Abstract

Venous thromboembolism (VTE) is a major preventable complication in critically ill patients. Although low-molecular-weight heparin (LMWH) and unfractionated heparin (UFH) are widely used for thromboprophylaxis, comparative data from Indonesian ICUs remain limited. This exploratory retrospective cohort study included 49 ICU patients with Wells score ? 2 who received LMWH or UFH at Haji Adam Malik General Hospital, Indonesia, in 2021. The primary outcome was ICU length of stay (LOS). Because LOS was non-normally distributed, groups were compared using the Mann-Whitney U test, with effect size estimated by the Hodges-Lehmann (HL) median difference. Of 49 patients, 41 received UFH and 8 received LMWH. Median ICU LOS was 5 days (IQR3–11) with UFH and 4 days (IQR 2–7) with LMWH. The HL median difference was-1.0 day (95% CI-5.0 to 3.0; p = 0.288), indicating no statistically significant difference. Wide confidence intervals reflected substantial uncertainty due to the small sample size and severe group imbalance. As LOS included patients who died during ICU admission, early mortality may have shortened LOS irrespective of treatment. No significant difference in ICU LOS was detected between LMWH and UFH groups. Findings are exploratory and hypothesis-generating only; larger prospective studies with severity adjustment are needed.

Abstract

Dysmenorrhea is one of the most common gynecological complaints among adolescent girls and may negatively affect daily activities, school attendance, and quality of life. This study aimed to examine the as sociation between exercise habits and the occurrence of dysmenorrhea among adolescent girls in Banda Aceh, Indonesia. An analytical cross-sectional study was conducted among 96 female students aged 15–17 years from a public senior high school in Banda Aceh. Participants were selected using purposive sampling. Data were collected through structured questionnaires assessing exercise habits, including regularity, frequency, duration, and type of exercise, as well as dysmenorrhea status. Associations were analyzed using the Chi-square test, and crude odds ratios (ORs) with 95% confidence intervals (CIs) were calculated. The results showed that dysmenorrhea was highly prevalent among respondents. No significant associations were found between dysmenorrhea and exercise regularity (p = 0.289), exercise frequency (p = 0.203), or type of exercise (p = 0.398). However, exercise duration was significantly associated with dysmenorrhea (p = 0.026). Adolescents who engaged in physical activity for at least 30 minutes per session had lower odds of experiencing dysmenorrhea compared with those exercising for less than 30 minutes (OR = 0.31; 95% CI: 0.11–0.90). These findings suggest that longer exercise duration may be associated with a lower occurrence of dysmenorrhea among adolescents. Given the cross-sectional nature of the study, the observed relationships should be interpreted as associations rather than causal effects.

Abstract

Postpartum blues is a common emotional disturbance during the early postpartum period and may progress to postpartum depression if left unmanaged. SAFE education has emerged as a non-pharmacological approach to improve maternal emotional preparedness. This study systematically evaluated the effectiveness of SAFE education in preventing postpartum blues through a Systematic Literature Review (SLR). Literature searches were conducted in PubMed, Scopus, and Google Scholar for studies published between 2021 and 2025. Following PRISMA 2020 guidelines, 309 records were initially identified from three electronic databases. After the removal of 102 duplicate records, 207 unique articles underwent title and abstract screening. Of these, 112 full-text articles were assessed for eligibility, and 12 studies involving approximately 1,900 participants fulfilled all predefined inclusion criteria and were included in the final qualitative synthesis. The findings consistently demonstrated that SAFE education reduced postpartum blues symptoms, anxiety, stress, and mild depressive symptoms while improving maternal knowledge, self-efficacy, emotional preparedness, coping skills, and perceived social support. Due to substantial heterogeneity in study designs, interventions, and outcome measures, a narrative synthesis was performed instead of a meta-analysis. Overall, SAFE education appears to be a promising antenatal intervention for promoting maternal mental health and reducing the risk of postpartum blues.

Abstract

Outpatient waiting time is a critical service-quality indicator in Indonesian hospitals. At Hospital X, Sidoarjo, the Internal Medicine Clinic's average wait time reached 83 minutes and 58 seconds, approximately 40% above the 60-minute standard. This qualitative study applies in-depth interviews, fishbone analysis, and Lean Six Sigma (LSS) to identify root causes and formulate improvement recommendations. Findings identify systemic waste and defects, including complex B1–B6 clinical assessments, electronic medical record (EMR) constraints, high staff workloads risking burnout, and unreliable wait-time estimates despite an effective online registration system. In the short term, implementing e-Signatures to eliminate physical documentation workflows is identified as the most immediately actionable intervention. Ambient AI Scribe and AI Predictive Analytics are proposed as longer-term, infrastructure-contingent interventions requiring contextual validation before adoption. Reducing waiting times below 60 minutes through this combined strategy is operationally plausible but remains an unvalidated projection requiring further quantitative confirmation.

Abstract

Adolescent reproductive health remains a major public health concern, particularly in developing countries where access to accurate information is often limited. This study examined the association between smartphone utilization and reproductive health knowledge among female adolescents in Aceh Besar, Indonesia. A quantitative cross-sectional study was conducted in May 2025 involving 81 female students selected through proportional stratified random sampling. Data were collected using structured questionnaires and analyzed using binary logistic regression. Overall, 72.8% of respondents used smartphones to access reproductive health information, and 55.6% had good reproductive health knowledge. Smartphone use for reproductive health information was significantly associated with good knowledge (OR = 2.98; 95% CI: 1.33-6.67; p = 0.001). Duration of smartphone use (p = 0.004) and social media platform (p = 0.006) were also significantly associated with knowledge. Smartphones may serve as effective channels for improving reproductive health knowledge among female adolescents, although causal relationships cannot be established due to the cross-sectional design.

Abstract

Post-cesarean anemia burdens low- and middle-income settings, yet oxytocin massage’s hematological effects remain underexplored. We evaluated its 48-hour impact on hemoglobin and hematocrit. Quasi-experimental pretest–posttest study at a tertiary hospital in Tasikmalaya, Indonesia (May - October 2024). Sixty-three post-cesarean mothers were allocated by ward to oxytocin massage (15 min at 12 and 48 h postpartum) plus standard care (n = 33) or standard care alone (n = 30). The 48-h blood draw preceded the second massage session to avoid peri-session effects; intraoperative blood loss was not systematically recorded. Co-primary outcomes (hemoglobin, hematocrit) were analyzed by baseline adjusted ANCOVA with Benjamini–Hochberg correction. All 63 mothers completed the study. Adjusted hemoglobin rose (+0.83 g/dL, 95% CI +0.11 to +1.56, p = 0.028, eta^2p = 0.078) and hematocrit (+3.68%, 95% CI +1.34 to +6.03, p = 0.003, eta^2p = 0.136); both survived correction. Critically, the hemoglobin gain fell below the informal MCID (˜1.0 g/dL); its lower confidence limit approached null (E-value 1.36), signaling residual unmeasured confounding. Lochia volume decreased (p < 0.001; unblinded); fundal height did not differ. Oxytocin massage may aid short-term hematological recovery, but the sub-MCID hemoglobin effect, cesarean-indication imbalance (fetal: 100% vs 39.4%), and unrecorded blood loss render findings preliminary. Adequately powered randomized trials are needed before adoption.

Abstract

Thirst and xerostomia are distressing symptoms in hemodialysis (HD) patients. This study examined the association between sipping ice therapy and these symptoms among adult HD patients. A quasi-experimental pretest–posttest study with a non-equivalent control group was conducted at Banjar Hospital, Indonesia. Forty adults were allocated via purposive sampling to an intervention (n = 20) or control group (n = 20). The intervention group received sipping ice therapy (5 ?mL ice cubes, up to eight daily) for six consecutive days at home, while controls received routine care. Symptoms were measured using the Dialysis Thirst Inventory and Summate Xerostomia Inventory. The intervention group demonstrated significant reductions in median thirst and xerostomia categories from severe to mild (both p < 0.001), whereas no significant changes occurred in controls. Posttest between group comparisons strongly favored the intervention for reducing thirst (p < 0.001,r = 0.91) and xerostomia (p < 0.001,r = 0.88). Sipping ice therapy substantially reduces thirst and xerostomia in adult HD patients, serving as a feasible, non-pharmacological adjunct in nephrology nursing care. Future multi-center randomized trials are warranted.

Abstract

Perineal trauma following vaginal delivery is a common postpartum complication that may delay recovery and increase the risk of adverse maternal outcomes. This study examined factors associated with perineal wound healing among postpartum women in Bireuen District, Indonesia. A cross-sectional study was conducted from December 2025 to April 2026 involving 120 postpartum women assessed 10-14 days after normal vaginal delivery. Participants were recruited consecutively from the Jeumpa, Gandapura, and Kutablang Primary Health Centers. Perineal wound healing was evaluated through clinical observation, while demographic, behavioral, psychosocial, and cultural characteristics were collected using structured questionnaires. Associations were analyzed using chi-square tests and multivariable logistic regression. In bivariate analyses, type of perineal tear, personal hygiene, cultural practices, and family support were significantly associated with perineal wound healing. In the multivariable model, family support (AOR = 31.21; 95% CI: 2.55-382.12; p = 0.007) and personal hygiene (AOR = 27.74; 95% CI: 2.41-319.86; p = 0.008) remained significantly associated with favorable wound healing. The wide confidence intervals indicate substantial uncertainty in the estimated effect sizes. These findings suggest that personal hygiene and family support are associated with perineal wound healing; however, the cross-sectional design precludes causal inference, and longitudinal studies are needed to confirm these associations.

Abstract

Gestational hypertension (GH) is a common pregnancy complication associated with adverse maternal and fetal outcomes and is often accompanied by anxiety, which may be associated with elevated blood pressure. This study examined the association between participation in a hypnotherapy program and changes in blood pressure, anxiety, and gestational duration among pregnant women with GH. A quasi-experimental pre-test–post-test control group study was conducted among 50 pregnant women with gestational hypertension in Aceh Barat Regency, Indonesia. Participants were recruited using purposive sampling and allocated through a non-random procedure to an intervention group (n = 25), which received structured hypnotherapy in addition to standard antenatal care, or a control group (n = 25), which received standard antenatal care alone. Anxiety was assessed using the Hamilton Anxiety Rating Scale (HARS), while systolic and diastolic blood pressure were measured before and after the intervention. Continuous outcome measures were analyzed using the Wilcoxon signed-rank test for within-group comparisons and the Mann Whitney U test for between-group comparisons. The intervention group demonstrated significantly greater reductions in systolic and diastolic blood pressure and HARS anxiety scores than the control group (*p* < 0.05). Although improvements in categorical blood pressure and anxiety status were observed in both groups, these descriptive findings did not reflect the magnitude of change captured by continuous measurements. No statistically significant between-group difference was found for gestational duration (*p* > 0.05). Participation in the hypnotherapy program was associated with greater improvements in blood pressure and anxiety than standard antenatal care alone.

Abstract

Overweight and obesity among toddlers have emerged as important public health concerns in Indonesia, particularly in coastal areas where dietary patterns and socioeconomic conditions may increase nutritional risk. However, evidence on their determinants in these settings remains limited. This study aimed to identify factors associated with overweight and obesity among toddlers aged 13–60 months in the working area of Puskesmas Munjungan, Trenggalek Regency. A case-control study was conducted involving 136 toddlers, consisting of overweight/obese cases and normal-weight controls selected through proportionate stratified random sampling. Data on dietary intake, early feeding practices, and maternal characteristics were collected using questionnaires and anthropometric assessment. Data were analyzed using Chi-square tests and multiple logistic regression. Backward stepwise selection was applied for exploratory model building, although its limitations related to variable retention stability were acknowledged. The results indicated that nutritional intake, early feeding practices, and maternal characteristics were significantly associated with overweight and obesity among toddlers. Multivariate analysis identified non-exclusive breastfeeding as the factor most strongly associated with overweight and obesity, followed by high energy and macronutrient intake, poor maternal nutritional knowledge, and higher maternal body mass index. Given the retrospective assessment of dietary exposure and feeding history, the possibility of reverse causation cannot be excluded; therefore, these findings should be interpreted as statistical associations rather than evidence of causal relationships. In conclusion, overweight and obesity among toddlers in coastal areas were associated with modifiable nutritional and maternal factors, with early feeding practices showing the strongest association. Strengthening exclusive breastfeeding promotion, improving maternal nutritional knowledge, and encouraging balanced dietary practices may support prevention efforts in coastal communities.
