Background: Adolescence is a critical period in establishing healthy lifestyle patterns that will determine health status in adulthood. Coronary heart disease can be prevented early through proper nutrition education. Aims: This community service activity aims to improve nutrition literacy and form healthy eating behaviors among SMPN 1 Kampar students through education on the My Plate (Isi Piringku) concept as an effort to prevent heart disease from adolescence. Methods: Implementation methods include observation, preparation of My Plate module, pre-test and post-test questionnaires, balanced nutrition education through interactive counseling, and poster design assignments. The activity was attended by 26 students of SMPN 1 Kampar. Results: Pre-test results showed an average student knowledge score of 51.30 (±17.89), which increased to 55.25 (±9.74) in the post-test. The decrease in standard deviation indicates a more homogeneous distribution of knowledge among students following the intervention. Post-test improvements were observed across key indicators, particularly in understanding balanced nutrition principles, identifying protein sources, and knowing daily water intake recommendations. Conclusion: This activity successfully increased students' knowledge about My Plate principles and is expected to form healthy eating behaviors to prevent heart disease in the future.
