Terapi Relaksasi Otot Progresif untuk Menurunkan Ansietas pada Pasien Skizofrenia dengan Risiko Perilaku Kekerasan

Abstract

Skizofrenia adalah gangguan jiwa berat yang ditandai oleh gangguan pada pola pikir, emosi, dan perilaku. Salah satu gejala yang sering muncul adalah ansietas, yang jika tidak ditangani dapat meningkatkan risiko perilaku kekerasan. Terapi relaksasi otot progresif merupakan intervensi non-farmakologis yang efektif untuk menurunkan kecemasan dan meningkatkan kontrol diri. Tujuan penelitian ini adalah mengetahui pengaruh terapi relaksasi otot progresif terhadap penurunan kecemasan dan peningkatan perilaku kontrol diri pada pasien skizofrenia dengan risiko kekerasan. Penelitian ini menggunakan desain studi kasus dengan pendekatan quasi-eksperimental (one group pre-test and post-test). Tiga pasien skizofrenia di RSJD Dr. Amino Gondohutomo Semarang yang mengalami kecemasan dan berisiko kekerasan diberikan intervensi terapi relaksasi otot progresif selama tiga hari berturut-turut, masing-masing 10–15 menit per sesi. Instrumen GAD-7 digunakan untuk mengukur ansietas dan SLKI untuk menilai kontrol diri. Hasil penelitian menunjukkan Seluruh pasien mengalami penurunan skor GAD-7 dari tingkat kecemasan sedang menjadi ringan, dengan rata-rata penurunan 54,2%. Peningkatan skor kontrol diri juga terjadi pada ketiga pasien, dengan rata-rata peningkatan 25,6%. Secara subjektif, pasien melaporkan rasa lebih tenang, kualitas tidur membaik, dan kemampuan mengendalikan emosi meningkat. Terapi relaksasi otot progresif terbukti efektif dalam menurunkan ansietas dan meningkatkan kontrol diri pada pasien skizofrenia dengan risiko perilaku kekerasan, serta layak dijadikan intervensi rutin dalam praktik keperawatan jiwa. Kata Kunci: Skizofrenia, Ansietas, Kontrol Diri, Perilaku Kekerasan, Terapi Relaksasi Otot Progresif Abstract Schizophrenia is a severe mental disorder characterized by disturbances in thought patterns, emotions, and behavior. One of the symptoms that often appears is anxiety, which if left untreated can increase the risk of violent behavior. Progressive muscle relaxation therapy is an effective non-pharmacological intervention to reduce anxiety and increase self-control. The purpose of this study was to determine the effect of progressive muscle relaxation therapy on reducing anxiety and increasing self-control behavior in schizophrenia patients at risk of violence. This study used a case study design with a quasi-experimental approach (one group pre-test and post-test). Three schizophrenia patients at Dr. Amino Gondohutomo Mental Hospital Semarang who experienced anxiety and were at risk of violence were given progressive muscle relaxation therapy intervention for three consecutive days, each session for 10–15 minutes. The GAD-7 instrument was used to measure anxiety and SLKI to assess self-control. The results showed that all patients experienced a decrease in GAD-7 scores from moderate to mild anxiety levels, with an average decrease of 54.2%. An increase in self-control scores also occurred in all three patients, with an average increase of 25.6%. Subjectively, patients reported feeling calmer, improved sleep quality, and increased ability to control emotions. Progressive muscle relaxation therapy has been shown to be effective in reducing anxiety and increasing self-control in schizophrenia patients at risk for violent behavior, and is worthy of being a routine intervention in psychiatric nursing practice.
