Abstract

Background: Maternal mortality remains a significant public health concern in Indonesia, where 19.3% of deliveries are still conducted outside healthcare facilities or without skilled birth attendants. The working area of Ambukha Public Health Centre (Puskesmas) has the lowest proportion of facility-based deliveries in the West Nias District (67% in 2023), indicating the need to analyse factors associated with delivery place choice. Objective: To identify factors related to the selection of delivery place among mothers in the working area of Ambukha Public Health Centre, Lolofitu Moi Subdistrict, West Nias District. Methods: This study employed an observational analytic design with a cross-sectional approach. The population consisted of all mothers who delivered within the Ambukha Public Health Centre area in 2023 (N=78), and samples were obtained using a total sampling approach. Data were collected using structured questionnaires and analysed using Chi-square tests and logistic regression. Results: Significant associations were found between the choice of delivery place and maternal knowledge (p=0.000; OR=2.267), maternal attitude (p=0.000; OR=4.250), national health insurance (JKN) membership (p=0.000; OR=5.333), access to healthcare facilities (p=0.000; OR=6.038), husband’s support (p=0.000; OR=4.250), perceived need factors (p=0.000; OR=3.333), as well as cultural beliefs (p=0.004; OR=2.473). Multivariate logistic regression identified maternal knowledge as the most influential factor in delivery place selection, as indicated by the most significant coefficient value (B = –4.204) and statistical significance (p=0.005). Conclusion: Maternal knowledge, attitude, JKN membership, access to healthcare facilities, husband’s support, perceived need, and cultural beliefs are significantly associated with the selection of delivery place. Efforts to increase facility-based delivery coverage in this area should prioritise improving maternal knowledge, followed by strengthening other supporting factors.

Abstract

malnutrition so that the child is too short for his age. Stunting has an impact on children's cognitive decline and productivity in the future. So far, addressing malnutrition has focused on babies from birth to five years old. In fact, the golden period is when the child is still in the womb until the age of two years. Therefore, a paradigm shift in handling malnutrition is needed. Therefore, the government is implementing the Acceleration of Nutrition Improvement Program with the First 1000 Days of Life Movement. SKI data for 2023, Dairi district is in second place with a figure of 32.6% of stunted toddlers out of 439 toddlers and based on the entry of toddler data on the e-PPGBM application in December 2024, the prevalence of stunted toddlers in the Sumbul Community Health Center, Sumbul District is around 13.98% or 258 toddlers. The incidence of stunting is influenced by several factors, namely maternal height, maternal education, compliance with taking blood supplement tablets and smoking. The aim of this research is to determine the determinants of stunting incidents at the Sumbul Community Health Center UPT, Sumbul District, Dairi Regency in 2024. This type of research is cross sectional with research methods. The population in this study were all mothers with toddlers. The number of samples in this study was 70 mothers.  Based on the research results, it was found that there was a relationship between height and stunting (p-value=0.004), there was a relationship between maternal education and stunting (p-value=0.004), there was a relationship between adherence to taking blood supplement tablets and stunting (p-value=0.000) and there was a relationship between smoking and stunting (p-value=0.000). Increasing health promotion to the community about the importance of parenting patterns and fulfilling nutrition in the First 1000 Days of Life, starting from pregnant women until children are 2 years old, as well as outreach about the health of young women and prospective brides.

Abstract

Background: Pulmonary tuberculosis (TB) is a chronic infectious disease that remains a major global and national public health problem, with Indonesia ranking second among countries with the highest TB burden worldwide. Non-adherence to treatment is one of the main barriers to successful TB control, as it may lead to treatment failure, drug resistance, and increased morbidity and mortality. At the Simalingkar Community Health Center (UPT Puskesmas Simalingkar), the specific factors determining treatment adherence among pulmonary TB patients have not been extensively studied. Objective: This study aimed to analyze the determinants of treatment adherence among pulmonary tuberculosis patients at the Simalingkar Community Health Center in 2024. Methods: This study employed an analytical cross-sectional design. The study population consisted of all pulmonary TB patients undergoing treatment at the Simalingkar Community Health Center, with a total sample of 90 respondents selected using a total sampling technique. Data were collected through questionnaires and medical records, and analyzed using univariate, bivariate (Chi-square test), and multivariate (logistic regression) analyses, with a significance level of p < 0.05. Results: A total of 77.8% of patients were adherent to treatment. Bivariate analysis showed significant associations between education (p < 0.001), employment status (p = 0.004), knowledge (p = 0.006), and health care services (p = 0.011) with treatment adherence. Age, sex, and family support were not significantly associated with adherence. Multivariate analysis indicated that the most dominant factor associated with treatment adherence was educational level (OR = 9.004; 95% CI: 2.58–31.49), followed by employment status (OR = 5.59; 95% CI: 1.48–21.12), and health care service support (OR = 3.99; 95% CI: 1.09–14.56). Conclusion: The significant determinants of treatment adherence among pulmonary TB patients at the Simalingkar Community Health Center were educational level, employment status, and health care service support, with education being the most dominant factor. It is recommended that TB control programs place greater emphasis on interventions aimed at improving health literacy, socio-economic support, and the quality and accessibility of health care services to enhance treatment adherence.

Abstract

Medical waste management is an essential component of hospital healthcare services. This study analyzed the implementation of area-based medical waste management in accordance with Regulation of the Minister of Health of the Republic of Indonesia Number 18 of 2020 at Dr. Komang Makes Indonesian Navy Hospital, Belawan. A qualitative descriptive approach was employed. Data were collected through in-depth interviews, observations, and documentation involving eight purposively selected informants comprising key, main, and supporting informants. The management process included waste segregation, transportation, temporary storage, and final treatment in collaboration with third parties. Overall, implementation was consistent with applicable regulations and supported by internal policies, management commitment, infrastructure, budget allocation, and integration with quality improvement, Infection Prevention and Control, and Occupational Health and Safety programs. However, several challenges remained, including inadequate waste containers during periods of high patient volume, limited capacity of the hazardous waste Temporary Storage Facility, and delays in third-party transportation caused by logistical and geographical constraints. The hospital should strengthen source-based segregation, optimize hazardous waste storage capacity, and improve operational supervision, particularly in high-workload units. The study concludes that medical waste management was generally aligned with Regulation of the Minister of Health Number 18 of 2020. Continuous improvement is required to address increasing waste volumes and regional coordination challenges

Abstract

Stunting remains a major public health problem in developing countries. It is a condition of chronic malnutrition characterized by a child’s height being below the standard for his or her age and may lead to impaired physical growth, cognitive development, and increased risks of health problems later in life. This study aimed to analyze the factors associated with stunting prevalence among children aged 24–59 months in Gariuai Village, Baucau Municipality, Timor-Leste. A quantitative study with a cross-sectional design was conducted among a population of 557 children. A sample of 253 children was selected using proportionate stratified random sampling. Data were analyzed using univariate analysis, the Chi-Square test for bivariate analysis, and multiple logistic regression for multivariate analysis. The results revealed that 11.1% of the children were stunted. Exclusive breastfeeding, family income, access to clean water, and toilet facilities were significantly associated with stunting prevalence, whereas maternal education and birth weight history were not significantly associated with stunting. Exclusive breastfeeding was identified as the most dominant factor associated with stunting. These findings indicate that local health interventions should prioritize strengthening exclusive breastfeeding support and counseling, particularly for vulnerable families, while continuing to improve household income, access to clean water, and sanitation facilities.

Abstract

Premarital sexual activity among adolescents is an important public health concern because it may contribute to HIV/AIDS, sexually transmitted infections (STIs), and unintended pregnancy. Maliana, Timor-Leste, has reported increasing HIV/AIDS cases and a high rate of early pregnancy. This study analyzed factors associated with premarital sex prevention behavior among Grade XI students at Dom Martinho da Costa Lopes Senior High School, Maliana, Timor-Leste. A quantitative cross-sectional study was conducted among 234 Grade XI students selected through simple random sampling. Data were analyzed using univariate analysis, bivariate analysis with the Chi-square test, and multivariate logistic regression. Bivariate analysis found significant associations between premarital sex prevention behavior and attitude (p<0.001), family role (p=0.002), peer influence (p<0.001), and social media/internet use (p<0.001). Knowledge (p=0.107), sociocultural factors (p=0.110), and socioeconomic factors (p=0.545) were not significantly associated with prevention behavior. Multivariate analysis identified social media use as the dominant factor associated with premarital sex prevention behavior (OR=4.372). Students with inappropriate social media use were 4.372 times more likely to demonstrate poor premarital sex prevention behavior. Premarital sex prevention behavior among Grade XI students was associated with attitudes, family roles, peer influence, and social media/internet use. Social media use was the strongest associated factor. School-based sexual health education, parental involvement, positive peer support, and digital-health literacy interventions are recommended to strengthen adolescents’ preventive behavior.
