Abstract

Background: Congenital Scoliosis (CS) defined as vertebral lateral deviation which forms during the intrauterine embryonic phase, carrying incidence between 0,5-1 per 1000 births. Mortality rate increases in untreated case, especially due to pulmonary problem (40%).Contents: CS can range from hemivertebrae (HV) which may be single or multiple, vertebral bar with or without HV, block vertebrae, wedge shaped or butterfly vertebrae. The risk factors for progression include: type of defect, site of defect, and the patient’s age. Plain radiographs remain standard diagnosis. MRI evaluation should be considered. The goal of managing CS is to obtain a balanced trunk and spine while maintaining as much normal spinal growth as possible and preventing neural deficit. Non-operative management is including observation, brace, and traction. The operative procedures are broadly divided into (a) those preventing further deformity: in situ fusion, convex hemiepiphysiodesis, and HV excision, (b) those that correct the present deformity: hemiepiphyseodesis & hemiarthrodesis, single & dual growing rods, HV excision, and reconstructive osteotomy.Conclusion: The treatment of CS remains one of the more challenging aspects of pediatric orthopaedic surgery. Operative treatment should be reserved for patients whom non-operative management didn’t meet the criteria or failed.

Abstract

Background: Osteoporosis prevalence in Indonesia is high. Risedronate commonly used for treating the osteoporosis. Unfortunately, long term therapy can cause hazardous adverse effect to the patient. Previous research in rat model osteoporosis, Zinc have a potential effect to prevent and rebuild bone structure with osteoblast stimulation in bone formation and osteoclast activity inhibition in bone resorption by increasing Osteoprotegerin production, that inhibit bond between RANKL and RANK.Purpose: To determine the effectiveness of Zink and Risedronate on bone turnover in osteoporotic model rats assessed the β-CrossLaps expression.Methods: This research is conducted as experimental comparative method used 25 female Galur Wistar rat that divided into Non-ovariectomy and Ovariectomy group to induce osteoporosis. The treatment is divided into Zinc-Risedronate, and Zinc, or Risedronate only. One Way Anova and Independent T-test was used to analyzed the significant difference between groups.Result: One Way Anova statistic test forβ-CrossLaps expression value in the 3rd Taken blood sample showed that p is less than 0.05 (p<0.05), which indicated that are significant difference in the mean between variable at 3rd Taken blood sample in all group. Meanwhile, Independent T-test was used to compare β-CrossLaps expression in 3rd taken blood sample between Ovari and Ovari+Zinc, Ovari and Ovari+Zinc+Rise showed p is less than 0.05 (p<0.05), which indicated that there are significant difference in the mean between variable 2 groups.Conclusion: Zinc-Risedronate administration will decrease bone turnover and β-CrossLaps expression more than Zinc or Risedronate only in rat model osteoporosis.

Abstract

Background: The treatment for acromioclavicular joint injury are debatable, there are fixation options include tension band wiring, AC joint reconstruction and hook plate These fixations are capable of providing a stable fixation, but controversy still exists that mentions the superiority of each of these fixationsPurpose: To compare biomechanical stability of 3 fixation include tension band wiring, double endo button, and hook plate to provide a scientific basis of the fixation.Methods: This research is an experimental in vitro. Using 27 acromioclavicular joints cadaver divided into three groups that performed tension band wiring fixation, double endo button and hook plate. Each fixation evaluated with 10, 20, 50 and 100 times repetitions with 100N traction force.Results: Tension band wiring gives the smallest displacement. In 10 times repetition average displacement of tension band wiring 0.056 mm (p = 0.000) compared to double endo button 1.622 mm and hook plate 0.867 mm. In 20 times repetitions, tension band wiring 0.1667 mm (p = 0,000) compared to double endo button 3.1778 mm and hook plate 1.1111 mm. In 50 times repetition, tension band wiring 0.3111 mm (p = 0.000) with double endo button 4.7778 mm and hook plate 1.3556 mm. In 100 times repetitions, tension band wire 0.556 mm (p = 0.000) while double endo button 5.4444 mm and hook plate 1.4556 mm.Conclusion: Tension band wiring have a good stability compared to double endo button and hook plate. But all of fixation provide stability for acriomioclavicular joint motion.

Abstract

Background: Extrapulmonary Tuberculosis is approximately 10% of all tuberculosis (TB) cases, and the musculoskeletal system’s involvement was only 2-4. The incidence of ankle TB was found to be less than 2% of all musculoskeletal TB. The rare occurrence, uncommon site, non-specific symptoms, lack of awareness frequently causes diagnosis difficulties.Case Report: A 2,5-year-old child came to the hospital with the main complaints of swelling and pain on the left ankle since one year ago. She was diagnosed with the ankle’s synovitis from the previous hospital and was given analgesics and a warm compress on the left ankle. However, the swelling of her left ankle increased during the last seven months, causing limitation of the left ankle movement, then she was admitted to the hospital for further examination.Discussion: Radiologic examination revealed peripheral osseous erosions, reduced bone mineral content, and soft tissue swelling. A Mantoux test was positive with 15 mm induration and 25 mm erythema. Anti-tuberculous drugs (ATD) therapy was given with isoniazid 50 mg, rifampicin 75 mg, pyrazinamide 150 mg, and ethambutol 230 mg for two months, as an intensive phase therapy following rifampicin 75 mg and isoniazid 50 mg for ten months.Conclusion: Tuberculous arthritis of the ankle poses a diagnostic-difficulties, it should be considered especially in the endemic area. It is often misdiagnosed with other musculoskeletal conditions, which can delay the initiation of appropriate treatment. TB must be confirmed by histological pathology. A combination of ATD and surgery can lead to clinical improvement.

Abstract

Background:  Plantar fasciitis (PF) is the most common heel pain in adult. It is estimated that it affects as much as 10% of the population over the course of their lifetime. Previous study mention that prolonged standing is one of the risk factor of PF, however other study suggests that there is low evidence of prolonged standing may cause PF. The objective of this study was to findout whether or not prolonged standing may correlate to PF. Methods: This cross-sectional with total sampling study involved 73 workers in total. 35 of them did not fulfill the inclusion criteria, hence only 38 of subjects were used. The data collected using questionnare and analyzed using Chi-square. with p-value (p) <0.05 and confidence interval (CI) 95%. Results: There is no correlation between prolonged standing and PF symptoms p= 0.249. Conclusion: There is no correlation between prolonged standing and plantar fasciitis and even though in other study suggest that BMI, and age also the risk factor of PF this was not consistently confirmed in this study. Further research may use larger number of subjects and use other measure to determine the definition of prolonged standing.

Abstract

Background: The incident of ankle sprain reported 1 of 10.000 case each day. The most incident was male about 22-24 year and sport active. We reported 2 cases (15 years old female and 21 years old male) both of them played routine basketball and futsal.Case: We reported 2 cases, male 21 years old and female 15 years old, both of them suffered from anterior talofibular ligament rupture, with AOFAS score fair (below 80 point) from the MRI there is rupture of ATFL.Discussion: The patient administered to the hospital after injury at sport. By physical examination (fair at AOFAS score) and radiological finding, they was decided underwent rupture anterior talofibular ligament and prepared for reconstruction with modified Brostrom-Gould technique using fiber wire. After + 3 month, the evaluation from physical diagnostic, radiology and we got an excellent AOFAS score, which means the 2 patient got really satisfaction and has no limitation at doing daily activities.Conclusion: ATFL is one of the lateral ligament of the ankle which protect the foot while moving at plantar flexion. Modified Brostrom-Gould technique using fiber wire gave good result for reconstruction ATFL rupture for these 2 patients.

Abstract

Introduction: Cervical spine is the most mobile part of the human spine, thus making it the most vulnerable compared to all the other vertebral structures. Surgical procedures are usually performed within the first 24 hours, or 4-6 weeks after trauma in order to prevent any secondary trauma. The research was conducted to evaluate the amount of time of the surgical procedure towards the effectivity and improvement of the neurological status in the cervical injury or acute spinal cord injury (ASCI).Methods: A meta-analysis research which evaluate the effectivity of surgical procedure on cervical trauma/ASCI, with the database procured from PubMed, Embase, and Cochrane. The main parameter is the decompression procedure and the clinical outcome which were categorized. The time of surgery or decompression are categorized into “<24 hours” and “>24 hours”, the neurological outcome is categorized into “improvement” and “no improvement”. The data was presented in odd ratio (OR) and confidence interval (CI) and were further analyzed by forest plot.Results: From PubMed, there were 353 articles, Embase 2 articles, and Cochrane 594 articles, but only 3 articles which fulfilled the inclusion criteria. The comparison between the surgical procedure in the cervical <24 hours with the surgical procedure >24 hours was identified for this research. Statistically, there was a significant difference on the neurological status (OR=1,85; 95%CI=1,21-2,84; p<0,01).Conclusion: With meta-analysis background, early decompressive procedure <24 hours for cervical trauma patients produced a significantly better result in improving the neurological status compared to the late decompressive procedure >24 hours.

Abstract

Osteoarthritis (OA) is the number eight disease causes Years of Disability in developing countries. OA worsen life quality, from movement limitation until inability to perform normal daily routines. Obesity is one of the risk factor of OA. The doubles increasing trend of obesity from 1980, might possibly causes the escalation of OA case. However, there is still no study that explains the correlation between OA grade, especially knee OA, with Body Mass Index (BMI). The purpose of this study was to determine whether there is correlation between knee osteoarthritis (OA) grade with Body Mass Index (BMI) in outpatients of Orthopaedic and Traumatology department RSUD Dr. Soetomo. This study was cross-sectional study with analitical observational design and survey was the method to determine whether there is correlation between knee OA grade and BMI. Height and weight measurement and also knee x-ray reading were performed to determine the BMI and knee OA grade. The BMI categories that were used were based on standard from Health Department of Republic of Indonesia, while the grading system that were used were based on Kellgren-Lawrence’s grading system. Populations in this study were taken from the outpatients of Orthopaedic and Traumatology Department RSUD Dr. Soetomo during August until November 2015. After statistic test using Spearman correlation test was done, the result was p=0,822. It can be concluded that there was no significant correlation between knee OA grade and BMI. Furthermore, knee OA grade was not affected by BMI only, but also other risk factors as well.

Abstract

Background: Osteochondroma is a benign bone tumor that most cases are asymptomatic. The symptoms may occur as a complication of osteochondroma, which is rarely happened. Surgical intervention is needed to treat the complications.Case:  A 20-year old male football athlete presented with pain on his left thigh's lump after he fell during his training. Mass and tenderness on the medial side of the left distal femur were found. Plain X-Ray showed an irregular mass on the left distal femur's surface with a fracture through the stalk of pedunculated osteochondroma. The patient underwent total excision of the osteochondroma.Discussion: The fracture of the osteochondroma often occurs in teenagers because they are more active physically. It happens due to direct or indirect injury. The treatment of this fracture is controversial between observation or surgical excision. In this case report, the patient could demonstrate full weight-bearing right after the surgery.Conclusion: Fracture as a complication in osteochondroma often happens to teenagers, and surgery proved to give a good outcome in this case.

Abstract

Background: Osteosarcoma is the most common bone neoplasm found in the community but evaluation osteosarcoma cases in RSUD Dr. Soetomo has not been updated since 1995.Purpose: This study o osteosarcoma patient characteristic as well as therapy at Dr. Soetomo General Hospital in 2007 to 2016. It expects to show survival rates of osteosarcoma patient, so it can be a reference for searching the problems in the treatment of osteosarcoma cases and helps to decide treatment for osteosarcoma.Research Methods: Descriptive retrospective study, conducted on osteosarcoma patients at Dr. Soetomo General Hospital during 2007-2016 periods. Data were obtained from Ortho tumor patient database, and contacting them by phone or home visit.Results: Osteosarcoma patients was found mostly in 2015, while the least in 2008, with trend increasing by time. Majority of the patients came with advanced stage. Osteosarcoma treated mostly by amputation, either with or without chemotherapy. The survival rate in the first, second, or the fifth year was found lower than other references. Most common cause of mortality was the metastasis.Conclusion: Awareness of the society about the cancer sign of cancer and desire to use medical treatment as a priority is still low. This causes a low early detection rate of osteosarcoma and a high rate of metastatic cases because of inappropriate early treatment. Further socialization and increased awareness of medics about the suspicion of osteosarcoma are needed to improve the success rate of treatment as well as the survival rate.
