Indonesian Journal of Cardiology (IJC) is a peer-reviewed and open-access journal established by Indonesian Heart Association (IHA)/Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) [www.inaheart.org] on the year 1979. This journal is published to meet the needs of physicians and other health professionals for scientific articles in the cardiovascular field. All articles (research, case report, review article, and others) should be original and has never been published in any magazine/journal. Prior to publication, every manuscript will be subjected to double-blind review by peer-reviewers. We consider articles on all aspects of the cardiovascular system including clinical, translational, epidemiological, and basic studies. Abstract

Strategi revaskularisasi pada penyakit arteri koroner (PAK) terus menjadi bahan debat di kalangan kardiologi. Berbagai uji klinis yang telah dilakukan menunjukkan hasil yang bervariasi dan harus ditelaah secara detil karena persoalan strategi revaskularisasi menjadi masalah yang tidak sederhana tetapi harus memperhatikan subset pasien yang befvariasi untuk mendapatkan hasil yang terbaik. Prinsip dasar yang harus tetap dijaga oleh para kardiolog adalah keputusan medis harus berorientasi pada pasien. Patient first!Perbandingan secara tidak langsung antara bedah pintas arteri koroner (BPAK/CABG) dengan intervensi koroner perkutan (IKP/PCI) menunjukkan dengan CABG keperluan pengulangan revaskularisasi lebih sedikit. Masalah yang selalu dipertanyakan dalam membandingkan kedua teknik revaskularisasi itu adalah pengaruh perkembangan teknologi stent terhadap luaran pasien. Berbeda dengan teknik CABG relatif tidak banyak berubah yaitu terdiri dari teknik on- atau off-pump, sedangkan teknologi stent berkembang cepat dan mungkin akan memberikan luaran yang berbeda karena stent generasi terakhir dipercaya memiliki patency rate yang lebih baik. Teknologi stent berkembang dari bare-metal stent ke drug-eluting stent, kemudian drug eluting stent juga berevolusi mulai dari generasi pertama yang memakai obat sirolimus atau paclitaxel ke generasi kedua yang memakai obat everolimus atau zotarolimus. Demikian juga farmako-teknologi polymer yang berkembang ke arah polymer-free stent. Dalam hal platform stent dimulai dari stainless steel, cobalt chromium, platinum dan terakhir memakai polylactyic acid yang dapat diserap.

Abstract

Objective: To develop a prognostic model for the thirty-day mortality risk after adult heart transplantation.Methods: In this report we developed a prediction model for the 30-day mortality risk after adult heart transplantation. Logistic regression analysis was used to develop the model in 1,262 adult patients undergoing primary heart transplantation. We evaluated the accuracy of the prediction model; the agreement between the predicted probability and the observed mortality (calibration); and the ability of the model to correctly discriminate between the discordant survival pairs (discrimination). The internal validity of the prediction model was evaluated using the bootstrapping procedures.Results: Recipients’ age and sex, pre-transplant diagnosis, transplant status, waiting time, cardiopulmonary bypass time, donors’ age and sex, donor-recipient mismatch for BMI and blood type were independent predictors for 30-day mortality risk after adult heart transplantation. The model showed a good calibration and reasonable discrimination (the corrected area under the receiver operating characteristic curve was 0.71). The internal validity of the prediction model was acceptable. For practical use, we converted the prediction model to score chart.Conclusion: The accuracy and the validity of the prediction model were acceptable. This easy-to-use instrument for predicting the 30-day mortality risk after adult heart transplantation would benefit decision-making by classifying recipients according to their mortality risk and allowing optimal allocation of a donor to a recipient for heart transplantation.

Abstract

Background: Silent myocardial ischemia (SMI) defined asmyocardial ischemiawithout anginal pain. SMI is frequently found in diabetes due to autonomic neuropathy, but in hypertension the involvement of autonomic neuropathy is still not known. Heart rate recovery(HRR) has been known as manifestation of autonomic neuropathy.The simple measurement of HRR is potential as screening tools for autonomic neuropathy. Nevertheless, SMI and delayed HRR are known as independent predictors for cardiovasluar mortality, hence emphasize the importance for early diagnosis. In diabetes, albuminuria has been proven to be an independent predictor for SMI and delayed HRR, but in hypertensive patients without diabetes the correlation is still unclear.Objectives: This study is aimed to determine the incidence of SMI, delayed HRR, and albuminuria in hypertensive patients without diabetes, the correlation between albuminuria and SMI and delayed HRR.Methods: This is an observational study with cross-sectional approach. Fourty consecutive hypertensive without diabetes, history of chest pain, and known CAD. They underwent treadmill exercise testing with collection of spot urine before exercise to measure albumin urine to creatinine ratio (ACR). SMI and HRR to 3 minutes were then recorded.Results: SMI was diagnosed in 15% patients. The incidence of delayed HRR in the first, second and third minute after peak exercise were 60%,80%, and52.5% respectively. Albuminuria was significantly associated with SMI (OR 13.889 (1.423 – 135.544), p = 0.014). There were no signification correlation between albuminuria and delayed HRRConclusion: Albuminuria is a good predictor for SMI in hypertensive patients.

Abstract

Background : Heart disease is the leading cause of death for both men and women, but heart disease is preventable and controllable. Ganoderma lucidum is widely used as traditional medicine for centuries particularly in China, Japan, and Korea. Previous study showed antioxidative activity of polysaccharide peptide (PsP) from Genoderma lucidum.Objective : This study was aimed to evaluate anti-inflammatory and antioxidant effect of polysaccharide peptide (PsP) from Ganoderma lucidum in atherosclerotic rats.Methods : The atherosclerotic rats were randomly divided into four groups (5 rats each group) : atherosclerotic model with high-fat diet, low dose PsP treated group (50 mg/kgBW), medium dose PsP treated group (150 mg/kgBW), high dose PsP treated group (300 mg/kgBW), with normal mice used as a control group. Parameters measured were the level of MDA, SOD, IL - 6 , IL - 10, hsCRP, TNF - ?, lipid profile and foam cell.Results : After PsP therapy for 5 weeks, the levels of MDA (p=0.01), hsCRP (p=0.018) in rats model of atherosclerosis decrease significantly. PsSP can reduce levels of IL - 6 (p=0.933) and increase levels of SOD (p=0.28) descriptively at PsP doses 150 mg/kgBW. While the levels of TNF-? (p=0.894) and IL-10 (p=0.98) was not affected by administration of PsP. PsP improve the lipid profile by increasing HDL (p=0.002) and lowering total cholesterol (p=0.04). The formation of foam cells (p=0.024) as a marker of atherogenesis significantly decreased by administration of PsP .Conclusion : PSP can be useful to reduce inflammatory processes and oxidative stress to prevent the process of atherogenesis.

Abstract

Cardiac myxomas are the most common primary cardiac tumors. Myxoma are more common in women. Clinical manifestations can mimic many cardiac and noncardiac conditions. Transthoracic echocardiography (TTE) is the gold standard method in the diagnosis of cardiac myxoma. The management of cardiac myxoma are medical therapy for the treatment of associated conditions and surgical removal as the definitive treatment.

Abstract

Ondasentron is selective serotonin 5-HT3 receptor antagonist. It is widely used as the drug of choice for preventing nausea and vomiting associated with chemotheraphy and post-surgery. This drug become more popular because it’s superior efficacy and minimal side effect. Although cardiovascular adverse effect caused by ondansentron administration is rare, some cardiovascular events have been reported, from atrial fibrillation to fatal ventricular tachycardia. Abstract

The incidence rate of ST-Elevation acute Myocardial Infarct (STEMI) keeps increasing. Currently, myocardial reperfusion, either with thrombolytic or Primary Percutaneous Coronary Intervention (PPCI), is the main treatment. However, mortality rate of STEMI patients undergoing PPCI is still high which is 6% to 14%. The main purpose of PPCI is to restore the patency of infarcted epicardial artery and to establish microvascular reperfusion as soon as possible so that it can reduce enlargement of irreversible necrotic myocardial tissue. In the other hand, restored patency of infarcted epicardial artery does not indicate sufficient reperfusion to microvascular. Such phenomenon is called no-reflow or microvascular obstruction (MVO). Specific pathogenic component of MVO is reperfusion injury. Reperfusion injury involves some mediator and mechanisms, one of which is the process of thrombolysis and angioplasty. Mediator that play a certain role is inflammatory neutrophil which is activated and accumulated. Trimetazidine is an anti-ischemic agent which selectively inhibits end stage activation of oxidation pathway of 3-ketoacyl coenzyme Athiolase. Abstract

A lot of hypertensive patients require two or more antihypertensive drugs to control and maintain normal blood pressure. Combination of antihypertension is recommended either as first or second line treatment. Among available antihypertension medications, combination of renin-angiotensinaldosteron (RAAS) inhibitor with calcium channel blocker or diuretics are more recommended as they have sinergistic clinical and biomolecular effects.Furthermore, single pill dosage is associated with higher patients compliants and lower faults incidence in medication. Severeal fixed-dose combinations are currently available in the market to give easier hypertension control and patients compliants. Abstract

Congenital heart disease (CHD) occurs in about 6 to 8 of 1000 live births with the increasing prevalence can be attributed to major improvements in diagnosis and treatment. Imaging has an important role in diagnosis of CHD. It outlines anatomy and physiology, helps to refine management, evaluates the consequences of interventions and helps guide prognosis. However, no single available imaging modality fulfills these roles for all patients and diseases. Therefore, assessment for CHD must involve multimodalities that can be used in a complementary fashion, and that together are sensitive, accurate, reproducible, and cost effective, whilst minimizing harm.In previous years, the diagnosis and the treatment of congenital malformations have often depended on cardiac catheterization. In many institutions, cardiac catheterization still remains the gold standard against which other modalities are measured. In the past decade, however, imaging methodologies have increasingly shifted toward the use of less invasive and noninvasive techniques. Although echocardiography is the most commonly used imaging modality for diagnosis and follow-up of subjects with CHD, the evolution of cardiovascular magnetic resonance (MR) imaging and increasingly computed tomography (CT) does offer new ways to visualize the heart and the great vessels.
