Critical Medical and Surgical Nursing Journal (CMSNJ) (p-ISSN: 2355-1992), (e-ISSN: 2656-470X) was established at the end of 2012 under Universitas Airlangga. The CMSNJ was prepared in response to the development of nursing/health science and technology to contribute to the development of sustainable quality nursing/health resources. CMSNJ is published two times per year every April and October. Articles published in CMSNJ is written in English since Vol. 10 No. 1 April 2021. Abstract

Introduction: Patients Modernisasi dan akulturasi budaya daerah dapat mempengaruhi pengetahuan masyarakat dalam penggunaan sumber daya local sebagai sumber pengobatan termasuk dalam kondisi kegawatdaruratan yang terjadi sehari-hari. Sumber daya local perlu dikaji secara detail terkait manfaat, metode penggunaan dan efek sahmping. Penelitian ini bertujuan untuk mengeksplorasi pengetahuan masyarakat mengenai kearifan lokal (ethnomedicine) dalam penanganan masalah kegawatdaaruratan sehari-hari di masyarakat Kota Ternate.Methods: Metode penelitian menggunakan deskriptif dengan teknik purposive sampling. Total sampel yang digunakan adalah 40 orang yang berpengalaman menggunakan kearifan lokal dalam menangani kegawatdaruratan sehari-hari. Data diperoleh melalui kuosioner, wawancara, observasi dan dokumentasi.Results: Hasil penelitian menunjukkan terdapat 11 masalah kegawatdaruratan yang terjadi dan ditangani dengan menggunakan 108 ramuan. Dari ramuan ini teridentifikasi sebanyak 22 macam jenis tanaman obat yang digunakan. Perlunya peningkatan edukatif terhadap masalah kegawatdaruratan sehari-hari yang terjadi di masyarakat dan bagaimana pemanfaatan kearifan lokal sebagai alternatif penanganan pertama yang dilakukan.Conclusion: Instansi terkait perlu memberikan dukungan terhadap pelestarian tanaman obat keluarga yang selain keindahan ini juga tersimpan banyak manfaatnya.

Abstract

Introduction: Caring for family members with cancer takes a long time. Various problems that arise during treatment will have an impact on the physical condition and psychological care giver so that this will have an impact on family adaptation. The purpose of this study was to analyze the factors that influence the adaptation ability of family caregivers who care for families with terminal condition cancer in the Oncology Poly RSUD Dr. Soetomo Surabaya. Methods: A cross-sectional study design. The population in this study are families who care for family members with terminal condition cancer in the Oncology Polyclinic Dr. Soetomo as many as 138 respondents were obtained by purposive sampling. The independent variables are attitude, trust, family support, coping mechanisms, and role functions while the dependent variable is the adaptability of family caregiver who care for families with terminal condition cancer. The research instrument used questionnaire attitudes, beliefs, family support, coping mechanisms, role functions and adaptability. Data analysis using Spearman Rho with significance level p ≤ 0.05. Results: Attitudes, family support, coping mechanisms, role functions are related to family caregiver's adaptability, while trust is not related to family caregiver's adaptability. Conclusion: Adaptability is needed for family caregivers in caring for families with terminal condition cancer. Future researchers are expected to intervene further to improve family support and adaptive coping mechanisms to help overcome problems that arise during the treatment of patients with cancer. Keywords: Attitude, Trust, Family Support, Coping Mechanisms, Role Function, Terminal Condition Cancer

Abstract

Introduction: Low back pain was discomfort sensation in the lower back area or above the gluteal. Low back pain was a potential health problem for ride-hailing riders. This study aimed to describe of low back pain in online ride-hailing in Surabaya.Method: This study was done in Surabaya, used a cross-sectional design. The sample was 100 respondents using a consecutive sampling technique. Data were obtained using a questionnaire and describe using crosstabs statistical.Results: Online ride-hailing experienced low back pain amounts 61% and has not experienced amounts of 39%. Respondents have experienced low back pain aged >30 years (43%), normal BMI (41%), work position (35%), work duration > 8 hours per day (50%), work period > 5 months (52%), smoking (50%), and not exercising (46%).Conclusion: Low back pain incident has a high value that amounts to 61%. Online ride-hailing are advised to pay attention to the duration of work and work position. Enough rest time and apply an ergonomic working position can prevent the fatigue that causes low back pain.

Abstract

Introduction: Families who do not support patients can influence the level of rehabilitation adherence, which in turn will reduce the level of independence of patients after stroke. The purpose of the study was to explain the relationship between family support and rehabilitation adherence with the independence of stroke patients. Methods: This study used a cross-sectional type with a sample size of 73 post-stroke patients out of a total of 126 people. Samples were taken by consecutive sampling technique. The independent variable is the level of family support and the level of rehabilitation compliance. The dependent variable is the level of independence of patients after stroke. The research data obtained were analyzed with Spearman's Rho α ≤0.05. Results and Analysis: There was a relationship between family support and the independence of post-stroke patients with a value of α≤0.05, p = 0.001 and r = 0.384. There is a relationship between rehabilitation adherence with the independence of post-stroke patients with a value of α≤0.05, p = 0.000 and r = 0.473. Conclusion: Family support and good rehabilitation adherence do not affect post-stroke patients with severe degrees of disability and cognitive changes.

Abstract

Introduction: The incidence of Diabetic Foot Ulcer (DFU) is currently still quite high resulting in a decrease in the quality of life and increase dependence on family and health services. But until now the factors that cause DFU are still not known with certainty. This study aimed to analyze the relationship of host factors with the incidence of foot ulcers in type 2 DM patients.Method: The method used in this study was cross sectional, involving 115 patients with Type 2 diabetes who did not have other comorbidities and mental disorders. The variables studied were blood sugar levels, foot care and stress levels. Measuring instruments used were the Nottingham Assessment of Functional Footcare questionnaire and the Diabetes Distress Scale questionnaire. The collected data were analyzed using the Spearman Rho test with a significance level of 0.05.Results: Most respondents had high levels of stress, poor foot care and blood sugar levels <200 md / dl. Bivariate test results showed that the host factors in this study were related to the incidence of DM ulcers including foot care (p = 0.003: r = 0.276), stress level (p = 0.049: r = 0.230), blood sugar control (p = 0.013: r = 0.184) which means that overall the variables are related to DFU events. Discussion: Overall the variables have a positive relationship. Good foot care will reduce the risk of DM ulcers. Stress levels and high blood sugar levels will increase the risk of DM ulcers.Conclusion: Factors of blood sugar levels, stress levels, and foot care can influence the incidence of DM ulcers so that interventions are needed that can improve the behavior of foot care, blood sugar levels and stress management.

Abstract

Background: Patients with cardiac stent have been risk of restenosis. Several studies suggest that diabetes mellitus is the factor that most consistently increases the risk of restenosis. This requires further research with the literature review to analyze the continuity of the journals found.Methods: A literature review searched based on PRISMA, by the process of identification, screening, eligibility, and included. Journals are identified based on their focus and  results. Then screening by analyzing the design, samples, variables, instruments, methods and results. Then assessed for their eligibility using The Joanna Briggs Institute (JBI) Critical Appraisal.Results: The total number of journals that reviewed in this study is 15 journals. Six journals stated DM as a restenosis factor, two journals stated BMI as the main factor, four journals mentioned other factors and two journals denied DM was a predictor of restenosis.Conclusion: The results of 15 journals that reviewed, diabetes mellitus was a factor that appeared consistently and had a role in the incidence of restenosis from lifestyle factors or disease history.

Abstract

Pendahuluan: Terapi mendengarkan Al-Qur’an dan Teknik Relaksasi Otot Progresif merupakan metode non-farmakologi yang dapat menurunkan tekanan darah. Tujuan penelitian ini adalah untuk mengetahui pengaruh terapi mendengarkan bacaan Al-Qur’an (Ayatussyifa) dan relaksasi otot progresif terhadap tekanan darah pada penderita hipertensi. Metode: Penelitian ini menggunakan quasy-experiment dengan pretest – posttest control group design dengan total sampel sebanyak 50 responden penderita hipertensi dengan menggunakan teknik non-probability sampling dengan tipe purposive sampling. Variabel independen dalam penelitian ini yaitu terapi mendengarkan bacaan Al-Qur’an (Ayatussyifa) dan relaksasi otot progresif sedangkan variabel dependennya yaitu tekanan darah sistolik dan diastolik lansia dengan hipertensi dengan menggunakan lembar observasi sebagai instrument penelitiannya. Analisis penelitian ini menggunakan Wilcoxon Signed Rank Test dan Mann-Whitney U Test (p=<0,05). Hasil: Hasil penelitian menunjukkan adanya pengaruh pada kelompok perlakuan (sistolik p=0,000), (diastolik p=0,000). Hasil uji Mann Whitney kelompok perlakuan dan kontrol menunjukkan hasil Pre-Sistolik (p=0,739) dan Post-Sistolik (p=0,006) sedangkan Pre-Diastolik (p=0,001) dan Post-Diastolik (p=0,410). Diskusi: Terapi mendengarkan Al-Qur’an (Ayatussyifa)  dan relaksasi otot progresif dapat menjadi stimulus baik yang dapat berpengaruh terhadap tekanan darah baik sistolik maupun diastolik pasien hipertensi. Perawat dapat memberikan terapi tersebut sebagai salah satu pilihan dalam menurunkan tekanan darah para lansia penderita hipertensi.

Abstract

Introduction: Hypertension is a common cause of the increase of mortality rate and morbidiety in the community. Complications of hypertension cause approximately 9.4 deaths around the world each year. The purpose of this study is to analyze the relationship of factors controlling hypertension with blood pressure.Methods: This study used a descriptive correlational design with the cross sectional approach. The study population was patients with hypertension in Puskesmas Pandanwangi Malang City. The sample used as many as 96 people based on purposive sampling technique. The instruments used were questionnaire sheets, spyghmomanometer and stethoscope. Analysis using the test Spearman rho test with a significance level of p <0.005.Result: The results showed there are a relationship between knowledge (p = 0.000 and r = 0.778), attitude (p = 0.000 and r = 0.626), diet (p = 0.000 and r = 0.609), physical activity (p = 0.000 and r = 0.449), family support (p = 0.000 and r = 0.607), and medication adherence (p = 0.000 and r = 0.726) with blood pressure .Conclusion: There is a significant relationship and the direction between the knowledge, attitude, diet, physical activity, family support, and medication adherence with blood pressure.

Abstract

Background: Hypertension is recognized as a major health problem throughout the world leading to various life-threatening heart diseases. The higher the level of blood pressure in the long run, the more likely there is a complication of hypertension. Slow breathing exercises are one of the non-pharmacological therapies that can be used to control or reduce blood pressure and strengthening breathing muscles. This systematic review is to analyze the effectiveness of slow breathing training in blood pressure in patients with hypertension.Methods: Literature search is through Scopus, Pubmed, Proquest, Science Direct, and EBSCO. with the last 5 years (2014-2019). Criteria for inclusion in this review are as follows: studies involving hypertension, ages 18 years and over, patients who have received or are still receiving slow breathing exercises. Keyword in this review was Slow Breathing, Blood Pressure, and Hypertension.Results: There are 10 journals chosen. Total respondents in this review were 1757 hypertensive patients who received slow respiratory therapy. Slow breathing training provides many benefits for hypertensive patients. Subjects with hypertension, pre-hypertension, and antihypertensive drugs, were given slow breathing intervention that is inhaling as much as ≤10 breaths/minute carried out for ≥5 minutes. This exercise is carried out at ≥3 days/week; total duration of intervention ≥4 weeks; Slow breathing training reduces blood pressure at rest, the average using this slow breathing exercise, blood pressure drops 10 -15 mmHg in systole and diastole. Overall, slow breathing exercises decreased SBP by -5.62 mmHg [-7.86, -3.38] and DBP by -2.97 mmHg [-4.28, -1.66].Conclusion: Slow breathing training provides many benefits for hypertensive patients. Especially for patients with isolated systolic hypertension. This non-pharmacological therapy can be carried out by individuals independently in controlling the breathing rate so that they can reach breathing speed both during inspiration and expiration. Slow breathing exercises can also help increase baroreflex sensitivity, then affect blood pressure reduction.

Abstract

Introduction : Diabetes Mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia caused by abnormalities in insulin secretion, thus impacting on blood glucose levels. Calabura muntingia leaves are traditionally used as an alternative medicine because they contain flavonoid compounds which have anti-diabetic properties. In general, it is known that how to make traditional medicine is by boiling it, where the solvent only uses water so that the polar compounds will be attracted. The purpose of this study is to explain effectiveness the effect of administration between calabura muntingia leaves and calabura muntingia fruit through a review of studies related to blood sugar levelsin the last ten years.Method : Search for journals or articles using Scopus, Proquest, and Scholar indexed databases using appropriate keywords. The Center for Review and Dissemination and The Joanna Briggs Institute was used to assess the quality of the study. The framework used for the review is PICOS and the inclusion criteria used are English and Indonesian journals published in 2010 to 2020. Data analysis and tabulation are carried out in articles or journals. Title, abstract, full text and methodology are assessed to determine the eligibility of the article or journal.Results : Six hundred and forty-one articles were identified, there were eleven articles that fit the research criteria. The leaves of muntingia calabura and fruit of muntingia calabura contain flavonoids and saponins which can reduce blood sugar levels. A flavonoid compound that is thought to have activity in reducing blood glucose levels is quercetin.Conclusion : The form of extract, infusion, juice and stew did not change the content of the calabura muntingia plant.
