Abstract

Hipertensi merupakan salah satu faktor risiko stroke. Beberapa penelitian menyatakan peningkatan tekanan darah sistol dan diastol pada stroke iskemik menyebabkan keluaran yang buruk pada pasien stroke. Namun berdasarkan penelitian lain, peningkatan tekanan darah sistemik memiliki efek proteksi sehingga menurunkan tingkat keparahan stroke. Penelitian ini bertujuan untuk mengetahui hubungan mean arterial blood pressure (MABP) dengan keluaran pasien stroke trombotik yang diukur dengan skor Delta National Institutes of Health Stroke Scale (NIHSS). Penelitian ini menggunakan subjek pasien stroke yang dirawat di Stroke Unit RS. Dr. Saiful Anwar Malang selama periode April - Juni 2016 dengan jumlah sample 30 pasien. Kriteria inklusi usia pasien >18 tahun, onset stroke <24 jam, dan diagnosis stroke dengan CT scan kepala sebagai gold standard. NIHSS dan tekanan darah diukur 24 jam pertama sejak terdiagnosis stroke dan NIHSS kembali diukur pada hari 14. Delta NIHSS didapatkan dari NIHSS hari ke 14 dikurangi NIHSS masuk. Penelitian ini menggunakan desain peneltian cross sectional. Berdasarkan hasil analisis didapatkan korelasi yang signifikan antara MABP dengan Delta NIHSS (p = 0,025, r = -0,408) dengan arah korelasi negatif. Semakin tinggi nilai MABP maka akan semakin negatif delta NIHSS. Semakin negatif nilai Delta NIHSS menunjukkan semakin baik klinis dari pasien stroke. Kata kunci: delta NIHSS, MABP, sistol, diastol, stroke.

Abstract

Background: The levels of asthma control is an indicator of asthma management outcomes. A complex immunological mechanism influences it. We aimed to investigate the effect of Poly-Phytopharm on the asthma control tests (ACT) score, blood eosinophil levels, and miR-126 relative expressions in asthma patients. Methods: Quasi experimental method is used in 15 stable asthma patients who were not fully controlled at the pulmonary outpatient clinic of Dr. Saiful Anwar General Hospital Malang. Assessment of ACT score, blood eosinophil levels, and miR-126 relative expressions are carried out before and after supplementation of Poly-Phytopharm that consist of Nigella sativa 100 mg, Kleinhovia hospita 100 mg, Curcuma xanthorrhiza 75 mg, and Ophiocephalus striatus 100 mg extract, three times a day, two capsules respectively for 12 weeks. The ACT was scored by the investigator by direct questioning of patients, the blood eosinophil level was measured with blood analysis, and the relative expressions of miR-126 was detected with qPCR. Results: There were significant increase of ACT score (18.07±2.57 to 22.06±1.83, P=0.001), decrease in blood eosinophil levels (653.15±276.15 pg/mL to 460.66±202.04 pg/mL, P=0.038), and enhancement of serum miR-126 relative expressions (1.83±1.89 to 5.89±1.34, P=0.038). Conclusion: The administration of Poly-Phytopharm increases ACT score, decreases blood eosinophil levels and increases serum miR-126 relative expressions in not fully controlled stable asthma patients. Abstract

Knee osteoarthritis (OA) is one of the most common cartilage degenerative disorders that is a major problem because it causes chronic pain and disability. Therefore the method of treatment and prevention of old age is a big challenge. The results of Dextrose Prolotherapy compared to Triamcinolone therapy research are still varied. Objective:  to  compare  the effectiveness  of  Dextrose  Prolotherapy  and  Triamcinolone Acetonide (TA) in the treatment of moderate knee knee OA.True experimental single blind study, at the Neurology Polyclinic Dr. Saiful Anwar General Hospital Malang for 6 months (July-December 2019). Group I; Dextrose Prolotherapy (15%) intraarticularly injected 3 times every 4 weeks and group II; TA one- time intraarticular injection. The parameters studied were The Western Ontario and McMaster  Universities  Osteoarthritis  Index  (WOMAC)  Score,  Numeric  Rating  Scale (NRS), and Range of Motion (ROM), weeks 0, 4, 8, 12 and 24. Forty samples consisted of; the 20 samples of the Dextrose Prolotherapy group consisted of; male: female = 2 (10%): 18 (90%), age 62.4 + 7.28, BMI 25.6 + 3.69 kg / m2, pain duration 22.35 + 20.10 months, history of physiotherapy 40.27 + 20.79 times. Kellgren–Lawrence osteoarthritis severity grade II; 17 (85%), grade III 3 (15%). TA group; male: female = 5 (25%):15 (75%), age 62.5 + 9.02, BMI 28.4 + 5.01 kg/m2, pain duration 15.95 +  16.55  months,  history  of  physiotherapy  34  +  24.87  times.  Kellgren  –  Lawrence osteoarthritis severity grade II; 18 (90%), grade III; 2 (10%). Comparison of NRS at rest Dextrose Prolotherapy vs TA of week 4 (2.60 vs 1.25), week 8 (1.65 vs 1.40), week 12 (0.8 vs 2.05), week 24 (0.75 vs 3.35). Comparison of NRS during activity Dextrose Prolotherapy vs TA week 4 (4.45 vs 3.35), week 8 (3.25 vs 3.55), week 12 (2.55 vs 4.30), week 24 (2.55 vs 5.80). Comparison of ROM flexion Dextrose Prolotherapy vs TA week 4 (127.05° vs 123.60°), week  8  (130.85°  vs  122.95°),  week  12  (130.85°  vs  122.95°),  week  24  (131.05°  vs 122.10°). Comparison of WOMAC score Dextrose Prolotherapy vs TA week 4 (30.05 vs 22.45), week 8 (22.90 vs 23.95), week 12 (19.30 vs 29.05), week 24 (18.95 vs 35.40). Intraarticular Injection Dextrose Prolotherapy is more effective for the long term based on pain scale NRS, WOMAC score and ROM. Whereas short-term TA is more effective than Dextrose Prolotherapy.

Abstract

Model pemberdayaan masyarakat yang inovatif dan sesuai dengan nilai budaya lokal diperlukan dalam pengendalian vektor Dengue. Ketertarikan masyarakat terhadap penghargaan daripada hukuman ditunjukkan dengan antusiamenya pada penghargaan Adipura. Tujuan penelitian ini adalah menjajaki peluang Lomba Kampung Tanggap DBD dengan pengisian Kalender Pemantauan Jentik Mandiri (KPJM), yang dapat menjadi salah satu cara dalam memperbaiki pengetahuan, perilaku, dan kontainer positif jentik. Penelitian quasi experimental nonrandomized with control group ini dilakukan di 15 RT dari 2 kelurahan di wilayah kerja Puskesmas Sukun kota Malang. Sebanyak 52 rumah tangga kontrol dari 7 RT dan 45 rumah tangga intervensi dari 8 RT dibandingkan dalam hal pengetahuan, perilaku, dan persentase kontainer positif jentik. Intervensi yang diberikan berupa pengikutsertaan dalam lomba Kampung Tanggap DBD, dimana RT peserta diberikan sejumlah KPJM yang harus diisi secara mandiri oleh keluarga terpilih seminggu sekali selama 2 bulan.  Uji statistik yang digunakan pada penelitian ini adalah uji Wilcoxon, dan uji Mann Whitney. Hasil menunjukkan perbedaan yang signifikan dalam hal perilaku sebelum-sesudah baik pada kelompok kontrol (p = 0,039) maupun kelompok perlakuan (p = 0,003), sedangkan untuk pengetahuan dan persentase kontainer positif jentik tidak signifikan (p > 0,05). Namun demikian, secara deskriptif angka persentase kontainer positif jentik  yang ditunjukkan pada kelompok perlakuan lebih baik dibandingkan kelompok kontrol. Dapat disimpulkan, Lomba Kampung Tanggap DBD dengan pengisian KPJM berpotensi untuk diangkat sebagai model  pengendalian DBD.

Abstract

In 2016, Indonesia has been launching the “Indonesia Sehat'' program which the strategy used is to strengthen health services through a family approach. According to the 2016 Ministry of Health technical guidelines, the larva inspector, also known as Juru Pemantau Jentik (Jumantik), is involved in Jumantik participating by each family. Their role is the same as 3M plus actions including in the inspection, monitoring, and eradication of mosquito larvae for controlling infectious diseases, especially Dengue Fever. The purpose of these activities was to increase community participation to conduct a “one house one Jumantik'' program. The study was the quasi-experimental post-test only design that was located in Karangbesuki sub-district, Malang City from August until September 2017. One cluster Rukun Warga (RW) from the Karangbesuki sub-district that consisted of 9 Rukun Tetangga (RT) was chosen as the location. The intervention was in collaboration with Mulyorejo Public Health Center (Pusat Kesehatan Masyarakat / Puskesmas) in Malang. The methods used were ovitrap utilization training and focus group discussion (FGD), health promotion and "one-house one-Jumantik'' training program, also the contest to choose the best RT. The results of this activity succeeded in establishing a Jumantik team in each RT although there was one RT that has not yet succeeded in forming a team. Furthermore, for the sustainability of the program, it is hoped that this program could be improved by collaborating with Mulyorejo Puskesmas staff who monitor activities routinely through the integrated service post (Pos Pelayanan Terpadu / Posyandu).

Abstract

Stroke is the most common cause of death and the third leading cause of disability globally. The incidence is increasing and causing various problems including disease complications, high health costs, and low quality of life. This case report discusses the diagnosis and management as a whole using a family medicine approach to the elderly who experience post-stroke immobilization and its complications. Abstrak Stroke adalah penyebab kematian paling umum dan penyebab kecacatan terbanyak ketiga secara global. Insidennya makin meningkat dan menyebabkan berbagai masalah meliputi komplikasi penyakit, biaya kesehatan yang tinggi, dan kualitas hidup yang rendah. Laporan kasus ini membahas diagnosis dan tatalaksana secara menyeluruh dengan pendekatan ilmu kedokteran keluarga pada lansia yang mengalami imobilisasi post stroke beserta komplikasinya.
