The development of toll road infrastructure has a strategic role in supporting economic growth, increasing connectivity between regions, and accelerating the distribution of goods and services. However, this development also poses challenges for Micro, Small, and Medium Enterprises (MSMEs) whose business sustainability depends on customer accessibility. This study aims to analyze the impact of toll road development on the sustainability of MSMEs, with a focus on the economic aspect, especially in the Langkat area. This shows that toll road development has positive impacts, such as increasing market access, distribution efficiency, and increasing asset values around the toll road area. However, there are also negative impacts, such as decreasing MSME turnover due to decreasing local customers, inequality in economic competition with large companies, and increasing living costs around the development area.
