The Relationship Between Body Mass Index (BMI) and the Risk of Patellofemoral Pain Syndrome in Young Adult Women

Abstract

Patellofemoral Pain Syndrome (PFPS) is one of the most common musculoskeletal disorders affecting young adult women and is characterized by pain in the anterior region of the knee. This condition can interfere with functional activities such as walking, ascending and descending stairs, squatting, and running. One of the factors presumed to be associated with an increased risk of PFPS is Body Mass Index (BMI). An elevated BMI may increase the mechanical load on the patellofemoral joint, resulting in greater stress on the patellar cartilage and potentially leading to anterior knee pain. This study aims to determine the relationship between Body Mass Index (BMI) and the risk of Patellofemoral Pain Syndrome (PFPS) among young adult women. This study employed a quantitative research design with a cross-sectional approach. A total of 80 young adult women were selected using a purposive sampling technique. BMI was assessed by measuring body weight and height, while knee function was evaluated using the Anterior Knee Pain Scale (AKPS), and PFPS risk factors were assessed using the PFPS Risk Questionnaire. Data were analyzed using univariate analysis and bivariate analysis with the Pearson Correlation test and Chi-Square test at a significance level of ? = 0.05. The results showed that the mean age of the respondents was 20.55 ± 1.81 years, the mean BMI was 23.11 ± 4.90 kg/m², and the mean AKPS score was 71.21 ± 12.54. Pearson Correlation analysis revealed a significant relationship between BMI and AKPS score (r = -0.408; p < 0.001), indicating that a higher BMI was associated with lower knee function. Furthermore, the Chi-Square test demonstrated a significant association between BMI categories and PFPS risk categories, as well as between BMI categories and AKPS-based knee function categories (p < 0.05). Conclusion here was a significant relationship between Body Mass Index and the risk of Patellofemoral Pain Syndrome as well as knee function in young adult women. An increased BMI was associated with decreased knee function and a higher risk of developing PFPS. These findings are expected to provide a basis for promotive and preventive strategies aimed at reducing the risk of Patellofemoral Pain Syndrome through maintaining an optimal Body Mass Index, thereby supporting better musculoskeletal health, particularly knee joint function, in young adult women.
