ABSTRACT Hypertension is a chronic disease that may cause health-related concerns and contribute to nursing problems of anxiety. One non-pharmacological intervention that can be used to help manage anxiety is Butterfly Hug relaxation therapy. This case study aimed to describe the application of Butterfly Hug relaxation therapy in hypertensive patients with anxiety. A descriptive case study design was conducted in the working area of Pagar Agung Public Health Center, Lahat Regency, in March 2025. The subjects were two female pre-older adults with hypertension and moderate anxiety. Data were collected through interviews, observations, physical examinations, blood pressure measurements, and the Hamilton Anxiety Rating Scale (HARS). Butterfly Hug was administered over five sessions as part of the nursing care process. The results showed that HARS scores decreased from 20 to 7 in S1 and from 21 to 6 in S2. The reduction was accompanied by decreased fear and worry, a calmer condition, improved sleep comfort, and increased ability to perform the Butterfly Hug independently.
