This research, titled “Analysis of Anchor System Disturbance to Support Crew Safety During the Letgo Process on MV. Isa Express,” examines the causes and impacts of an anchor system failure that occurred on 10 April 2025. During the letgo operation, the windlass brake malfunctioned, causing an uncontrolled anchor chain runaway and creating a serious safety hazard for the crew on the forecastle deck. The study aims to identify the factors contributing to the disturbance, assess its impact on crew safety, and propose corrective and preventive measures. A qualitative descriptive case study approach was employed using data collected through onboard observation, interviews with the Master, deck officers, Bosun, and deck ratings, and documentation analysis. The findings reveal that the incident resulted from three interconnected factors: excessive wear of the windlass brake lining combined with corrosion of the brake lever, a Planned Maintenance System that remained largely reactive, and delayed follow-up on the Bosun’s earlier brake condition report. These deficiencies significantly increased the risk of injury to the deck crew, although a more serious accident was prevented by the prompt response of the duty officer. To improve operational safety, the study recommends implementing a preventive maintenance strategy, conducting brake function tests before every letgo operation, and ensuring timely follow-up of maintenance reports to enhance anchor system reliability and crew safety.
