Abstract

The dissatisfaction of men due to their penile size is found in a number of studies. Concerns can be focused on the penile length or width or even both, whether erect or flaccid. Men who are worried about the penile size also want to increase the size of the penis. Concerns about men’s penile size are also increasing because they think normal penile size is what is shown in pornography. These worries can affect self-esteem, sexual function, and satisfaction, to physical and mental health. Small penis anxiety is an anxiety that arises when a man observes his flaccid penile size and feels concerned that his penile size is less than the normal size. In addition, this concern persists even though it has been refuted by clinical examination. Penile dysmorphic disorder is excessive self-concern, distress, and a preoccupation with trivial or even non-existent deficiencies in penile size and shape that can lead to impaired function. Until now, there have not been effective guidelines for managing patients with complaints of penile size. Some of the treatments that can be conducted are psychosexual counseling, cognitive behavioral therapy, the use of selective serotonin reuptake inhibitors, penile traction devices, and invasive procedures. Various modalities of therapy have been tried to overcome excessive concern about penile size. Yet, so far, no good results have been obtained. Thus, clear and effective guidelines are needed to make treatment can be carried out properly.

Abstract

Osteoporosis is a chronic metabolic disorder of the musculoskeletal system associated with reduced bone strength. One of the causes of secondary osteoporosis is diabetes mellitus (DM). The prevalence of both disorders keeps increasing with time. Therefore, this review is conducted to find a possible solution to prevent DM-induced osteoporosis. Diabetes mellitus mainly affects the bone through glucose uptake during the bone remodeling process. Glucose uptake through GLUT 1 is regulated by MST 1, which is an upstream kinase of the Hippo signaling pathway. MST 1 is responsible for regulating cell growth, proliferation, and apoptosis. In the bone remodeling process, MST 1 plays a role by regulating actin ring structures and the integrin signaling pathway. Moreover, DM is also associated with increased oxidative stress. Increased oxidative stress will activate Hippo signaling pathway. This will trigger cellular apoptosis as the Hippo signaling pathway plays a role mainly as a tumor suppressor. Increased cellular apoptosis will cause an imbalance in the bone remodeling process, disrupting bone quality. Inhibition of MST 1 through the Hippo signaling pathway will increase cell growth and reduce cellular apoptosis. Increased cell growth might increase osteogenesis during the bone remodeling process, thus resulting in better bone quality in DM-induced osteoporosis.

Abstract

Infertility is the inability of couples who have regular sexual intercourse without using protection to achieve pregnancy within one year. The prevalence of infertility was 21.3% in Indonesia. Intrauterine Insemination (IUI) is an assisted reproductive technology chosen by infertile couples to have children. Various factors influence the success of IUI. One factor that influences the success of IUI is the female partner’s age. To determine the relationship between the age of female partners and the success of IUI. This study is descriptive-analytical research with the Cross-Sectional method using secondary data obtained from medical records of patients who underwent IUI procedures from January to March 2020 at Rumah Sakit Ibu dan Anak Puri Bunda Denpasar. Total research subjects are 31 patients with an average age of 32 years. Positive pregnancies in female partners aged 25-35 years are 8 patients (33,3%) and female partners aged 35-40 are 1 patient (14.3%). Most patients had secondary infertility with a duration of 1-3 years. There is no relation between the increasing age of female partners and the decreasing success of IUI at Rumah Sakit Ibu dan Anak Puri Bunda Denpasar (p=0,329). But female partners aged 25-35 years tended to have a positive pregnancy 2,333 times higher than female partners aged 35-40 years (RR=2,333; CI 95%: 0,349-15,612).

Abstract

Infertility can occur in men, characterized by the inability to make pregnancy in the partner. The causes of male infertility are multifactorial, one of which is obesity. Obesity parameter by body mass index >25 kg/m2. This study purposes of determining the relationship between obesity with body mass index measurement and male infertility. This type of research is an analytical cross-sectional study. This research was located at the Kasih Ibu General Hospital Denpasar. Purposive sampling was chosen as the sampling technique. The samples were 50 male patients aged 25-45 years who checked themselves at the Kasih Ibu General Hospital Denpasar between January and December 2020. All research data could be seen from the patient's medical records. The Pearson Chi-Square test was used to analyze the data. The results were 26 subjects (52%) with BMI >25 kg/m2 (obese) and 24 subjects (48%) with BMI <25 kg/m2 (non-obese). About 28 subjects (56%) were infertile, and 22 subjects (44%) were fertile. This study found that body mass index was significantly associated with infertility in male patients at Kasih Ibu General Hospital Denpasar. Obese men had a 2,308 higher risk of infertility than men were not obese (p = 0.002, RR = 2.308 (CI95% 1.262-4.220). The study has found that obesity may contribute to the event of male infertility. For further research should use a larger sample size to make it more accurate in population.

Abstract

Background: Around 10 % of infertile men and 1 percent of all males have azoospermia. There are two types of azoospermia, which are obstructive and non-obstructive azoospermia. Non-obstructive azoospermia's main mechanism is because the testes fail to produce the sex hormone and induce spermatogenesis (primary testicular failure). Case: A patient is 28 years old and has a job as a car paint worker. He came with the chief complaint of infertility since two and a half years ago. He and his wife were having intercourse 3-4 times a week. Past medical history is unremarkable. His wife’s medical history is also unremarkable. Physical examination and ultrasound of the testes are normal. The semen analysis in this patient was azoospermia for 2 different times in the span of 2 weeks with no abnormalities in the accessory gland. Hormonal profiles results are testosterone level 2,32 ng/mL and FSH 15,03 mIU/mL, which indicatehypergonadotropic hypogonadism. The patient was suggested to evaluate further (complete hormonal profile, karyotyping analysis, and Y-Chromosome microdeletion) and educate about the possibility to conceive with assisted reproductive technology (ART). Discussion: Hypergonadotropic hypogonadism is a challenging case that needs a complete assessment such as complete hormonal profile, karyotyping analysis, Y-chromosome microdeletion analysis, and also, in this case, the paint thinner exposure in the workplace is needed to be considered. The chance of normal conception is very small, and the assisted reproductive procedure is necessary. Conclusion: Some abnormalities are usually present in the physical examination of azoospermia patients. This case convinces us of the importance of thorough history taking and other investigations. Managing this patient will be challenging, with the goal of the therapy is to achieve spermatogenesis to be able to use the spermatozoa available for ICSI.

Abstract

The incidence of obesity in the world has increased recently and doubled from 1980 to 2014. Obesity can increase the risk of many diseases. Medical students are a vulnerable group affected by obesity because of stressful conditions that can increase the incidence of obesity. This analytical research uses a cross sectional method approach using primary data. The populations in this study were medical students at the Faculty of Medicine, Airlangga University. In this study, 300 samples were taken from first-fifth semester students. The independent variables that will be studied in this study are Gender, physical activity, family obesity history, and consumption of fruit or vegetables. The dependent variable in this study is nutritional status. Based on this study found a significant association between gender, family history of obesity, eating of fruits or vegetables to nutritional status. Still, there is no significant association between physical activity on the nutritional group of medical students. Three variables in this research correlated with the nutritional status of medical education students at the Faculty of Medicine, Airlangga University, Surabaya, which are gender, family obesity history, and consumption of fruit or vegetables. From this research, it was found that the incidence of over-nutrition was still high among medical education students and there was still a lack of awareness among students to exercise and consume fruit or vegetables in their meals.
