Abstract

Patients with hyperlipidaemia is at increased risk of developing atherosclerotic cardiovascular disease (ASCVD). The use of appropriate statin therapy is important to obtain optimal health outcomes. However, there was limited information regarding the risk of ASCVD among Indonesian hyperlipidaemia population and the appropriateness of statin therapy given to these patients. This study aimed to assess the ASCVD risks among the patients and evaluate the use of statin therapy at hospitalized hyperlidaemia patients. This cross sectional study used medical records as data source. This study was conducted at one hospital in Pontianak, Indonesia during Januari-June 2017. We included 50 subjects. The 10-year risk of ASCVD was assessed using ASCVD pooled cohort risk assessment equation (PCRAE). The results showed that 66% of the included subjects had a relatively high risk of developing ASCVD. Subjects with high risk of ASCVD received inappropriate statin therapy. High intensity statin should be given to these patients. In conclusion, large proportion of the subjects were at high risk of developing ASCVD. Nevertheless, inappropriate statin therapy were given to these patients. Improvement in medication practice should be done to prevent complication and increase quality of health outcomes.Keywords: ASCVD, dyslipidemia, statin

Abstract

Phaleria macrocarpa (Scheff.) Boerl is originated from Papua Island, Indonesia. There have been only limited attempts to explore the biological properties of this plant in relation to their medicinal use. This study aimed to examine antioxidant activity of P. macrocarpa fruit. Extraction of pericarp and mesocarp of P. macrocarpa were performed using soxhlet method with ethyl acetate as the solvent. Antioxidant activity was characterized in various in vitro model systems, including DPPH and ferric reducing antioxidant assay. We found that the highest amount of phenolic compounds and flavonoids were found in the pericarp (58.3±0.07 mg/g DW and 127.8±1.08 mg/g DW, respectively). The results showed that pericarp had higher antioxidant activity (IC50= 122.4±1.14 µg/ml) compared to mesocarp (IC50=175.48 ±1.75 µg/ml). In conclusion, the result of this study indicated the possible application of P. macrocarpa as a source of natural antioxidant compound.Keywords: antioxidant, Phaleria macrocarpa, phenolic compounds, flavonoids

Abstract

Pharmacists play a major role in educating patients regarding drug therapy, in order to achieve optimal health outcomes. The aim of this study was to investigate the effect of pharmaceutical counseling on quality of life (QoL) of hypertensive patients with renal impairment at one of public hospital in Bandung, Indonesia. This study used a prospective experimental design with pretest-posttest design for 2 months. Inclusion criteria in this study were hypertensive patients with renal impairment who received antihypertensive drugs, aged >18 years, and signed the informed consent. Exclusion criteria were patients who were not able to fill in the questionnaire, patients with end stage renal diseases, diabetes, and pregnant or lactating patients. Pharmaceutical counseling was given during 2 months of the therapy. Pharmaceutical counseling consisted of education on the appropriate use of drugs and disease management. Schedule of their own medication and leaflet containing educational information were given to the subjects. QoL was assessed using a previously validated Mini-Questionnaire of QoL in Arterial Hypertension (MINICHAL). The questionnaire consisted of 16 two dimensions questions, i.e., mental health and somatic manifestation. We found that the mean increase in mental health status (49.5±5.3) was higher than that of somatic dimension (32.7±3.6). Both dimensions had statistically significant improvement (P value <0.05). In conclusion, pharmaceutical counseling is beneficial to improve QoL of hypertension patients.Keywords: hypertension, renal impairment, quality of life

Abstract

Tuberculosis is a devastating disease caused by Mycobacterium tuberculosis. If left untreated or not properly treated, it could lead to progressive tissue damage and even death. Short-term chemotherapy containing isoniazid, rifampicin. and pyrazinamide were proven to be very effective in the treatment of TB. However, the concern regarding its potential hepatotoxicity might hinder the completion of treatment. Information regarding risk factor of hepatotoxicity among Indonesian tuberculosis patients were limited. This study aimed to investigate risk factors of hepatotoxicity among Indonesian tuberculosis patients. This was a case-control study with retrospective approach conducted at one of the public hospital in Lampung, Indonesia. We included 320 tuberculosis patients who were classified as case (64 patients who were diagnosed with hepatotoxicity during hospitalization) and control (256 patients). Results of multivariate logistic regression analysis showed that age was significant risk factor of hepatotoxicity (adjusted odds ratio (OR) 1.056, 95% CI 1.0121, 1.091), while longer duration of hospitalization had a 1.4 lower odds of hepatotoxicity compared to control (adjusted OR 0.757, 95% CI 0.682, 0.839) (p<0.005). The results indicated that older patients were more likely to have hepatotoxicity, while patients with shorter duration of hospitalization tend to have higher risk of hepatotoxicity. In conclusion, age was the risk factor associated with hepatotoxicity among tuberculosis patients.Keywords: tuberculosis, hepatotoxicity, age, hospitalization

Abstract

Treatment for pneumonia has always been a challenge due to the difficulties in diagnosis and the growing incidence of antibiotic resistance. Beta-lactam antibiotics are the first line treatment for pneumonia. The combination of beta-lactam with other antibiotics are preferred than single antibiotic therapy. However, there was limited information regarding the effectiveness of combination between beta-lactam with either macrolides or fluoroquinolone for the treatment of pneumonia.The purpose of this study was to determine the most effective combination of antibiotics for hospitalized adults pneumonia patients. This was a cross-sectional study with prospective data collection. The data source was the medical record of the subjects. We included adult pneumonia patients hospitalized at Dr. Hasan Sadikin General Hospital during June-August 2015. We found that mean reduction of body temperature in group who received combination of a beta-lactam antibiotic and azithromycin was 1.53 0C, while in levofloxacin group, the reduction was 1.22 0C (p=0.210). Reduction in leukocytes and respiratory rate were 7800 and 2.29 times/minute, respectively, in the former group, while in the latter group the reduction of leukocytes and respiratory rate were 2600 and 5 times/minute. The differences were not statistically significant in both parameters (p=0.036 and 0.149, respectively). In conclusion, better clinical outcomes were observed in patients treated with combination of beta-lactam and azithromycin compared to combination of beta-lactam and levofloxacin, although the difference was not statistically significant.Keywords: pneumonia, pneumonia therapy, combination therapy for pneumonia
