Abstract

Background: The increase in the number of home births highlights the importance of understanding the factors that influence access to health services for childbirth. Purpose: The aim of this scoping review was to identify the best evidence on factors influencing access to health services and choice to give birth at home. Methods: The database was searched from 2019 to 2024 from various databases, including PubMed, Science Direct, and Wiley. As well as using search engines such as Google Scholar and Research Rabbit and selecting them using Mendeley's help. Critical appraisal and data charts were adopted from the JBI checklist, including cross-sectional, case-control, and qualitative exploratory JBI. From the 1598 articles identified, only eleven met the inclusion criteria. Results: The results of this review identified three themes, including factors influencing access to health services for home birth, the impact of limitations and the main barriers faced by mothers. Conclusion: Access to health services for mothers who give birth at home is limited by various factors, including but not limited to geography, economy, education, socio-culture, and occupation, among others. Improving access to maternal health services should focus on more affordable facilities, better maternal and family health education, and socio-cultural changes that support delivery in health facilities. As a result, these barriers reduce the utilization of health facilities, which increases the risk of complications and maternal mortality.

Abstract

Self-management in patients with breast cancer plays an essential role in improving the quality of life of breast cancer patients. Self-management behaviour changes along with the treatment period and varies in patients with different demographic and psychosocial characteristics. However, systematic identification of self-management factors in this population is still lacking. This review aimed to identify factors that influence self-management in patients with breast cancer. This study used a scoping review. Literature was searched from three databases: EbscoHost: Medline Ultimate, Pubmed, and Scopus, and additional literature was identified using Google Scholar, using the keywords "Self-management OR self-care AND breast cancer OR breast neoplasm OR breast tumours AND factors or causes or influences or reasons or determinants or predictors or contributors." The analysis was carried out thematically with a descriptive exploratory approach. A total of 11 articles were analyzed in this review. Several factors influence self-management in patients with breast cancer, including chemotherapy status, duration of illness, patient's personal experience, age, education level, social support, anxiety, self-efficacy, self-compassion, understanding of the condition, adaptation and acceptance of life with lymphedema. This findings underscore the importance of developing comprehensive interventions that address these diverse factors to enhance self-management. These interventions should include patient education, psychological support, and social assistance to improve patients' ability to manage their condition. A holistic approach, incorporating medical care, emotional well-being, and social support, is essential to improving the overall quality of life for breast cancer patients.

Abstract

Digital National Early Warning Score (NEWS) is a system designed to detect early clinical deterioration in patients by monitoring vital parameters using digital technology. The implementation of Digital NEWS in hospitals is increasingly gaining attention because of its ability to improve patient safety, efficiency in clinical decision-making, and reduce the workload of health workers. However, although many benefits have been identified, implementing Digital NEWS is open to various challenges and obstacles. This study aimed to explore the benefits, factors, and obstacles faced in implementing digital NEWS in Hospitals. This scoping review used the Arksey and O'Malley framework following the PRISMA for Scoping Review. Literature searches used three main databases: EbscoHost: Medline Ultimate, Pubmed, and Scopus, and also one search engine, Google Scholar. Ten relevant articles were analyzed in this study. Implementation of digital NEWS can improve early detection of deteriorating patient conditions, reduce medical response time, and improve communication between health teams. However, the success of its implementation is influenced by several factors, such as the availability of technological infrastructure, adequate training, and technological literacy among health staff. The main challenges include lack of resources, alarm fatigue, and cultural resistance to technological change. This review concludes that implementing digital NEWS has many benefits, but health workers still face many challenges and barriers. A comprehensive approach is needed to optimize the benefits of digital NEWS implementation, including investment in infrastructure, training, and managerial support.
