This study aims to improve learning outcomes in Science and Social Science (IPAS) subjects through the implementation of the Problem Based Learning (PBL) model among fifth-grade students at SD Negeri 060912 Medan Denai in the 2024/2025 academic year. The research method used was Classroom Action Research (CAR) conducted in two cycles, each comprising planning, action, observation, and reflection stages. The results indicated that the PBL model effectively enhanced students’ learning outcomes. In the pretest, only 3 students (12.5%) met the minimum passing criteria. After applying the PBL model in the first cycle, the number increased to 9 students (37.5%), and in the second cycle to 21 students (87.5%). Student learning activities and teacher performance also showed significant improvements. Therefore, the Problem Based Learning model is proven to be an effective alternative for improving students’ IPAS learning outcomes.
