Abstract

Cancer cell have been shown to extensively use glucose for proliferation, even at hypoxia environment. The consumption of glucose in a hypoxia environment induced HIF and VEGF, which play a role in cancer cell infiltration process . The objective of study is to determine the relationship between fasting blood glucose levels with the amount of infiltrated axillary lymph node in patients with infiltrating ductal carcinoma. This is an analytic observational study with cross sectional design. The data obtained were tested with chi-square test (α < 0.05). This study was conducted on 24 infiltrating ductal carcinoma patients. The results showed p value of 0.004, indicating there is a significant correlation between fasting blood glucose levels with the amount of infiltrated axillary lymph nodes. The correlation coefficient ( r) value was 0.505; indicating medium level correlation. As conclusion, there is a significant correlation between fasting blood glucose levels with the amount of infiltrated axillary lymph node in patients with infiltrating ductal carcinoma.Keywords: axillary lymph nodes, breast cancer, fasting blood glucose level, infiltrating ductal carcinoma

Abstract

Kanker payudara merupakan kanker paling umum pada wanita di seluruh dunia dan merupakan kanker paling banyak terjadi pada wanita. WHO telah merekomendasikan klasifikasi IMT termasuk derajat underweight atau overweight berhubungan dengan peningkatan risiko beberapa penyakit tidak menular. Differensiasi tingkat keganasan untuk kanker payudara, menggunakan kriteria WHO yaitu system grading Nottingham (juga disebut modifikasi Elston_Ellis dari sistem grading Scarff-Bloom-Richardson). Penelitian ini bertujuan untuk mengetahui hubungan Index Massa Tubuh dengan Grading pada kanker payudara. Penelitian ini merupakan studi observasional analitik dengan desain Cross Sectional untuk mempelajari hubungan Indeks Massa Tubuh dengan Grading kanker payudara. Karaktersitik berdasarkan umur minimum 28 tahun dan usia maksimum 64 tahun, berat badan pasien minimum 40 kg dan maksimum 75 kg. Tinggi badan pasien minimum 141 cm dan maksimum 163 cm,skor IMT minimum 17.22 dan maksimum 31.18. Berdasarkan IMT dalam kategori Underwight ada 2 orang (6,7%), IMT dalam kategori normowight ada 18 orang (60,0%), dan pasien dengan IMT dalam kategori overwight ada 10 orang (33.3%), dengan demikian responden mayoritas dengan hasil pengukuran IMT dalam kategori normowight. Derajat diferensiasi sel dalam kategori Grade I ada 2 orang (6,7%), derajat diferensiasi sel dalam kategori Grade II ada 8 orang (26,7%), dan derajat diferensiasi sel dalam kategori Grade III ada 20 orang (66.7%), dengan demikian responden mayoritas dengan hasil pengukuran derajatdiferensiasiseldalam kategori Grade III. Pasien dengan grading kanker payudara grade 1 dengan IMT underweight ada 1 orang dan normowight ada 1 orang, pada grading kanker payudara grade II yang normoweight ada 7 orang dan overweight ada 1 orang, dan pada grading kanker payudara grade III dengan IMT underweight ada 1 orang normoweight ada 10 orang dan overweight ada 9 orang. IMT overwight lebih berisiko terhadap tingginya grade pada grading kanker payudara, nilai koefisen korelasi sebesar 0,396 dengan nilai p=0,045 (p<0,05) yang berarti terdapat hubungan yang signifikan antara Index Masa Tubuh dengan Grading pada kanker payudara. Kata Kunci: Kanker Payudara, Index Masa Tubuh, Differensiasi Histopatologi

Abstract

Background: Conjunctival tumor patients tend to exhibit a compromised health-related quality of life (HR-QoL) as a consequence of their condition.Objectives: To explore the quality-of-life (QoL) of conjunctival tumor patients and its association with psychological distress in Indonesia.Methods: A cross-sectional study with simple random sampling was used. Data collection occurred from May 9 to August 31, 2024. The health-related dimensions of quality of life were measured utilizing the reliable RAND SF-36, while psychological distress was assessed with the Hospital Anxiety and Depression Scale (HADS). We performed statistical analyses using version 26 of IBM SPSS, with normality assessed via skewness.Results: A total of 260 conjunctival tumors completed health assessments, revealing significant psychological distress and the RAND SF 36 questionnaire. The mean age was 51, 52.3% male, and 86.2% married. Participants had an average HADS anxiety score of 9.52, with 60.8% exhibiting anxiety, notably higher in those with malignant tumors (mean score: 10.64) compared to benign tumors (mean score: 8.57, p<0.001). Depression scores averaged 6.97, with 37.7% showing potential mild disorders. Health-related quality of life (HR-\QoL) was appraised across various domains, alongside overall physical functioning, averaging 60.76; benign patients scored significantly higher (71.95) than malignant patients (47.50). Correlations indicated that higher psychological distress negatively impacted QoL, with anxiety showing a strong correlation coefficient of -0.433 and depression at -0.306 (both p<0.001).Conclusion: Psychological distress is prevalent in conjunctival tumor patients, adversely affecting their QoL. These findings highlight the necessity of targeted interventions to enhance both physical fitness and mental wellness—challenges in this population. Further research is essential to develop effective support strategies tailored to their needs.

Abstract

Introduction: Invasive papillary carcinoma represents a rare subtype of breast cancer, constituting about 0.5% of all invasive breast cancers and mainly occurring in postmenopausal women. Because most information comes from limited case reports and small retrospective analyses, awareness of this tumor remains low, posing challenges for clinical management. There are distinct pathological characteristics and biological behaviors seen in invasive papillary carcinoma, according to the research.  One reason for its generally good prognosis is because it is seldom involved with lymph nodes and is classified as a luminal subtype. [1, 2]. Case Presentation: We report a rare case of Invasive Papillary Breast Carcinoma in a 48-year-old premenopausal woman who presented with a gradually enlarging, painless retroareolar mass in her right breast, persisting for four months. The patient previously underwent an incisional biopsy at another hospital, and the histopathology results showed invasive papillary breast carcinoma. The patient has undergone a chest  CT-scan for diagnostic confirmation and surgical guidance. The patient underwent a right-sided modified radical mastectomy, and histopathological examination of the specimen confirmed the diagnosis of Invasive Papillary Carcinoma. Immunohistochemical analysis further validated this diagnosis. Conclusion: Although the treatment of Invasive Papillary Breast Carcinoma generally aligns with that of more common breast cancers, its protocol remains unclear due to the condition's rarity. We chose to present this Invasive Papillary Breast Carcinoma case because of the disease's uncommon nature, the severity of the symptoms, and the need for urgent intervention.

Abstract

Background: The Short Form 36 (SF-36) questionnaire was used to measure quality of life (QoL) in Indonesia for patients with various conditions. However, its validity and reliability, specifically for patients with conjunctival tumors, remain understudied. Given the limited research on health-related quality of life (HR-QoL) in this patient population, our research aims to fill this gap by validating the SF-36 specifically for conjunctival tumor patients. Method: This observational, cross-sectional study recruited patients with conjunctival tumors receiving treatment at the Department of Ophthalmology, Dr Moewardi Hospital, between May and July 2024, using consecutive sampling. Participants completed the SF-36 questionnaire in the Indonesian context. Validity was assessed through bivariate correlation analysis, examining the Pearson correlation value for each item's questionnaire with the total QoL score. Reliability was determined using scale reliability analysis, focusing on Cronbach's alpha coefficient. The data was processed using IBM SPSS version 26.Results: The Pearson correlation coefficients for the 36-item scores range from 0.345 to 0.797, indicating a moderate to significant correlation. The scale-level Cronbach's alpha for the comprehensive tool is 0.942, suggesting excellent internal consistency. The SF-36 demonstrated robust reliability across its eight domains, confirming its suitability for measuring QoL in this demographic. These findings confirm that the SF-36 questionnaire in the Indonesian context is highly reliable and has construct validity in conjunctival tumor patients.Conclusion: The SF-36 questionnaire in the Indonesian context exhibits a satisfactory reliability scale and construct validity in assessing the QoL of conjunctival tumor patients. Further research with a larger participant pool is recommended.

Abstract

Introduction: Myxofibrosarcoma (MFS) is a fibroblast-derived sarcoma, which accounts for approximately 5–10% of all soft tissue malignant tumors. The mean age in patients with MFS is between the fifth and seventh decades. Around 77% of MFS cases occur in the upper extremities. Other areas of the body including the trunk (12%), retroperitoneum or mediastinum (8%), abdominal wall, and heart have also been reported. Case Presentation: A 57-year-old Indonesian female visited our center with complaints of a lump in the left lower abdomen for 4 months. The lump has grown rapidly in the last two months. A contrast abdominal MRI examination found a soft tissue mass on the left lumbar abdominal wall to the left iliac with a size of 15 x 11,4 x 13,4 cm and bordered by part of the left external oblique muscle and the left internal oblique muscle pushing the intestinal system medially. The biopsy of the mass revealed a high-grade myxofibrosarcoma. The patient underwent a wide excision with an elliptical incision design, with in-toto removal of the lump. The immunohistochemical result of the tumor showed high-grade myxofibrosarcoma with free margins at the incision margin. Follow-up was performed at 1 month, 6 months, and 12 months postoperatively. The patient did not receive radiotherapy. Conclusions: This report describes a rare case of MFS of the abdominal wall. Recognizing the histopathological features of MFS and applying the appropriate immunohistochemical examinations are crucial in establishing the correct diagnosis. This case provides a diagnosis and treatment experience of MFS that occurs in the abdominal wall.

Abstract

BACKGROUND As quality of life (QoL) research continues to expand, there remains a study gap of less common malignancies such as conjunctival tumors. This study aimed to investigate the differences in QoL between Indonesian patients with benign and malignant conjunctival tumors. METHODS This cross-sectional study was conducted from May–September 2024 and included 273 patients diagnosed with conjunctival tumors. The participants provided informed consent and completed the RAND SF-36 questionnaire to assess their QoL. A consecutive sampling method was employed to adequately represent benign and malignant tumor types. Data were analyzed using IBM SPSS version 26 to identify QoL differences among the patients. RESULTS This study found that squamous cell carcinoma was the most frequently observed conjunctival tumor, accounting for 28.9% of the cases. Patients with benign tumors had significantly higher QoL scores (69.61) than those with malignant tumors (43.05). Key factors influencing QoL included tumor type (p<0.001), age (p<0.001), sex (p = 0.039), occupation (p = 0.027), residence (p = 0.044), income (p = 0.010), and comorbidities (p = 0.045). CONCLUSIONS The results show significant disparities in QoL between patients with benign and malignant conjunctival tumors, emphasizing the impact of sociodemographic and tumor type on patient well-being. Focused healthcare plans are needed to help improve the QoL of patients with conjunctival tumors. Prioritizing early detection and providing complete support to enhance patient outcomes in Indonesia are crucial.

Abstract

Introduction: Mediastinum tumors are a group of tumors that grow in the mediastinum. Teratomas of the anterior mediastinum are a rare case in adulthood, accounting for only 1-3% of all teratomas. While mature teratoma in the mediastinum is typically benign, it can grow significantly large and invade vital structures such as the esophagus and trachea, and can be misdiagnosed as a neck mass. This case highlights the importance of thorough evaluation in diagnosing mediastinum tumors that involve both the cervical and thoracic regions to prevent misleading diagnoses.Case Presentation: This report discusses the case of a 33-year-old male initially diagnosed with a thyroid tumor. However, upon further examination, it was found that the mass originated from the anterior mediastinum, compressing and displacing the trachea, esophagus, and thyroid gland, necessitating surgical excision involving a multidisciplinary team. In addition to the removal of the tumor, partial esophageal wall excision and a gastrostomy procedure were performed to maintain nutritional flow during the healing process.Conclusions: Mature teratoma in the anterior mediastinum can cause compression and invasion of vital structures. A thorough evaluation in diagnosing mediastinum tumors that involve both the cervical and thoracic regions should be done. This case underscores the need for a multidisciplinary approach to prevent misdiagnosis and long-term follow-up to monitor the potential for recurrence or malignant transformation in patients with mediastinal tumors.

Abstract

Introduction Squamous cell carcinoma (SCC) continues to be a health problem worldwide with far reaching effects not only on survival but on patient's quality of life (QoL). Health-related quality of life (HRQoL) assessment sheds lights on overall disease and its treatment impact. Method In this cross-sectional study was performed on 53 patients diagnosed with SCC, which included 36 intraoral and 17 extraoral cases. Sociodemographic data was collected and the QoL was assessed by using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and short form-36 (SF-36). The data were analyzed using Mann-Whitney U test and Spearman’s correlation. Results Among 53 SCC patients (36 intraoral, 17 extraoral; mainly middle-aged females of low socioeconomic status), overall Quality of Life (QoL) score did not differ between groups. In summary, the analyses revealed additional differences in QoL determinants as patients with intraoral head and neck Squamous cell carcinoma (SCC) QoL was driven towards functional domains whereas symptom-related determinants were dominated for extraoral SCC patients. There was no statistically significant difference in overall QoL (p = 0.839) between groups. Discussion This study provides evidence suggesting that QoL impairments associated with intraoral SCC and with extraoral SCC are primarily driven by symptom burden and pain, and that there is no difference between the groups in overall quality of life (QoL). The relationship between QoL and functional limitations was more pronounced for intraoral SCC, whereas symptom severity had a greater impact on mental status for extraoral SCC. These results are consistent with existing evidence that SCC affects multiple dimensions of health irrespective across the sites of the tumor. There are QoL data available for management of various chronic conditions, and they highlight the importance of more comprehensive, patient-centred care that emphasizes early symptom control along with optimal pain management and functional rehabilitation strategies. Conclusion Demographics do not affect the quality of life of SCC patients, as clinical variables like symptoms, pain, and limitations impact it more than demographic data. Patients with intraoral SCC also experienced moderate QoL, highlighting opportunities for better symptom control and psychosocial support to improve overall patient health-related QoL.

Abstract

Introduction: Breast cancer is the most frequently diagnosed cancer and the leading cause of cancer-related death among women worldwide, accounting for 25% of all cancer cases and 15% of cancer-related mortality in women. Adjuvant therapies include chemotherapy, hormonal therapy, targeted therapy, radiation, or combinations thereof. The quality of life (QoL) of breast cancer patients varies depending on type of therapy, patient’s condition, and associated side effects. Evaluating QoL differences between hormonal therapy and chemotherapy is essential to assess treatment outcomes and guide clinical decisions. Methods: This cross-sectional study involved 240 breast cancer survivors treated at the Oncology Department of Dr. Moewardi Hospital. Selected participants using random sampling in two groups:  120 patients each groups undergoing chemotherapy and hormonal therapy. Data were collected through interviews and standardized questionnaires using the EORTC QLQ-C30 instrument. QoL data were categorized, and statistical analysis was performed using chi-square tests to examine associations between treatment type and QoL. Logistic regression was used to assess the strength of relationships between variables. Results: Groups receiving hormonal therapy, 116 (96.7%) reported good QoL, and 4 (3.3%) reported moderate QoL. In the chemotherapy group, 80 (66.7%) reported good QoL, 39 (32.5%) moderate, and 1 (0.8%) poor QoL. A statistically significant difference in QoL was observed between the two groups (p = 0.001). Dyspnoea was the only symptom that showed a significant association with treatment type (r = 0.208; p = 0.020). Across general health status, functional scales, and symptom scales, hormonal therapy was associated with better QoL outcomes than chemotherapy. Conclusion: There is a significant difference in the QoL of breast cancer patients undergoing hormonal therapy compared to chemotherapy. Hormonal therapy was a better overall QoL.
