Abstract

The majority of alternative antihypertensive therapies require pharmacoeconomic studies, particularly Cost Effectiveness Analysis, which is useful in balancing patients’ expenditure by determining alternative treatments that represented the most plausible health outcomes with a more reasonable budget. The objective of this study was to compare the cost effectiveness using antihypertensive fixed dose combinations (FDC) of Valsartan-Amlodipine + Furosemide and FDC of Valsartan-HCT + Amlodipin. This research applied the descriptive analytical cross-sectional method and conducted data collection retrospectively from the medical record of hypertensive patients. Meanwhile, the medication cost breakdown was obtained from Outpatient Financial Department of a Type B Hospital in south Jakarta for the period of January - June 2018. Total sample were 74 patients, consisted of 37 patients who used  FDC of Valsartan-Amlodipin + Furosemide, and 37 patients who used FDC of Valsartan-HCT + Amlodipin. The parameters of this research were direct medication cost (consisted of examination cost, laboratory cost and medicines cost), indirect medication cost (consisted accommodation cost and lost productivity cost), while the effectiveness used the MAP average (Mean Arterial Pressure). The result of this research showed that the biggest medication effectiveness to lower the blood pressure is FDC of  Valsartan-Amlodipin + Furosemide, with 32  patients had the average MAP 101,29 mmHg, while the FDC of Valsartan-HCT + Amlodipin with 29 patients had the average MAP 103,59 mmHg. The cost effectiveness based on ACER value of  FDC of Valsartan –HCT + Amlodipin and FDC Valsartan-HCT + Amlodipin sequentially is Rp 3.922.040/MAP and Rp 4.458.034/MAP. In conclusion, the FDC of Valsartan-Amlodipin + Furosemide  was more cost-effective.

Abstract

The level of satisfaction achievement on the services provided to patients, both the quality of health services and the health care system, will be the first choice for patients to visit the hospital. The objective of this study was to determine inpatients satisfaction level in the internal diseases room on drug services at Pharmacy Installation Hospital of X Tahuna. This research was a descriptive research with crosssectional design. The research instrument was a form of a questionnaire of hope and reality which was adopted from Nangaro in 2019. The validity and reliability test of the questionnaire was conducted on 30 respondents and data analysis using index of total quality service. There are 115 people who have met the inclusion criteria, such as being willing to be a respondent, being able to read and write, aged ≥ 18 years old, and were currently undergoing treatment in the interna room and have received service at X Tahuna Hospital. Analysis of satisfaction data was using the Weighted Servqual model. The results showed an overall satisfaction index of -0.20, while the satisfaction index for each dimension was as follows: -0.29 for the tangibles dimension, -0.278 emphaty dimensions, -0.17 reliability dimensions, -0.274 dimensions responsiveness, and - 0.32 for the assurance dimension. The overall satisfaction index as well as each dimension shows a negative value, which means that the patient is not satisfied with the drug services at X Tahuna Hospital Pharmacy Installation. The Hospital must re-evaluate the number of human resources (pharmacist-patient ratio), the flow of drug services, as well as the completeness of the available facilities.

Abstract

Geriatric patients have more comorbidities and often receive polypharmacy prescriptions. The prevalence of inappropriate drug use in geriatric patients is in the range of 11.5% - 62.5%. The consequence of polypharmacy is Adverse Drug Reactions (ADR). An effort to reduce the occurrence of medication errors in geriatric patients is the STOPP and START criteria. This study aimed to identify PIMs (Potentially Inappropriate Medications) and PPO (Potentially Prescribing Omission) in geriatric patients according to STOPP / START criteria at Gatot Soebroto Army Hospital and the Jakarta Cempaka Putih Islamic Hospital. This study used a descriptive-analytical method with a cross-sectional design and a prospective data collection method based on analysis of medical records, inpatient installation, and pharmacy installation at Gatot Soebroto Army Hospital in Jakarta on October 2017 and Jakarta Cempaka Putih Islamic Hospital in January 2019. The tool used to collect data in this research was the STOPP START toolkit supporting medication review version 1 2016. The results show that of 85 hospitalized geriatric patients at Gatot Soebroto Army Hospital with the use of 579 drugs, PIMs were present in 8.24% of patients and the prevalence of PPO was 5.88%. Meanwhile, out of 124 hospitalized geriatric patients in Jakarta Cempaka Putih Islamic Hospital with the use of 945 drugs, PIMs were present in 35.48% of patients and the prevalence of PPO was 6.45%. Polypharmacy found to be an independent predictor for PIMs incident.

Abstract

The EuroQuality of life-5 dimension 5 level (EQ-5D-5L) and Short Form-6 Dimension (SF-6D) are the most widely used generic instruments for assessing utility as an outcome parameter in pharmacoeconomic studies. Different instruments cause different utility values and influence the decision making. The purpose of this study was to compare the utility score index measured using EuroQuality of life-5 dimension 5 levels (EQ-5D-5L) and Short Form-6 Dimension (SF-6D) in general population. The study was conducted cross sectional design in general population in the Yogyakarta. EQ-5D-5L and SF-6D questionnaires were given to 470 respondents to measure the utility score index. Differences of utility index scores based on demographic characteristics were analyzed using Mann Whitney and Kruskal Wallis test, while correlations between EQ-5D and SF-6D domains with utility scores were analyzed using Pearson Correlation. Limits of agreement were described by Bland and Altman Plot. The results of the study showed the utility scores measured using EQ-5D-5L, EQ-VAS, and SF-6D were 0.944 (SD = 0.093), 0.829 (SD = 0.089) and 0.915 (SD = 0.081) respectively. EQ-5D-5L and EQ-VAS can distinguish utility values based on age. There is a strong correlation (> 0.700) between the domain of mobility and pain with the EQ-5D-5L utility index, and the domain of physical function and pain with the SF-6D utility index. Ceiling effect of EQ-5D-5L (59%) is greater than SF-6D (22.4%). The Bland and Altman plot shows the difference in utility scores of  EQ-5D-5L with SF-6D, SF-6D with EQ-VAS, and EQ-5D-5L with VAS, respectively at the limit of agreement 92.98%, 96 , 38%, and 95.74%. EQ-5D-5L can distinguish utility scores based on age, but showed higher ceiling effect. Both EQ-5D-5L and SF-6D showed a strong correlation between the domain of mobility (physical function) and pain with the utility score.

Abstract

The increasing number of patients admited to primary healthcare (PHC) in the JKN era demands the readiness of PHC’s staff, especially pharmaceutical staff, to serve the community. Knowledge is a major necessity in collaborating with other health profesional to improve the quality of health services. Pharmaceutical staff who lack knowledge needs special training to improve their competences in drug management and clinical pharmacy services. This study aims to determine the effect of pharmacy service training on the knowledge of drug manager in PHC’s of Ogan Komering Ilir district. This research is a quasi-experimental design research with pre and post analysis approach in the intervention group and using the control group as a comparison. Subjects in the study were drug managers in the PHC’s of Ogan Komering Ilir district that met the criteria. The intervention was carried out by providing training to drug manager and providing pharmacy service manuals. The training material provided was based on the Pharmaceutical Services Training Curriculum compiled by Ministry of Health. Another instrument used was a validated questionnaire to measure the knowledge score of drug managers before and after training. A total of 13 drug managers participated in this study. After the training, the knowledge score of drug manager in the intervention group increased from 9.83 ± 2.401 to 12.67 ± 2.333 with value of p = 0.013. The majority of intervention group drug managers experienced an increase in knowledge both from the aspect of drug management and clinical pharmacy service aspects. This study concludes that training of drug managers has an effect on increasing knowledge of aspects of drug management and clinical pharmacy services.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a lifelong metabolic disorder characterized by hyperglycemia, which requires continuous therapy. Oral antidiabetic drugs (OAD) become the main line in T2DM therapy. There are several factors influencing drug compliance and consumption, one of them is perception. Perception is a process of processing sensory information to form a meaning that will make T2DM patient take a decision to take the medication regularly and continously. The purpose of the study is to determine the perception of T2DM patients about the use of OAD in the Endocrine Polyclinic, Dr. Hasan Sadikin General Hospital, Bandung. This study uses categorical descriptive method with cross-sectional design. Data is collected using a questionnaire that has been through the process of validity and reliability tests. Sampling was done by convenience sampling method to 46 respondents. The variables in this study are respondents' characteristics and perceptions, which is grouped into four categories. The data was processed using the Rasch Model to be categorized into positive and negative perceptions based on the mean logit respondent value. The results showed positive perception of treatment benefits is 73.91%, positive perception of drug consumption procedures is 78.26%, positive perception of treatment side effects is 82.61%, and positive perception of drug consumption encouragement is 67.39%. Overall, the perception of T2DM patients about the use of OAD in RSHS is a positive perception. This is a good foundation for patients for doing the therapy regularly and continuously.

Abstract

Postoperative pain was suffered by 80% of the total patients. One of the many surgical procedures performed is orthopedics. Ketorolac and fentanyl are often used for postoperative orthopedic pain. Appropriate postoperative pain management with adequate analgesic administration will accelerate pain reduction and shorten the patient's hospital stay. This study aims to determine the difference in the effect of continuous fentanyl and intravenous ketorolac on pain targets achievement and Length of Stay (LOS) of patients after orthopedic surgery. This study is a prospective cohort study, involving inpatients after orthopedic surgery in Dr. Sardjito Yogyakarta in July-October 2019. The independent variable is continuous fentanyl and intravenous ketorolac, while the dependent variable is the achievement of pain targets and LOS. The analysis was conducted by Fisher's exact test, Mann Whitney's test, and multivariate analysis to determine the dominance of variables on the research outcomes. There were 59 patients after orthopedic surgery in this study. A total of 34 patients in the continuous fentanyil group and 25 patients in the intravenous ketorolac group. There was no difference in the effect of continuous fentanyl administration and intravenous ketorolac on pain targets achievement (P = 0.711), while there were differences in the effect of continuous fentanyl administration and intravenous ketorolac on the average LOS of patients in the hospital (P = 0,000). Continuous fentanyl and intravenous ketorolac can both produce the achievement of adequate postoperative orthopedic pain targets, but the mean LOS becomes shorter in patients with intravenous ketorolac.
