Adjustment Disorder with Anxiety is an emotional and behavioral response to identifiable psychosocial stressors. This case study aims to comprehensively understand the psychological dynamics, establish a diagnosis, and evaluate the effectiveness of an intervention combining Cognitive Restructuring and Deep Breathing to reduce anxiety in an early adult female within the health service setting at Puskesmas MK Surabaya. A qualitative approach with a single clinical case study design was used. Comprehensive assessments were conducted through clinical interviews, direct observation, and psychological testing using the Sixteen Personality Factors (16PF), Sacks Sentence Completion Test (SSCT), and Depression Anxiety Stress Scales (DASS-42). The intervention was delivered over 5 sessions incorporating Cognitive Restructuring Form (CRF), Deep Breathing exercises, and termination. Initial assessment indicated severe anxiety and stress, alongside moderate depression, triggered by recurring academic demands and high parental expectations. Following the intervention, subjek demonstrated significant reductions in anxiety intensity and physiological tension. Post-intervention DASS-42 scores dropped to normal for depression (score 4) and stress (score 10), and mild for anxiety (score 9). Subjek successfully adopted rational, positive self-talk. In conclusion, the combination of Cognitive Restructuring and Deep Breathing techniques is effective in alleviating symptoms of Adjustment Disorder with Anxiety.
