Abstract

Pedophilia is a frequent case in the world including Indonesia. This makes Indonesia a pedophilia emergency. The data on child abuse based on Commission Child Protection Indonesia (KPAI) in 2010-2014 was increased 42-58 percent of pedophilia. Besides, the victims will eventually become pedophiles that are called victim-to-abuser cycles. Recently, it needed the successful key to therapy against dystonic ego of a pedophilia abuser with Raja Yoga meditation. Meditation is a strategy that has been used to control libidinal (id) and self-regulatory (superego) used in mental health counseling and psychiatry because it can decrease arousal, anxiety, depression, and behavioral disorder. One type of meditation is Raja Yoga meditation that has been studied for years of ability to change behavior and self-regulatory. Supported by several studies, this meditation also can repair abnormalities in the limbic system. Raja Yoga meditation is effective therapy and prevention in pedophilia.

Abstract

Introduction: Preeclampsia is a pregnancy disorder characterized by significantly increased blood pressure (systolic pressure ?140 mmHg, diastolic pressure ?90 mmHg) induced by pregnancy and proteinuria. Preeclampsia is often associated with hyperglycemic state, and studies found that hyperglicemia increases the risk of preeclampsia up to three times higher.Discussion: There are correlations between insulin resistance and glucose intolerance with preeclampsia, shown by the statistics that 20% and 15% of pregnant women with DM Type 1 and Type 2 respectively will develop preeclampsia. Excessive glucose level could induce intracellular changes in cytotrophoblasts that causes placental abnormalities and leads to preclampsia. Recent studies found that hyperglicemia treatment could reduce the risk of preeclampsia up to 54% (RR:0,46;95% CI:0,22-0,97). Turmeric extract contains curcumin that is known for its antidiabetic activity. Curcumin can reduce the insulin resistance and glucose intolerance by inhibition of ROS production to maintain the viability of islet cells, suppression of inflamatory cytokines (TNF-?, IL-6, NFkB) production, stimulation of glucose transporter (GLUT-4) expression and glucose clearance, and suppression of FFA level.Conclusion: Curcumin is potential to be used as a new modality to prevent preeclampsia on women with insulin resistance and glucose intolerance, but the exact amount of turmeric extract or curcumin concentration needed to achieve the desired effect is still unknown.

Abstract

This article is aimed to know the role of soluble growth stimulation gene-2 (sST2) as a prognostic factor in patients with severe preeclamsia. A search was conducted on Pubmed, GoogleScholar and ScienceDirect based on selected clinical question. The articles selection was based on inclution and exclution criteria. Three articles were critically appraised for its validity, importance and applicability. All three choosen articles state that higher sST2 plasma is correlated with overall survival patients with severe preeclamsia. SST2 level may serve as a prognostic factor regarding overall survival in patients with severe preeclamsia.

Abstract

Introduction: Autoimmune disease is immune response that damages self-tissues and disrupts physiologicalfunctions of the body. The prevalence of autoimmune diseases has continued to increase globally since World WarII. In US, this disease is the leading cause of death and commonly seen in women. Conventional autoimmunemedicine suppresses most of immune system and causes patients to be more susceptible to opportunistic infectionsand cancer. Based on these facts, therapeutic innovations that are specific to pathological immune cells are needed.Molecular therapy using stable, target-specific Tregs engineered is potential as the latest modality for autoimmunetherapy.Discussion: Autoimmune occurs when adaptive immune response attacks self-antigens. The autoimmunemechanism is then followed by activation of a large number of T cells. All activated T cells are controlled by Tregulators or Tregs. Tregs have weaknesses which are low selectivity and stability. For this reason, Tregengineering with selective receptor expression and Treg stabilizer is needed. The selective receptors that can bemodified with T cells are Chimeric Antigen Receptor (CAR). CAR is known to selectively inhibits pathologicalimmune responses and stops inflammation. CAR is combined with Treg stabilizer which gives the effect ofamplification and stabilization, that is Forkhead box P3 (FoxP3). FoxP3 is core protein that functions as the mainregulator of Tregs development and self-tolerance.Conclusion:CAR/FoxP3-Engineered Tregs express FoxP3 that have strong affinity for self-antigens through CARreceptors. The application of CAR/FoxP3-Engineered Tregs increases the selectivity and stability of Treg cells tosupress autoimmune.

Abstract

Introduction: Leprosy is a chronic infectious disease that caused by M. leprae, which has predilections to skin and peripheral nerve. WHO predicts that there are aproximately 12 million people world-wide suffered from leprosy, and almost 80% come from tropical countries. Nowadays, leprosy becomes main problem from medical and social views. Prevention of leprosy appears to be a promising strategy in order to eradicate leprosy. In this case, vaccination is the most suitable strategy. Unfortunately, these day vaccines only have protective efficacy less than 70%. More proactive strategies are necessary in order to develop specific vaccine which enhance immune system against M. leprae.Method: This study used literature reviews, using database from www.pubmed.com and scholar.google.com that met the inclusion criteria. From 108 journals, we obtained 78 journals.Discussion: MIP is well-known to be an effective vaccine that has been through clinical trial test phase III. MIP has shown some cross-reactive component antigen which can enhance T cell response to M. leprae. Furthermore, the combination with fucoidan microsphere and antigen specific ML0405 which has predilection for machrophage and as immunomodulator, can maximize the ability of MIP as vaccine to prevent leprosy. Product of combination MIP specific antigen ML0405 and fucoidan microsphere called i-LEPRA. Conclusion: However, further research is needed to identify i-LEPRA efficacy and efficiency. Therefore, i-LEPRA is expected to be effective and efficient as an innovative vaccine against M. leprae in the future.

Abstract

Introduction: Atherosclerosis known as the cause most of cardiovascular disease. Atherosclerosis is a complex pathological process that occurs in the blood vessel. This disease can be triggered by lipid modification, which cause chronic inflammation, eventually lead to thrombosis or stenosis.Result and Discussion: Capsaicin loaded nanoliposome was constructed by extracting capsaicin from chili (Capsicum frutescens L.) using Microwave Assisted Extraction (MAE) method. Then the capsaicin extraction results were constructed with nanoliposome using a thin film hydration method. Capsaicin has LD50 in male Swiss albino mice at 190 mg/kg per oral. Once administered orally, hydrophobic capsaicin will pass through the plasma membrane of endothelial cells and lead to the receptor side of the intracellular vanilloid-binding site reseptor TRPV1 (Transient Receptor Potential Vanilloid 1). The activated TRPV1 is capable of increasing the calcium ion influx which affect the increased expression of ATP-binding cassette transporter A1 (ABCA1) and the decreased expression of LRP1 (low-density lipoprotein protective protein 1). Due to increased Ca2+ influx, endothelial TRPV1 activation also increases PKA phosporilation (protein kinase A) which implies increased UCP2 expression resulting in decreased ROS formation and foam cell formation in atherosclerosis.Conclusion: Capsaicin loaded nanoliposome has potential in the management of atherosclerosis.

Abstract

Introduction: Burns caused by heat, radiation, electricity, and chemical exposure. Because of the high of mortality and morbidity on this cases, treatments should be concern. Prevalences of burns in Indonesia are 0,7% and the highest risk are children.Discussion: Dressing Silver sulfadiazine (SSD) is baku emas in management burn healing wound. However SSD can delay burn wound healing and increase scar hypertrophy formation. Honey has antiinflamatory, antibacterial effect, and accelerate burn wound healing, so that scar hypertrophy become smaller.Conclusion: Based on some study, dressing honey give better effect than dressing Silver sulfadiazine in burn wound healing.
