Type 2 diabetes mellitus is a chronic disease that requires long-term therapy and considerable healthcare resources. This study aimed to determine the antidiabetic therapy patterns and estimate the Cost of Illness (COI) among hospitalized patients with type 2 diabetes mellitus at Sunan Kudus Islamic Hospital. This study used a descriptive observational design with a retrospective approach through the collection of medical records and patient cost data. Descriptive analysis was performed to evaluate patient characteristics, antidiabetic therapy patterns, and direct medical cost components. The results showed that 54 patients met the inclusion criteria. The most frequently used therapy was a combination of Oral Antidiabetic Drugs (OAD) and insulin, administered to 39 patients (72.22%). The total Cost of Illness during one hospitalization episode was Rp106,816,540, with an average cost of Rp1,978,083 per patient. The largest cost components were hospitalization costs (45.10%) and medication costs (44.38%). This study indicates that type 2 diabetes mellitus contributes to a substantial economic burden during hospitalization, mainly influenced by therapy requirements and healthcare services provided.
