Indonesian Journal of Cardiology (IJC) is a peer-reviewed and open-access journal established by Indonesian Heart Association (IHA)/Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) [www.inaheart.org] on the year 1979. This journal is published to meet the needs of physicians and other health professionals for scientific articles in the cardiovascular field. All articles (research, case report, review article, and others) should be original and has never been published in any magazine/journal. Prior to publication, every manuscript will be subjected to double-blind review by peer-reviewers. We consider articles on all aspects of the cardiovascular system including clinical, translational, epidemiological, and basic studies. Abstract

Cerita dimulai dengan terbitnya Sistem Jaminan Sosial Nasional (SJSN) yang merupakan salah satu bentuk perlindungan sosial untuk menjamin seluruh rakyat agar dapat memenuhi kebutuhan dasar hidup yang layak.1 Sistem ini tentu saja ditunggu oleh berbagai pihak yang mengharapkan peningkatan kesejahteraan masyarakat dengan jaminan sosial dari negara.

Abstract

Background: The clinical implications of precordial ST segment depression (PSTD) during acute inferior myocardial infarction has been an area of debate, and still under investigation with conflicting results. Based on previous studies, the presence of PSTD defines a high risk subset of patients with acute inferior myocardial infarction due to a more extensive myocardial ischemia that lead to a higher incidence of major adverse cardiovascular events (MACE). Despite of these results, others still considered this ECG finding as a benign electrical phenomenon. The aim of this study is to compare the incidence of in-hospital MACE in patients of acute inferior myocardial infarction with or without PSTD and to know whether PSTD can be used as a predictor of in-hospital MACE in acute inferior myocardial infarction.Methods: A total of 96 acute inferior myocardial infarction patients admitted from December 2013-2015 at Cardiology Department of Haji Adam Malik General Hospital were retrospectively analyzed. Patients were divided into two groups based on the presence of PSTD on admission ECG. Bivariate and multivariate analyses were performed to study the association between PSTD and in-hospital MACE, p value<0.05 was considered statistically significant.Results: The bivariate analysis showed that in-hospital MACE was significantly higher in patients of acute inferior myocardial infarction with PSTD than without PSTD (92% vs 8%, p<0.001). On multiple logistic regression analysis, patients of acute inferior myocardial infarction with PSTD have a 5.4 fold increased risk of in-hospital MACE than patients without PSTD (OR 5.480; 95% CI 1.759-17.067, p=0.003).Conclusion: The presence of precordial ST segment depression on admission ECG in acute inferior myocardial infarction patients was associated with a higher in-hospital MACE and was an independent predictor of in-hospital MACE.

Abstract

Background: In patients with significant coronary heart disease (CHD), increased preload and afterload during a squat can cause wall motion abnormalities (WMA) which can be detected on echocardiography. This study was conducted to determine the diagnostic value of stress echocardiography squatting as a non-invasive examination of a relatively simple, inexpensive, and safe in the detection of coronary artery stenosis in stable CHD and unstable angina patients.Methods: This study was a cross-sectional design. The subjects were all patients with stable CHD and unstable angina whom were treated in Instalasi Pusat Jantung Rumah Sakit Dr. M. Djamil Padang from May to July 2016. Subjects underwent squatting stress echocardiography procedures followed by coronary angiography. Diagnostic test was used to determine the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of squatting stress echocardiography.Results: The sensitivity, specificity, and accuracy of squatting echocardiography for diagnosis of CAD were 90.3%, 88.9% and 89.7%, respectively.Conclusion: Squatting stress echocardiography can be proposed as a non-invasive examination which is relatively simple, inexpensive and safe to detect coronary artery stenosis on patients with stable CHD and unstable CHD.

Abstract

Surgical coronary revascularization can affect the quality of life and psychosocial functions, including the return of sexual activity. A 43-year-old male patient underwent four graft coronary artery bypass (CABG) surgery at National Cardiovascular Center Harapan Kita. The second phase of rehabilitation results showed patient’s functional capacity is 9.75 METs and ischemic negative response. Before surgery, patient often experienced chest pain during intercourse. The patient must consume sublingual isosorbide dinitrate (ISDN). Currently the patient is no longer feel chest pain during intercourse, but he was still afraid to do intercourse and reduce the activity frequency.Return to sexual activity after undergoing CABG surgery is a common concern. Cardiac rehabilitation program and sexual counseling can help patients to return to their sexual function and activity.

Abstract

Low-voltage on the surface electrocardiogram (ECG) is classically defined as peak-to-peak QRS voltage less than 5 mm in all limb leads and less than 10 mm in all precordial leads. There are many causes of low QRS voltage (LQRSV), and they can be differentiated into those due to the deficient heart’s generated potentials (cardiac causes) and those due to the attenuating influences of the pericardial space and pericardium, or the passive body volume conductor, enveloping the heart (extracardiac causes). In some patients, LQRSV voltage may only represent a normal variant. The ECG challenge for this issue is to examine the physiological, anatomical and electrical equipment problems of low-voltage, or low-amplitude, ECG and to suggest methods for troubleshooting the low-voltage ECG to ensure reliable cardiac monitoring.

Abstract

Globally, coronary heart disease (CHD) is a leading cause of mortality and morbidity, especially in developing countries. Both marine and plants sources of omega-3 fatty acids has been shown to be beneficial in reducing CHD mortality. Beside of anti-arrhythmic effects, omega-3 has been shown anti-thrombotic and anti-atherosclerotic effect. In 2011, US population spent about 25 billion US dollar for omega-3 supplement; it is projected to be 35 billion US dollar in the year 2016. Several reviews on randomized controlled trial conclude that omega-3 fatty acids reduced plasma triglyceride level consistently in a dose-dependent fashion. Other beneficial effects on blood pressure, endothelial function and high density lipoprotein (HDL) level participate in lowering CHD mortality. American Heart Association (AHA) recommended routine fish consumption as secondary prevention in patient at risk. Omega-3 fatty acids role in primary prevention is inconclusive and need further investigation regarding cost-benefit and bleeding risk.
