Abstract

Indonesia is predicted to have the seventh-highest number of people with type 2 DM worldwide in 2030. Consistent adherence to taking medication as prescribed is the key to diabetes self-care. Poor treatment adherence is associated with poor glycemic control, microvascular and macrovascular complications, and increased care costs. Treatment success is influenced by patient adherence to treatment which is a major factor in therapy outcomes. Objective to identify the relationship between patient characteristics and adherence to medication for patients with type 2 diabetes. This type of research is an analytic observational study with a cross-sectional approach. The samples used were 126 patients who were taken using a purposive sampling technique according to the inclusion criteria: being diagnosed with type 2 diabetes mellitus and receiving a prescription for antidiabetic drugs. The medication adherence instrument used the Morisky Medication Adherence questionnaire (MMAS-8). Data were analyzed using descriptive statistics and the Kendal-tau test. Adherence to taking medication in patients with Type 2 DM was mostly moderate (54%). There was a significant relationship between age (p=0.043), education level (p=0.000), gender (p=0.008), duration of suffering from Type 2 DM (p=0.000), frequency of taking medication (p=0.001), comorbidities (p=0.000), with adherence to taking DM medication. Still, there was no significant relationship between work type and adherence to DM medication (p=0,333). Type 2 DM patients mostly have a moderate level of medication adherence. Medication Compliance in Patients with Type 2 DM is influenced by several factors, including age, gender, education level, duration of suffering from DM, number of drugs taken, frequency of medication, and comorbidities.

Abstract

Antioxidants are found in various types of fruits and vegetables, one of which is tomatoes ( Licopersicon esculentum Mill ). Tomatoes contain lycopene, flavonoids and vitamin C compounds as antioxidants. Tomato fruit is made into juice by using the extraction method. Tomatoes with direct use are less effective, so they are made in the form of pharmaceutical preparations in the form of hand body lotion gel using tomato juice. The purpose of this study was to determine the effectiveness of tomatoes as antioxidants in handbody lotion gel preparations using the DPPH method. This research used experimental research method with 3 concentration formulas of tomato juice handbody lotion, namely 5%, 10% and 15%. The physical evaluation test showed that the results had met the physical requirements of the handbody lotion (organoleptic, homogeneity, spreadability, adhesion, viscosity, pH, lotion type and hedonic test). The results showed that tomatoes have effectiveness as antioxidants. This test was carried out using the DPPH method on the preparation of handbody lotion gel tomato juice formula II, the IC50 was 6,620 g/mL, which was greater than formulas I and III. The smallest IC50 value was obtained from formula III, which was 5,205 g/mL. The smaller the IC50 value, the stronger the antioxidant power.

Abstract

The pandemic situation poses great challenges for health care facilities such as rapidly changing hospitals in meeting the demands of infrastructure and operations. In addition to the need for adaptation related to the services provided, hospitals must be able to reduce the number of Covid-19 deaths as a top priority for health authorities. Uncertain conditions lead to the emergence of risks that cause losses so it is important to have risk management in hospitals This study was conducted to review risk management in hospitals in Semarang City that already have a framework for dealing with the COVID-19 pandemic and assess the impact of implementing the framework to the risk management process. This study is a cross-sectional study to see the relationship between the application of a risk management framework to the implementation of the Covid-19 risk management process in hospitals. The population in this study is all regulatory documents and reports on the implementation of the COVID-19 risk management process in hospitals in Semarang City in the period January 2020 to December 2021. Data is collected from documents and recording data in hospital related to regulations and risk management assessment instrument. Data were analyzed using Partial Least Square (PLS) software with SmartPLS 3.0 to test the hypothesis by looking at the probability value and its t-statistics. The study shows that the framework has a positive effect on the implementation of the risk management process in hospitals (T-Statistic > 1.96, p value < 0.05). The stronger the implementation of the COVID-19 risk management framework, the more positive the implementation of the COVID-19 risk management process will be.

Abstract

Soursop leaves have acetogenin compounds that can be used to fight cancer in the presence of adenosine triphosphate (ATP) inhibition. Acetogenin is a long-chain fatty acid derivative (C23 or C34) which is very difficult to dissolve in water so that it will cause low oral bioavailability. The use of Selfnanoemulsifying Drug Delivery System (SNEDDS) as a drug delivery system can increase bioavailability and increase the solubility of a drug. Optimization of the use of VCO as the oil phase, tween 80 as surfactant and propylene glycol as co-surfactant in the SNEDDS formula using the Simplex Lattice Design method. The responses used in the optimization are % transmittance and emulsification time. Verification of the optimal SNEDDS formula obtained with one sample t-test. The optimal formula of SNEDDS ethyl acetate fraction of soursop leaves was tested for particle size with Partcle Size Analyzer, zeta potential with zeta sizer and antioxidant activity using ABTS. The optimal formula was obtained at the concentration of the components of VCO oil (0.56 ml), Surfactant (3.50 ml), and Propylene glycol (0.93 ml) with a desirability value of 0.769. The results of the verification of the optimal formula show that there is no significant difference between the prediction and the actual, which means that the Simplex Lattice Design method is valid and can describe the % transmittance response and emulsification time of SNEDDS ethyl acetate fraction of soursop leaves. The optimal formula for SNEDDS has an emulsification time of 53±1.527 seconds, transmittance value of % 96.3±0.360%. The particle size results obtained from the optimal formula were 13.83 nm ± 0.650, the polydispersity index was 0.143 ± 0.128. The zeta potential test results obtained were -0.23 mV ± 0.057. The optimal formula for the ethyl acetate fraction of soursop leaf SNEDDS has a very strong antioxidant activity with an IC50 value of 16.89 ppm.

Abstract

One of the Dengue Control programs is DHF Health Surveillance Officer (GASURKES) according to the Semarang City Regulation Number 5 of 2010, which was then supplemented by the Semarang City Mayor Regulation No. 27 B of 2012 concerning Instructions for Implementing Regional Regulation No. 5 of 2010. However, the incidence of DHF in Semarang City in 2015-2020 has not yet reached the national DHF incidence rate target. Therefore, there is a need for an in-depth study of the performance of Health Surveillance Officers and a description of the factors that affect performance as a description of the condition of health workers. The purpose of this research is to describe and analyze the performance of GASURKES in a dengue control program. This research is qualitative research conducted in North Semarang District. The main informant in this study was the DHF surveillance officer, while the triangulation informant was the coordinator of the health surveillance officer. The sample was determined through the purposive sampling technique. Data collection was carried out by in-depth interview method. Data were analyzed using qualitative data analysis. The results of the study indicate that the knowledge of health surveillance officers (GASURKES) in the North Semarang District is quite good. The description of GASURKES' workload is still high and tends to be multi-job. The motivation factor for GASURKES is still lacking because it is related to the lack of rewards received compared to the high workload. Health Resources for the implementation of dengue control efforts are sufficient. However, the performance of health surveillance officers (GASURKES) in the North Semarang District has reached the work target.
