Food handler hygiene is essential for preventing foodborne illness. This cross-sectional study examined associations of knowledge, attitudes, and facility availability with hygiene practices among 93 food handlers at 39 restaurants in South Aceh Regency in August 2025. Data were collected using structured questionnaires and observations and analyzed using an exact test for categorical data. Good knowledge was recorded in 52.7% of respondents, positive attitudes in 76.3%, adequate facilities in 62.4%, and good hygiene practices in 50.5%. The reported bivariate analyses identified associations between hygiene practices and knowledge (p < 0.001), attitudes (p = 0.002), and facility availability (p = 0.011). Because of the cross-sectional design, these results do not establish causation. Continuous hygiene education, accessible handwashing facilities, and supervision are recommended.
