Physical activity in early childhood supports growth and active habits, yet home practice may be inconsistent because mothers face limited time, fatigue, and limited practical guidance. This community service program aimed to strengthen mothers’ roles in establishing children’s physical activity routines through family-based practical mentoring. The program involved 31 mothers at TK Pembangunan V YAPIS Heram, Jayapura City. Activities included contextual education, simple movement simulation, approximately 15 minutes of home practice, checklist-based monitoring, and pretest-posttest evaluation using the same core items. Data were analyzed descriptively. The mean knowledge and attitude score increased from 56.3 to 78.5, while good practice increased from 32.3% to 74.2%. During implementation, 83.9% of mothers performed activities with their children, 80.6% provided direct assistance, and 87.1% of children showed enthusiasm. The program supported the translation of understanding into action, increased maternal involvement, encouraged early physical activity routines within families, and offers a practical school-family health education model.
