Abstract

Jemursari Islamic Hospital has a hospital management information system (SIMRS) in 2013 until now. Inpatient installation information system is a system of admission, hospitalization to hospital. It will be slow, from the beginning of SIMRS implementation until now there are still problems found in the implementation of SIMRS. The problem is the service process that still exists a few stages long, inpatient services in the way available still requires more than 15 minutes have been using information systems, incompatibility of inpatient data with the system, mismatch manual manual report on the system and the nonconformity of the class ordered by the patient with the initial order and the patient's information about the medical record.            Therefore, to overcome the problem above the solution is to conduct an information system audit at Jemursari Islamic Hospital of Surabaya by using COBIT 4.1 method. COBIT 4.1  method there are 4 stages of planning, preparation, implementation and reporting audit. Four stages produce a solution of a hospital management information system (SIMRS) and results for this information. The final value of the information system is 3.91 which has a manageable and scalable meaning.

Abstract

Health centre (Puskesmas) I Toili has several health services, one of them is the apotek. The problem with apotek is when restocking medicines, the pharmacist does not know exactly the demand of medicine for the next periode resulting cumulation of medicines that have not yet been sold. Other than that, the phartmacist when selling the medicines sometime does not paying attention on the expire date. The solution for the problems that has been described is to create a program for forecasting demand for the medicine consisting of: forecasting demand for medicine with Double Exponential Smoothing (DES) methode, medicine sale with First Expider Firs Out (FEFO) methode. The result of this research has shown that the program has able to do forecast for medicine demand using (DES), to save the sales of medicine with FEFO methode and medicine restock report, medicine forecast, and the sales of medicine, presented on chart with high accuracy and fast.

Abstract

Bioli Lestari is one of the company that run manufacturing industry. This company have problems at controlling their production schedule to fulfill customer product orders. The problem arise because their schedule based on raw material which available at the time, not based on costumer order. Production schedule problem cause company can’t give information about product finish due to the costumer. Based on problem explained before, a system that can handle customer orders and can make production schedule based on customers order being needed. In this case system use Shortest Processing Time (SPT) as a method to create production schedule, after that system will create production schedule based on customer orders which can help company to answer customers question about product orders finish due and can help company run production process based on customers orders

Abstract

PT Bumi Borneo Perkasa (BBP) is a mining contractors company. process of mining needs help of  heavy equipment. PT BBP has heavy equipment about 150 units. in Condition, the heavy equipment of PT Bumi Perkasa Burneo often damaged because it was late in the replacement of spare parts, and delayed in obtaining a license for heavy equipment, and no reports for maintenance costs of each heavy equipment.The solution the above problems is designing and building maintenance applications of heavy equipment at PT BBP using SDLC method With the first stage is communication, planning, construction modeling. The results based from the application trials, maintenance applications of heavy equipment  has been made can solve the problem in the division of heavy equipment. This Application generate alert of  replacement spare parts by regularly, give an alert to renew permits heavy equipment and provide maintenance cost  reports of each Heavy equipment. maintenance applications of heavy equipment save the data of heavy equipment,  spare parts and heavy equipment license.

Abstract

Employee performance evaluation in Paru Surabaya Hospital consists of two ratings. Those are employee performance appraisal and assessment of employee performance at the working unit. Human Resources (HR) find miscalculation cases on those assessments due to miscalculation of the applied assessment formula. This miscalculation mostly occurred on three to five employees each year. Another problem that also occurs on managerial level is the evaluation report establishment takes one to two weeks in average due to must open the recapitulation file of employee performance evaluation. The applied solution to overcome these problems is developing the application of employee performance appraisal using Key Performance Indicator by looking at four aspects: quantity, quality, time and cost. While the employee performance evaluation at the departement using Graphic Rating Scale method. Based on trial results between manual calculation and application is made to the nine employees who represent their respective departments, the result that there is no difference between manual calculations with the application so that the application can perform calculations accurately. The trial results show a report with twenty employees value the data takes approximately thirty seconds so as to display the entire data value employee report 216 employees take approximately five minutes.

Abstract

Abstract: Local drinking water company which later can be abbreviated PDAM according to the mayor (200 ) is a company belonging to local government that move in the field of drinking water services. PDAM has a duty to pursue the provision of drinking water that meets the terms of health care for people in the city of surabaya and surrounding areas. The problems that occur on PDAM Surya Sembada of surabaya city difficulty in optimize the cost of supplies and economical determine orders to the needs of production. This is because the process of planning supplies raw materials done with see the need of raw materials for the production for a year be submitted to a section of supplies at the end of the current year, insufficient production needs in reality in the next year. Where as all raw materials required for the production of  order done once a year by auction supplier , then raw materials will be sent to PDAM every month.Of the existing problems then required raw materials management planning application needs taps on PDAM Surya Sembada of surabaya city that can help in making planning senior managers supplies of raw materials .This application applies the methods of Triple Exponential Smoothing Winter, Re-Order Point (ROP), and Economic Order Quantity (EOQ). Through this method planning raw materials produced needs to suit the needs accurate production line. In the step of experiment try and evaluation, this research produce raw materials management planning application needs taps on solar system sembada city surabaya. It can be seen than average on a page control supervisor supplies are 4.2, the average conformity on a page planning supervisor supplies are 4.5, the average conformity on a page is 4.3 production supervisor, the average manager conformity on a page supplies are 4.5, and the average senior managers conformity on a page supplies and provision is 4.4. The one generated by the application of the accuracy with the kind of having microsoft excel. This application can be developed with the complement of the security system,use mobile platform, and equipped functional report notifikasi delivery via email and via sms a gateway.

Abstract

PT. Diana Abadi Santosa is a company that one of their business is in motion provide service. PT. Diana Santosa Abadi is constrained in the decision to choose K9 dog that used to resolve something case. In addition the company has not apply a computerized system and is still doing manually. This condition causes the company can not take good decisions in choosing and deciding K9 dog that according to the case or the existing problems. Based on the description above, needed a decision support system in the company. The system consists of the K9 dog needs, and recommend the selection of K9 dog that suits as required. From the test results of decision support systems in PT. Diana Abadi Santosa is concluded that the information system created to help in selection process of K9 dog and recommend the selection of K9 dog as required.

Abstract

Academic information system is a web-based system that can give a student's academic activity reports periodically. Information needed MTsN Pandaan is the start of recruitment of new students until graduation. The problem that occurs is summarizing data they use sheets of paper stored in the main book, which can cause data loss, when it occurred in the reporting of academic data of each period, the information generated will not actual data recap the process becomes inefficient.One of the best solutions is to build a web-based academic information system to facilitate the school in determining the information and manage the data well and up to date. This system can perform maintenance and management of data into useful information to the school about the academic activities of students in the school.The Results by using this system is to reduce errors in the input data, and manage the data well and minimize data loss.
