Abstract

Objective: Arnold Chiari Malformation is a pathological herniation of the hindbrain through the foramen magnum into the cervical canal, characterized by herniation of the inferior cerebellar tonsils of 3-5 mm 30-70% accompanied by syringomyelia.Methods: This case presents a 16-year-old woman who came to the Neurophysiology Section of  Wahidin Sudirohusodo Hospital with complaints of weakness and numbness in her left arm since last year. It all started with pain in the back of the head and neck after a heavy sneeze three years ago, and since then, the pain has been inconsistent. The patient has gone to the doctor and was given medication, but his symptoms are not improving. Clinical manifestations get worse after the patient uses a computer or mobile phone. Physical examination revealed atrophy of the thenar and hypothenar muscles and sensory dissociation in the left arm's 5th cervical to a first thoracic dermatome. NCV and F-wave examination showed no signs of radiculopathy, plexopathy, or neuropathy. Furthermore, cervical magnetic resonance imaging (MRI) showed Arnold Chiari Malformation Type-1 with syringomyelia extending from cervical spinal cord segment-2 to thoracic-1.Result: This patient was diagnosed based on history, physical examination, nerve conduction study, and MRI spine. There was no clinical improvement after therapy.Conclusion: The clinical picture of Arnold Chiari Malformation Type-1 is so varied that it requires detailed information to diagnose.

Abstract

Objectives: Tolosa-Hunt syndrome (THS) is caused by an idiopathic inflammatory process in the cavernous sinus, superior orbital fissure, or orbit. THS is a rare disease. The main clinical symptoms are ophthalmoplegic pain, accompanied by ipsilateral headache, paresis of one or more ipsilateral III, IV, and or VI cranial nerves, MRI or biopsy abnormalities, and cannot be categorized as other diseases. THS resolves adequately with corticosteroids within 24-72 hours of therapy.Methods: Male, 53 years old, was admitted for painful right ophthalmoplegia, constant onset, two weeks, ipsilateral headache, accompanied by diplopia and right eyelid ptosis.  There was no history of fever, trauma, or similar illness. Neurological examination showed exotropia due to paresis of the right III and IV cranial nerves. Analgesics can not relieve the pain. Laboratory findings, CT angiography, and brain MRI were normal.Result: There was clinical improvement within 24 hours after initiation of methylprednisolone followed by tapering-off oral prednisone.Conclusion: THS is a diagnosis of exclusion and must be distinguished from other causes of painful ophthalmoplegia. Careful follow-up is required to diagnose THS. The diagnosis should not depend on MRI alone but should be adjusted according to clinical findings and therapeutic response.

Abstract

Objective: Overdenture is a denture design that can be made in partial or complete loss of teeth , that has support from teeth and natural tooth roots that are still strong and healthy, and already received adequate endodontic treatment. If the tooth is extracted, the process of alveolar bone resorption will follow and worsen the retention and stability of the denture. One alternative in making complete dentures by retaining the remaining teeth in certain conditions is using an overdenture Methods: A 41 year old female, came to the Dental Hospital of UNHAS with a chief complaint of difficulty in eating and lack of confidence. Clinical examination shown a complete edentulous in the upper jaw and partial edentulous in the lower jaw. The remaining teeth were 33, 34 and 43. Magnetic Attachments were planned on 33 and 43 and metal coping on 34 as retained for mandibular overdenture.Results: Treatment with an overdenture is a good consideration because it is considered to be able to retain teeth in the alveolar process, which will provide better denture support than conventional dentures.Conclusion: Magnetic attachments significantly increase the retention of mandibular overdenture, patient is more satisfied and comfortable. Patient doesn’t experience difficulties in insert and removing the denture because when the denture is slightly shifted, the magnetic attachment returns it to its original position.

Abstract

Objective: This case report will be discussing the dental and oral care of a child with intellectual disability and hypothyroid disease. Intraoral examination showed the residual teeth 16, superficial caries in teeth 26, agenesis  12, delayed resorption  53, maxillary anterior teeth diastemas, crowding of the lower anterior teeth, deep bite, glossitis, and macroglossia tongue.Method: Various methods of behavioral approach were carried out at each visit. Treatment began from changing children's behavior, providing dental health education, filling cavities, scalling and applying fluoride topically.Result: In six visits, noticeable changes in plaque scores are getting better and children were more cooperative, caries dental has successfully restored, and topical fluoride  applied.Conclussion : Oral health management of patients with intellectual disabilities is different from normal patient.  Dentists must approach the patient according to the level of intellectual disability and know the complications of hypothyroid disease to avoid complications during dental treatment.

Abstract

Objective: The purpose of this article is to report the clinical manifestations and management of bone and gingival damage caused by fiber post drills.Methods: A 43-year-old patient reported that she had pain for 3 weeks that began after post placement. Once the pain had subsided, she noticed gingival recession on the mandibular left canine. A review of her history revealed that post drills were used without water cooling. In the clinical examination, bone exposure and attachment loss were observed. During periodontal plastic surgery, the necrotic bone was removed, and a free gingival graft was placed in the area.Results: After 10 days sutures were removed and 8 months later gingival healing was quite good.Conclusion: This case report shows periodontium may be damaged when fiber post drills are used to remove gutta-percha. Therefore, it is recommended that clinicians use instruments according to the manufacturer’s instructions and refer patients to a specialist when complications occur.

Abstract

Objective: The aim of this treatment were correcting crossbite with fixed orthodontic appliance by sequence wire. for correction anterior maxillary crossbite and correction maxillary and mandibula crowding, correction overbite and overjet, correction shifting mandibula midline. Methods: Initial treatment with fixed orthodontic appliance by leveling and alignment using niti round wire. Then followed expantion arch of maxillary with Australian wire and stopped in first molar. In mandibula followed using stainless steel wire and keeping not flaring. Addition elastic maxillary up n low for correction openbite caninus and correction interdigitation.Result: crossbite and  crowding anterior were corrected, relation of caninus class 1 angle, overjet and overbite is normal, shifting mandibula midline was corrected. Facial esthetic was better.Conclusion: Orthodonti treatment with simple technique without extraction can achieved significant change  by using some wire  in fixed orthodontic appliance for correction anterior crossbite.
