to expand, there remains a study gap of less common malignancies such as conjunctival tumors. This study aimed to investigate the differences in QoL between Indonesian patients with benign and malignant conjunctival tumors. METHODS This cross-sectional study was conducted from May–September 2024 and included 273 patients diagnosed with conjunctival tumors. The participants provided informed consent and completed the RAND SF-36 questionnaire to assess their QoL. A consecutive sampling method was employed to adequately represent benign and malignant tumor types. Data were analyzed using IBM SPSS version 26 to identify QoL differences among the patients. RESULTS This study found that squamous cell carcinoma was the most frequently observed conjunctival tumor, accounting for 28.9% of the cases. Patients with benign tumors had significantly higher QoL scores (69.61) than those with malignant tumors (43.05). Key factors influencing QoL included tumor type (p<0.001), age (p<0.001), sex (p = 0.039), occupation (p = 0.027), residence (p = 0.044), income (p = 0.010), and comorbidities (p = 0.045). CONCLUSIONS The results show significant disparities in QoL between patients with benign and malignant conjunctival tumors, emphasizing the impact of sociodemographic and tumor type on patient well-being. Focused healthcare plans are needed to help improve the QoL of patients with conjunctival tumors. Prioritizing early detection and providing complete support to enhance patient outcomes in Indonesia are crucial.

Abstract

Pendahuluan: Pembangunan kesehatan bertujuan meningkatkan mutu sumber daya manusia dan lingkungan melalui pendekatan paradigma kesehatan. Di Desa Kragilan, yang merupakan daerah endemis demam berdarah, mahasiswa KKN kedokteran dalam rangka meningkatkan kesadaran kesehatan masyarakat mengadakan kegiatan pengabdian masyarakat berupa “Pemberantasan Sarang Nyamuk” untuk meningkatkan derajat kesehatan masyarakatMetode: Kegiatan ini dilakukan sebanyak 3 kali pada tanggal 20-22 Juli 2024 pada setiap rumah di RT Jetis, Sidorejo, dan Bibisari Desa Kragilan. Rumah yang didatangi di RT Jetis sebanyak 50 rumah, RT Sidorejo sebanyak 25 rumah, dan RT Bibisari sebanyak 40 rumah. Sosialisasi dilakukan menggunakan poster mengenai demam berdarah secara umum dan upaya pencegahan dengan metode 3M (Menutup, Menguras, Mengubur). Setelah itu dilanjutkan dengan melakukan pemeriksaan bak mandi dan genangan air serta penyerahan abate pada akhir kegiatan.Hasil dan pembahasan: Kegiatan “Pemberantasan Sarang Nyamuk” pada 20-22 Juli 2024 mencakup 40 rumah per RT. Mahasiswa KKN dan kader jumantik memeriksa jentik nyamuk di bak dan genangan air, memberikan abate, dan memberikan edukasi terkait penggunaannya serta pentingnya menguras bak mandi. Warga dan kader antusias, tetapi ada kendala karena beberapa warga menolak kunjungan dengan alasan privasi, sehingga beberapa rumah tidak diperiksa. Secara keseluruhan, kegiatan ini berhasil menambah pengetahuan masyarakat terkait pencegahan demam berdarah.Kesimpulan: Kegiatan “Pemberantasan Sarang Nyamuk” meningkatkan kesadaran khususnya masyarakat Desa Kragilan  mengenai penyakit Demam Berdarah, baik itu gejala, faktor risiko, dan cara pencegahan serta menambah pengetahuan masyarakat mengenai penyakit Demam Berdarah.
