Children with intellectual disabilities (tunagrahita) have limited intellectual function and adaptive behavior that require special attention in implementing clean and healthy living behaviors, including hand hygiene. Data from East Kalimantan Province shows that only 12.6% of children with disabilities aged 5–17 years routinely wash their hands correctly, and a preliminary study at SLB Negeri Samarinda found that no hand hygiene education had been provided and that students with intellectual disabilities had low knowledge regarding this matter. The purpose of this study was to analyze the effectiveness of puzzle game media in improving hand hygiene knowledge and attitudes among children with intellectual disabilities at SLB Negeri Samarinda. The study used a pre-experimental method with a one-group pretest-posttest design. The research sample was taken using a total sampling technique, involving the entire population of junior high school-level (SMPLB) students with intellectual disabilities, totaling 24 respondents. The results showed that puzzle game media was effective in improving respondents' knowledge and attitudes. Good-category knowledge increased from 33.3% (8 respondents) at pretest to 70.9% (17 respondents) at posttest, while positive attitudes increased from 37.5% (9 respondents) to 91.7% (22 respondents). The significance values obtained for both variables (ρ value = 0.000 < α = 0.05) indicate that puzzle game media had a significant effect on improving respondents' knowledge and attitudes. Puzzle game media is effective in improving hand hygiene knowledge and attitudes among children with intellectual disabilities at SLB Negeri Samarinda.
