Abstract

Introduction: Stroke can cause various kinds of complications. One of the complications of stroke is dysphagia. Therefore, dysphagia management is necessary for stroke patients with dysphagia. Dysphagia management can be done by screening stroke patients with dysphagia and carrying out swallowing therapy with shaker exercise and effortful swallow. Purpose: This case study aims to evaluate the effect of shaker exercise and effortful swallowing on changes in the degree of dysphagia in patients with stroke. Methods: The method used is case study analysis based on nursing intervention with description analysis. The sample used was a dysphagia patient with a stroke. The instrument used to measure the degree of dysphagia is the Gugging Swallowing Screen (GUSS). Discussion: After the shaker exercise and effortful swallow intervention for 5 consecutive days in the morning and evening, there was a change in the degree of dysphagia from initially getting 4 which means a severe degree of dysphagia with a high risk of aspiration to 18 which means mild dysphagia with a low risk of aspiration. Conclusion: Shaker exercise and effortful swallowing can improve the degree of dysphagia in stroke patients.

Abstract

Introduction: Chronic rhinosinusitis is a disease with a high incidence rate, this disease also has an impact on reducing the productivity and quality of life of sufferers. Rhinosinusitis can be caused by multifactorial causes, but most are also caused by inflammation due to viral infections or allergies, resulting in mucosal edema which results in obstruction of the sinus ostium and impaired drainage of secretions. Purpose: The purpose of this study was to determine the effect of nasal irrigation or nasal washing using saline solution on the symptoms complained of by patients and on the condition of airway clearance from patients, so that it can become additional insight for nurses in caring for patients with rhinosinusitis. Methods: This systematic review was conducted according to the PICO model. Keywords in the search for evidence based research in this literature review include: “nasal irrigation” “rhinosinusitis” A database search was conducted in November 2022 using the Google Scholar, PubMed, Science Direct database. Results and discussion: Based on a review of several journals regarding the implementation of nasal irrigation with saline solution, it was found that nasal irrigation using saline solution had a better effect than standard therapy without nasal irrigation with 0.9% NaCl. In addition, it provides good symptom improvement compared to anti- allergy treatment in patients, improves quality of life, and prevents disease recurrence. Conclusion: Based on the results of 10 journal analyzes that have been conducted by the authors, it can be concluded that nasal irrigation using saline solution is proven to be effective in cleaning the nose of accumulated secretions, crusts, and irritant components, thus improving mucosal cleaning. In addition, nasal irrigation has advantages over other techniques such as nasal sprays.

Abstract

Introduction: Hypertension is a condition where humans experience an increase in blood pressure above normal, namely 140/90 mmHg. Based on data from the World Health Organization (WHO), it shows that around 1.28 billion adults suffer from hypertension aged 30-79 years worldwide. Hypertension in the elderly is caused by decreased elasticity of the oral walls, thickening of the heart valves which makes the valves stiff, decreased pumping ability of the heart, decreased elasticity of peripheral blood vessels, increased resistance of peripheral blood vessels, and decreased function of blood vessels. Purpose: Foot soak therapy using warm ginger water aims to improve blood circulation, reduce edema and improve muscle circulation. Methods: The research method used is the case study method. This intervention was given to 2 elderly people with hypertension, intervention was carried out 3 times a week with a duration of 15-20 minutes per meeting. Data analysis used in this research was by using the Wilcoxon test. Discussion: The client's blood pressure after being given the intervention decreased with an average blood pressure for the two elderly people of 143 mmHg (systolic) and 84 mmHg (diastolic). The Wilcoxon test results showed that the Sig (2-tailed) value for systolic and diastolic pressure was 0.027 (<0.05) Conclusion: There is a significant effect of providing foot soak therapy using boiled red ginger water on blood pressure in elderly people with hypertension.

Abstract

Introduction: Diabetes mellitus is also known as a "lifelong disease," but it can be managed. Indonesia ranks fifth in the world for the highest number of sufferers, with 19.5 million sufferers in 2021, and this number is predicted to increase by 2045. Management is needed to improve the quality of life and prevent complications in diabetes mellitus sufferers. Foot exercises are considered a simple treatment option, but many people are unfamiliar with it. Therefore, health education is needed using media that can increase knowledge, such as booklets. Objective: This study aims to determine the effect of implementing health education in the form of diabetic foot exercises using booklets as media on diabetes mellitus sufferers. Methods: This research method used a case report and the DKQ-24 (Diabetes Knowledge Questionnaire), a questionnaire assessing knowledge about diabetic foot exercises. Descriptive data analysis was then conducted by comparing pretest and posttest scores. Discussion: After health education using a booklet during one meeting and demonstration of diabetic foot exercises during three meetings, there was an increase in respondents' knowledge as evidenced by an increase in posttest results. Conclusion: Implementation of health education on diabetic foot exercises using booklets for Mrs. TM can increase the knowledge of diabetes mellitus sufferers.
