The implementation of Primary Health Care Integration (Integrasi Layanan Primer/ILP) requires performance management systems that can connect planning, budgeting, implementation, and monitoring. This study analyzes E-SAKIP-related performance documents and the Revised 2025 Performance Agreement were used to support planning and the implementation of ILP at UPTD Puskesmas Cicurug, Sukabumi Regency. A descriptive single-case study with a sequential explanatory mixed-methods design was used. The quantitative stage calculated achievement gaps from monthly service, while the qualitative stage analyzed performance-accountability documents to interpret the observed patterns. The operational 12 basic health service categories. Eight indicators reached or exceeded the 100% target, one approached the target at 99.92%, and five remained below target. The largest gap occurred in TB treatment coverage, which reached 71.28% of the target. Cadre training covered five of eight service areas (62.5%), while digital readiness was assessed at 75%. Leadership and internal cross-program coordination appeared comparatively strong, whereas external communication, community human-resource development, and cross-sector TB coordination required strengthening. The findings do not establish causal effects, but they indicate that E-SAKIP is most useful when performance data are translated into targeted budgeting, cadre development, referral coordination, and routine feedback. Evidence base for linking performance accountability, ILP implementation, and gap-based management.
