Abstract

Introduction: 14-3-3 protein plays an important role in protecting cardiac cells from hypertrophy and endoplasmic reticulum stress during pressure overload elicited by aortic banding (AB) surgery; however, the relation among these protective roles is largely unknown. Methods: We investigated the in vivo role of 14-3-3 protein in two protocols involving C57/BL-6 mice and dominant negative (DN) 14-3-3η mice subjected to three- or seven-days pressure overload stimulation by applying AB surgery. The protein expressions of cardiac hypertrophy and ER stress markers, including atrial natriuretic peptide (ANP), galectin-3, glucose-regulated protein (GRP)78, calreticulin as well as 14-3-3 protein was analyzed by western blot. Results: Three- or seven-day pressure overload stimulation significantly increased the heart weight/body weight (HW/BW) ratio; cardiomyocyte diameter; and the protein expression levels of ANP, GRP78 as well as 14-3-3 in the C57/BL-6 mice. Partial inactivation of 14-3-3 protein in the DN 14-3-3η mice significantly increased the protein expression of ANP, Galectin-3, GRP78, and calreticulin after three- or seven-days AB surgery. Conclusion: These results suggest that 14-3-3 protein, as a molecular chaperone, protects against pathological cardiac hypertrophy, at least in part, by maintaining the normal ER function through the regulation of GRP78 and calreticulin.

Abstract

Background: On December 31, 2019, WHO China Country Office reported a case of pneumonia of unknown etiology in Wuhan City, China. On January 7, 2020, China identified pneumonia of unknown etiology as a grew type of coronavirus (coronavirus disease, COVID-19).Case report: A 39-year-old woman – health care worker - came to the hospital with the chief complaint of coughing three days earlier. Dry cough with no phlegm, fever and no shortness of breath. There are also no other complaints. On physical examination found no abnormalities. There is no history of traveling abroad but there is a history of contact with sufferers of COVID-19 when treating patients. Patients diagnosed as acute respiratory infection were given cefixim 2 x 200 mg for 5 days, paracetamol if necessary and vitamins. Chest X-ray examination is normal while CT X-ray examination without contrast shows a picture of ground glass opacity (GGO). The PCR swab was found to be positive. The patient was diagnosed as positive for COVID-9 with Mild Pneumonia. The patient underwent independent isolation by taking the hydrochloroquin sulfate on day 1: 2 x 2 tab, days 2 - 14: 2 x 1 tab. In addition, the patient also took 1 x 1000 mg of Vitamin C supplements, 1 x 200 IU of vitamin D3 and 1 x 20 mg of Zinc.Results:Clinical symptoms are getting better. On the chest CT scan, the GGO image has improved. The PCR swab turned negative. The patient with COVID-19 with Mild Pneumonia cured.

Abstract

Objectives/Background: The aim of this study was to investigate nutritional habits, biochemical markers and the allele and genotype frequencies of GCK rs4607517 -30G>A polymorphism of patients with Type 2 Diabetes Mellitus (T2DM).Methods:  The study was conducted with 200 T2DM patients and 100 controls. The analysis of GCK Gene rs4607517 -30G>A polymorphism was performed by Real-Time PCR method. The Attitude Scale for Healthy Nutrition was applied to the intervention group. Additionally, the sociodemographic data of the patients were collected, anthropometric measurements were performed and their biochemical markers were examined.Results: The mean duration of diabetes of the patients was 7.94±5.81 years, the mean age 55.88±12.23 years, the mean BMI 30.14±5.91 kg/m2, and the mean HbA1c value was 7.74±1.97. There was no difference between allele and genotype frequencies of two groups (p>0.05). The anthropometric and biochemical values of the patients with different genotype was similar (p>0,05). Furthermore, as the HbA1c values of the patients increased, the Malnutrition score was found to increase, and there was a statistically significant found between the groups (p=0.012). Conclusion: Although there are studies related to the effect of rs4607517 polymorphism in the GCK gene on the development of T2DM, the fact that the frequency of polymorphism of the patient group in our study was similar to the control group makes it necessary for the variant to be screened in larger patient populations. The majority of patients with T2DM were found to have high levels of HbA1c, LDL cholesterol, and triglycerides, inadequate level of nutrition knowledge and were obese.

Abstract

Background : Cardiovascular disease have overtaken communicable disease for being the main killer worldwide. Hypertension or high blood pressure is one of the risk factors. Elderly women statistically have a high prevalence of hypertension. Vitamin D is known to affect blood pressure and many studies showed the association between vitamin D and blood pressure with varying result. This study aim to evaluate the effect of vitamin D supplementation (alphacalcidol) for 12 weeks and change in blood pressure of elderly women patients in community health service clinic (KPKM) Medical Faculty Syarif Hidayatullah State Islamic University of Jakarta (FK UIN Jakarta), with pre and hypertension grade 1 patients in majority. Methods :  Analytical pre-post study design was conducted as a part of an RCT (Randomized Controlled Trial) studies with 106 participants of elderly women (45 intervention group, 61 control group).Results : Vitamin D deficiency in the intervention group significantly decreased post-supplementation from 22.2% to 8.9%. Subgroup analysis showed the decrease in blood pressure is greater in the subgroup middle-age elderly than young-age elderly category. Even though there was a significant change in the diastolic blood pressure in the subgroup of middle-age elderly post supplementation (p=0.03), Mann-Whitney statistical analysis showed that 12 weeks vitamin D supplementation (alphacalcidol) did not significantly effect the pre and post systolic nor diastolic blood pressure (p>0,05). Conclusion : The study did not found significant effect of 12 weeks Vitamin D (alphacalcidol) supplementation on blood pressure in elderly women patients.

Abstract

Background: An acute ST-elevation myocardial infarction (STEMI) anterior occurs due to occlusion of left anterior descending artery (LAD) branch of left coronary artery. The occlusion causing transmural myocardial ischemia which in turn to myocardial infarction due to myocardial injury or necrosis. Not only Primary Percutaneous Coronary Intervention (PCI) should be performed in STEMI less than 120 minutes as the risk of the adverse events would increase if exceeding the total ischemic time, but also the procedure can be high risk in patients with multiple clinical factors or comorbidities which can increase the risk of complications.Case Report: We report a case of a 57 years old man who presented Acute STEMI Anterior, Coronary Artery Disease (CAD) 3 Vessel Diseases (VD), Hypertension Heart Disease (HHD), Chronic Kidney Disease (CKD) on Hemodialysis (HD), with COVID-19 confirmed.Results: The left coronary artery was successfully stented by PCI and Coronary Angiogram (CAG) procedures with TIMI 3 flow, minimal bleeding and others complications.

Abstract

Background: Anemia is a common problem that often occurs in patients with CKD undergoing hemodialysis. 56.5% of hemodialysis patients were still have anemia, while on the other hand, the achievement of hemodialysis.  adequacy only reached 52.7%. Disparity that data is the basis for this research. The aim of this study is to analysis the relationship between the achievement of hemodialysis adequacy with the incidence of anemia in patients with CKD undergoing hemodialysis.Methods: This study uses a descriptive correlation with a cross sectional approach to the analysis of the independent variable (HD Adequacy) and the dependent variable (Anemia). The relationship between these variables was tested using the Chi Square test.Result: The study was conducted on 134 hemodialysis patients aged 13-86 years, the most common vascular access was AV fistula (68.7%), and women were the most respondents (55.2%). The Kt / V value in this study was 1.67 (1.03-2.75) and an adequate one was 54.5%. Adequate URR reached 80.6% with a mean of 75.13 ± 6.38. The incidence of anemia reached 53.7% with the Hb value at 8.93 ± 2.82 mg / dl. The results of the Chi Square test showed that HD adequacy (Kt / V and URR) with the incidence of anemia had P values of 0.670 and 0.217.Conclusion: There is no significant relationship between HD adequacy and the incidence of anemia
