Abstract

Pregnancy complications remain a major cause of high maternal and infant morbidity and mortality rates in various countries. Delays in detecting complications such as preeclampsia, gestational diabetes, preterm labor, and hypertensive disorders of pregnancy can lead to conditions that threaten the safety of both the mother and the fetus. The development of Artificial Intelligence (AI) technology provides opportunities to improve early detection capabilities through rapid and accurate clinical data analysis. This study aims to develop and evaluate an AI system for early detection of pregnancy complications using a Hybrid Research and Development (H&R) design. The research stages include needs analysis, data collection, model development, system validation, and prototype implementation. The data used comes from medical records of pregnant women, including age, blood pressure, body mass index, blood glucose levels, gestational age, and history of previous complications. Several machine learning algorithms were tested, namely Random Forest, Support Vector Machine, Decision Tree, Logistic Regression, and Artificial Neural Network. The results showed that the Random Forest algorithm provided the best performance with an accuracy rate of 92%, sensitivity of 94%, and specificity of 90%. The most influential variables in prediction were blood pressure, history of preeclampsia, and body mass index. The system implementation demonstrated high user acceptance, as it helped healthcare workers identify high-risk pregnant women more quickly and accurately. Therefore, the application of AI has the potential to become an effective clinical decision support system for improving the quality of antenatal care and supporting efforts to reduce maternal and infant mortality.

Abstract

Background: Technological developments in health education are driving a transformation of learning toward interactive digital media. In midwifery education, interactive learning media such as videos, digital simulations, and e-learning are expected to improve student learning outcomes, encompassing cognitive, affective, and psychomotor aspects. However, the effectiveness of their implementation still requires in-depth study within the context of student experiences. Objective: This study aims to explore the influence of the use of interactive learning media on midwifery student learning outcomes based on their learning experiences. Methods: This study used a qualitative approach with a phenomenological design. Participants consisted of 12 midwifery students selected using purposive sampling based on the criteria of having participated in interactive media-based learning. Data were collected through in-depth interviews, observation, and documentation. Data were then analyzed using the Miles, Huberman, and Saldaña model, encompassing the stages of data reduction, data presentation, and conclusion drawing. Data validity was tested using triangulation of sources, methods, and member checking. Results: The study revealed four main themes: improved understanding of learning materials, increased motivation and engagement in learning, increased readiness for clinical practice, and challenges in implementing technology. Interactive learning media helped students understand complex midwifery concepts through visualization and simulation, increased learning motivation through more engaging learning, and improved readiness for clinical practice. However, limited internet access and technological infrastructure remain barriers to optimal implementation. Conclusion: Interactive learning media positively impacted midwifery students' learning outcomes across cognitive, affective, and psychomotor domains. Integrating interactive learning media into the midwifery education curriculum is highly recommended to improve the quality of competent and adaptable graduates in the digital era.

Abstract

Digital transformation in healthcare requires hospitals not only to adopt technology but also to strengthen leadership capacity, healthcare worker engagement, and organizational support to achieve optimal service performance. This study aims to analyze the influence of Digital Leadership, Employee Engagement, and Organizational Support on the performance of healthcare workers in hospitals. A quantitative approach with a cross-sectional design was employed. The study involved 200 healthcare workers selected based on established inclusion criteria. Data were collected using Likert-scale questionnaires and analyzed using Partial Least Squares-Structural Equation Modeling (PLS-SEM). Model evaluation encompassed convergent validity, discriminant validity, construct reliability, the coefficient of determination (R²), effect size, predictive relevance, and hypothesis testing via bootstrapping. The results indicate that Digital Leadership has a positive and significant effect on healthcare worker performance (β = 0.218; p < 0.001). Employee Engagement exerts a positive and significant influence, representing the strongest effect (β = 0.421; p < 0.001), while Organizational Support also shows a positive and significant impact (β = 0.287; p < 0.001). Collectively, these three variables explain 68.4% of the variance in healthcare worker performance (R² = 0.684). These findings demonstrate that enhancing performance in the digital transformation era requires adaptive digital leadership, high levels of employee engagement, and sustained organizational support. The study offers implications for hospital management in designing digital-based human resource development strategies

Abstract

Digital transformation in midwifery education requires students to possess adequate digital literacy to effectively engage in technology-based learning processes. This study aims to develop and evaluate a digital literacy learning product and to analyze the impact of digital literacy on the learning readiness of midwifery students in the technological era. The study employs a Research and Development (R&D) approach comprising stages of needs analysis, product design, development, expert validation, limited trials, and effectiveness testing. The research subjects consisted of 100 midwifery students whose digital literacy and learning readiness were measured before and after using the product. Data were collected using digital literacy and learning readiness instruments that had undergone validity and reliability testing. Analysis was conducted using descriptive statistics, paired t-tests, Pearson correlation, and multiple linear regression. The results indicate that the developed learning product was rated as highly feasible by experts. Scores for digital literacy and learning readiness increased following the intervention. Bivariate analysis revealed a positive and significant relationship between digital literacy and learning readiness, while multivariate analysis showed that specific dimensions of digital literacy collectively contributed to students' learning readiness. These findings suggest that the systematic development of digital literacy can serve as a crucial strategy to enhance midwifery students' readiness for increasingly digitized learning and healthcare practice

Abstract

Digital transformation in healthcare requires nursing management to adopt information technology capable of enhancing the effectiveness of resource management, service coordination, documentation accuracy, and decision-making quality. This study aims to develop an information technology-based nursing management innovation model that is effective, user-friendly, and aligned with the operational needs of nursing services. The study employs a Research and Development (R&D) design comprising stages such as needs analysis, design, product development, expert validation, revision, limited trials, and product evaluation. Needs analysis was conducted by identifying nursing management issues and user requirements, while validation involved experts in nursing management, health information technology, and nursing practice. The developed product is a nursing management information system that integrates nurse scheduling, nursing care documentation, workload monitoring, service indicator reporting, and the provision of information to support managerial decision-making. Development results indicate that this information technology-based innovation has the potential to improve management process efficiency, information accessibility, documentation accuracy, and staff coordination. The product also received favorable feasibility ratings based on expert validation and user trials. Thus, information technology-based nursing management innovation can serve as a strategic solution to support nursing services that are effective, integrated, adaptive, and focused on quality improvement and patient safety.

Abstract

Patient safety is a crucial aspect in improving the quality of healthcare services, particularly in nursing services that interact directly with patients. Risk management is essential for identifying, preventing, and controlling various potential risks that can lead to adverse events. This study aims to explore the implementation of risk management in nursing services as an effort to improve patient safety. The study employed a qualitative method with a descriptive qualitative design. Informants were selected using purposive sampling based on their involvement and experience in nursing services and patient safety. Data were collected through in-depth semi-structured interviews, observation, and documentation. Data were analyzed using thematic analysis through coding, category grouping, theme identification, and data interpretation. Data validity was verified through source and technique triangulation and member checking. The results revealed five main themes: nurses' understanding of risk management and patient safety, risk identification and prevention in nursing services, communication and reporting of patient safety incidents, barriers to implementing risk management, and strategies for improving patient safety through strengthening a culture of safety. Barriers identified included workload, limited staff, suboptimal communication, time constraints, and concerns about the consequences of reporting incidents. Effective risk management requires nurse involvement, open communication, a supportive reporting system, ongoing training, and organizational and leadership support.

Abstract

Background: Nurses' workload is one of the factors that can influence the quality of service and the implementation of patient safety. High workload can increase the physical and mental demands of nurses, thus potentially influencing the formation of a patient safety culture in hospitals. This study aims to analyze the relationship between nurses' workload and patient safety culture in hospital inpatient wards. Methods: This study used a quantitative design with a cross-sectional approach. The study sample consisted of 150 nurses working in inpatient wards and were selected according to the study's inclusion criteria. Data collection was carried out using a nurses' workload questionnaire and a patient safety culture questionnaire that had been adjusted to the characteristics of nursing services. Data analysis included univariate and bivariate analyses. The Spearman correlation test was used to determine the relationship between nurses' workload and patient safety culture with a significance level of α = 0.05. Results: Most nurses had a moderate workload, namely 78 respondents (52.0%), while the patient safety culture was mostly in the sufficient category, namely 83 respondents (55.3%). Spearman's test results showed a significant relationship between nurse workload and patient safety culture (r = -0.386; p < 0.001), with a negative relationship and moderate strength. Conclusion: There is a significant relationship between nurse workload and patient safety culture. Workload management, staffing adequacy, and strengthening a safety culture require hospital management attention.
