Abstract

Background; Tuberculosis (TB) is a contagious disease caused by Mycobacterium tuberculosis (Acid-Fast Bacilli/AFB) that still be a world burden. Extra Pulmonary TB occurred when there were infection and development of Mycobacterium tuberculosis outside lungs, which about 10% cases was Milliary TB. There will be reported case with Lung TB New Case coexistence with Multiple Extra Pulmonary TB. Case; A female, 39 y.o., came to the Soetomo General Teaching Hospital with chief complaint body weakness since 12 months ago following with cough, decrease of appetite, decrease of body weight, low grade fever, and night sweating. The cough was on and off, not productive, and didn`t improve with usual therapy. Her other complaint were painless mass at the left supraclavicular area, swollen and pain at the right knee, and pain at the backbones. Patient had subsequently given Anti Tuberculosis Drugs (ATD) with Streptomycin injection based on clinical judgement. Discussion;  In several cases, Primary Focus as the result of AFB infection in the lung can spread to the other organs. The diagnosis had been made from comprehensive summary of clinical aspects, radiology data, laboratory data, and Histopathology data. Consideration of ATD choice based on grouping of TB diseases (Pulmonary/Extra Pulmonary), severity level of Extra Pulmonary TB and clinical judgement Conclusion; Had reported female patient diagnosed with Lung TB New Case, Osteomyelitis TB, Spondilitis TB, and Lymphadenitis TB. Based on therapeutic evaluation there was improvement condition of the patient after administration of ATD and injection of Streptomycin.

Abstract

Background: Multi Drug Resistant Tuberculosis (MDR TB) is an infection disease caused by Mycobacterium tuberculosis and has been resistant to isoniazid and rifampicin. Based on medical record of MDR TB Clinic RSUD Dr. Soetomo Surabaya, there is an increase of drop out rate which indicates that MDR TB patient’s adherence is low althouh there are some policies to improve their adherence. Therefore family role is needed for increasing MDR TB patients’ adherence and decreaseing drop out rate.Objective: to know the relationship between family role and MDR TB patient’s adherence on treatment in RSUD Dr. Soetomo Surabaya. Method: This analytical study in cross sectional approach is held on October 2017-June 2018 in MDR TB Clinic of RSUD Dr. Soetomo Surabaya and patients’ house. There are 24 patients and their family which are enrolled and interviewed in this study. Inferential statistic which was Fisher Exact Test used to find out and analyze the relationship between family role and MDR TB patient’s adherence on treatment in RSUD Dr. Soetomo Surabaya. Result: There is no difference frequencies of high and low family role. The MDR TB patients have high adherence. There is significant (p = 0,018) and moderate (c = 0,452) relationship of family role and MDR TB patients’ adherence on treatment in RSUD Dr. Soetomo Surabaya.Conclusion: To improve MDR TB patients’ adherence, family must have a significant role and give support. Health provider also should give education about importance of family role in MDR TB patients’ treatment.

Abstract

Background. Barium aspiration is one of rare major complication of esophagography. Aspiration pneumonia occurs about 40% without clinical symptom and can cause 30% of dead. Case. A female 62 years old choked a mount of barium when swallowing barium liquid in esophagography procedure. There was ronkhi on lower left haemithorax and granuler pattern on left paracardial impressed the rest of barium in chest x-ray photo. Patient suffered symptom of infection in eight days evaluation with infiltrate and rest of barium from chest x-ray photo. Microbiology examination of bronchus lavage founded pathogen microbe Burcholderia Gladioli and Pseudomonas and containing 1,22 mg/L of barium. Discussion. Barium aspiration can occur in esophagography procedure in extreme age. Chest x-ray and Computed Tomography can be used as primer tool to diagnose and evaluate barium aspiration which imaging hiperdens pattern due to barium with high atomic number (56). Prognosis of aspiration barium can be affected by the quantity and density of barium liquid which is choked and comorbid factor in patient. Barium aspiration mixed with secrete of oropharing which containing colony of pathogen can cause penumonia as disease complication. There is still no guideline for barium aspiration management, but bronchoscopy is recommended by many clinician. Conclusion. Pneumonia occur when aspiration barium mixed with secrete oropharing which containing colony of pathogen. Chest x-ray and CT scan can be used as tool to diagnose barium aspiration. Bronchoscopy is recommended to diagnose and to manage barium aspiration. When pneumonia is suspicious, antibiotic with anaerob activity is recommended given.

Abstract

Lung cancer is the leading cause of cancer death in Asia. In Indonesia, it contributes the highest number to cancer incidence in male and is ranked fifth in female cancer incidence. Determinated attempts have been delivered to eliminate cancer with chemotherapy being the most important management strategy for advance stage lung cancer. As most medicines come with side effects, so does chemotherapy. Beside killing the target cells, the cytotoxic compound of cancer drug may also seriously alter normal cells. Therefore, formidable toxic effect of chemotherapy to normal tissue becomes the main problem, which clinicians should consider. Myelotoxicity is one of the most common chemotherapy-induced adverse events. It takes place in bone marrow-producing regions causing netropenia, thrombocytopenia and anemia. Each of those cytopenias requires separate intervention.  This article will review the physiology of hematopoiesis, pathophysiology of cytopenias, and current guidelines and recommendations for prevention and treatment of myelotoxicity of cytotoxic chemotherapy.

Abstract

Extravasation is a problem for patient receiving chemotherapy. This will cause pain, ulcer, necrosis and might be a permanent disability. Extravasation is a leakage of a fluid or medicine to the surrounding subcutaneous tissue of the vein or vascular, that may cause skin or tissue necrotic. There was a wide span of incidence data of extravasation on the literature, between  0,01% and 11% reported from child to adult. Management of extravasation was based on proper maintenance of the intravenous (IV) line and application of cold or warm compresses, plus the use of antidotes when available. Antidotes for extravasation that have been shown to be useful are sodium thiosulfate for nitrogen mustard, dimethylsulfoxide for anthracyclines and mitomycin, and hyaluronidase for the vinca alkaloids. New treatments include dexrazoxane, sargramostim, and hyperbaric oxygen for doxorubicin extravasations. In order to improve patient service  quality and reduce morbidity, every medical staff handle cancer patient receiving chemotherapy should have sufficient knowledge about how to manage extravasation event.
