Degenerative diseases such as hypertension, diabetes mellitus, osteoarthritis, and cardiovascular disease are common health problems among older adults. These conditions are often associated with aging, physical inactivity, and limited knowledge of healthy lifestyles. The SMART Elderly (Preventing Degenerative Diseases Through Healthy Lifestyle Activities) program was implemented as a community service initiative at the Binjai Elderly Social Care Center to improve older adults' knowledge and healthy lifestyle behaviors. The program included health education, elderly exercise sessions, basic health screenings, balanced nutrition counseling, and assistance in adopting appropriate physical activities. The implementation consisted of four stages: preparation, execution, evaluation, and follow-up. The results indicated increased awareness of degenerative disease prevention, greater participation in regular physical activities, and improved commitment to maintaining healthy lifestyle practices among participants. This program demonstrates that community-based health promotion can effectively empower older adults to enhance their quality of life and reduce the risk of degenerative diseases through sustainable healthy living practices.
