Hallucinations are the client's perception of the environment without any real stimulus, which means the client interprets something real without external stimulus. The purpose of this writing is to describe the nursing management of sensory perception disorders: auditory hallucinations in Mr. N and Mr. R at RSJ Palembang. The method used is to provide management in the form of patient care to control the auditory hallucinations they experience. Patient management was carried out for 5 days for Mr. N and Mr. R. Data collection techniques were carried out using interview techniques, physical examination and observation. Management results showed that clients were able to control the auditory hallucinations they experienced, namely by shouting, having conversations, scheduled activities and taking medication. Clients are able to overcome the hallucinations they experience as a whole so that the treatment given can provide optimal results. Suggestions for RSJ are expected to improve nursing care for sensory perception disorders: hallucinations by involving clients in various activities in the room so that clients remain focused and do not get lost in their hallucinations

Violent behavior is a response to stressors faced by a person, by carrying out acts of violent behavior, either towards oneself, other people or the environment, verbally or non-verbally. Acts of violent behavior aim to injure other people physically or psychologically, as seen in the behavior. actually committing violence. Clients need to receive treatment, namely implementation in the form of one implementation strategy for clients in the Cempaka room. This research uses a descriptive method in the form of a case study to explore the implementation of nursing deep breathing exercises in schizophrenic patients with violent behavior problems at the Ernaldi Bahar Hospital, South Sumatra Province. The subjects of this research were two patients, namely schizophrenic patients with violent behavior problems. Carried out for seven days on April 8-15 2021. The results of this study show that deep breathing exercises are effective in controlling violent behavior in schizophrenia patients..

Medication adherence is the level of individual accuracy with medical advice that describes the use of medication according to the instructions and instructions on the prescription, including using it at the correct time. Patients who are not compliant in taking medication tend to experience relapses easily. Frequent relapses in sufferers result in bad effects on the brain which makes it difficult for sufferers to recover. This research aims to obtain an overview of the level of adherence to taking medication and provide knowledge about the importance of taking medication in patients with sensory perception disorders, auditory hallucinations. This case study research is descriptive in nature, with an implementation method using nursing care which was carried out on 8-15 April 2021, the case study subjects were two patients with sensory perception disorders, auditory hallucinations in the Cempaka ward of the Ernaldi Bahar Hospital, South Sumatra Province. After implementing it on patients, the result was that taking medication regularly was very effective in controlling hallucinations because patients were taught to know the five correct principles in taking medication, namely the correct patient, the correct drug, the correct dose, the correct method/route of administration, and the correct time for administering the drug. where compliance occurs if the rules for taking prescribed medication and administering it in the hospital are followed correctly

Self-care deficit is a person's inability to meet basic self-care needs independently. The purpose of the study was to achieve an overview of the implementation of adequate self-care needs in patients with self-care deficits. Descriptive research, with an implementation method using nursing care, with an assessment process, interviews, and direct observation carried out from 12 to 18 April 2021, The subjects of the case study were two schizophrenic patients with self-care deficit problems in the Bangau Room of Ernaldi Bahar Hospital, South Sumatra Province. The results of this study indicate that the provision of self-care needs is very effective in overcoming the problem of self-care deficits.

Hospitalization causes anxiety in children and hinders the healing process, distraction techniques: story telling is a nursing action that is able to distract anxiety and is able to create a comfortable environment so that the level of anxiety experienced by children can decrease. Method: This type of research design uses a descriptive design in the form of a case study. The approach used is nursing care which includes assessment, nursing diagnosis, planning, implementation, evaluation and documentation. The case study subjects used were 2 pediatric patients suffering from diarrhea aged 4 years for patient 1 on March 18 2022 and aged 6 years for patient 2 on March 23 2022 who had the same nursing problem, namely anxiety. This research was conducted at Siti Fatimah Regional Hospital, Palembang in 2022. Results: The implementation of nursing in the form of distraction techniques: story telling has an effective effect on reducing the anxiety experienced by pediatric patients during treatment. The results show that the patient's anxiety score has decreased significantly and determine the patient's anxiety level before and after administering the distraction technique. Conclusion: Providing the distraction technique: story telling with anxiety problems was effective in reducing anxiety, there was a significant reduction after using the distraction technique: story telling for three days.

Based on medical record data from Siti Fatimah Hospital Palembang, children aged 6-12 years who suffered from diarrhoeal diseases in 2019 to 2021 were 28 patients. Diarrhoea in children must be treated immediately because diarrhoea that is too frequent in children can cause disruption of the gastrointestinal system such as malabsorption which can cause children to enter less nutrients into the body and can cause nutritional deficits in children with diarrhoea. Methods: This type of research design uses a descriptive design in the form of a case study. The approach used is a nursing care approach which includes assessment, nursing diagnosis, planning, implementation, evaluation and documentation. The case study subjects were 2 pediatric patients suffering from diarrhoea, patient 1 aged 6 years and patient 2 aged 7 years who had nutritional deficit nursing problems. This research was conducted at Siti Fatimah Hospital Palembang in 2022. Results: Implementation of nutrition management nursing is effective to overcome the problem of nutritional deficits that occur in children with diarrhoea by showing the results of weight gain in children every day during the nursing process. Discussion: It is hoped that the patient's parents can recognise the signs of diarrhoea in children so that nutritional deficiencies do not occur and it is hoped that parents can apply the nutritional management that the author has taught.

This study aims to explain and evaluate the implementation of nursing care for patients who experience auditory hallucinations. The research method uses a case study approach by collecting data from initial assessments, nursing diagnoses, care plans, nursing interventions, evaluations, and discharge planning. The research results show that an initial assessment is carried out by identifying mental health history, behavior related to hallucinations, and triggering factors that may influence the patient's condition. The nursing diagnosis was determined as sensory perception disorder: auditory hallucinations. The treatment plan involves collaboration with the mental health team, behavioral monitoring, emotional support, and education of the patient and family. Nursing interventions include explanation of the nature of hallucinations, assistance in identifying reality, and use of relaxation and distraction techniques. Evaluations are carried out to assess the patient's response to the interventions provided and monitor changes in the intensity or frequency of hallucinations, discharge planning and provide information regarding warning signs. Conclusion: Nursing care for patients with sensory perception disorders requires a holistic approach and collaboration between nurses and the mental health team. Education for patients and families also has an important role in supporting the management of this condition.

This research aims to examine and design nursing care for two patients who are at risk of violent behavior. Observation, interviews, and analysis of patient data are data collection methods. The results showed that both patients had a history of previous violence, environmental factors, and mental health disorders. The results of the assessment can identify potential triggers, detect the level of risk, and evaluate the patient's social support and coping. The nursing diagnosis of risk disorder for violent behavior in patients underlies a treatment plan that focuses on violence prevention. Nursing intervention takes the form of self-control skills training, adaptive coping strategies, and close monitoring of behavior changes. Periodic evaluations are performed to assess the effectiveness of the treatment plan and detect changes in behavior. Discharge planning is developed by considering family support, social networks, and mental health services in the community to ensure continuity of patient care. The research concluded that nursing care for violent behavior requires a holistic approach involving interprofessional collaboration and ongoing support. Implementation of coordinated prevention strategies can help mitigate the potential for violence exhibited by patients

The aim of this study was to examine the implementation of nursing care for two patients who experienced hallucinatory sensory perception disorders. The research method uses a case study approach with data analysis through medical records, observations, and interviews with patients and families. The results showed that both patients experienced sensory perception disorders in the form of auditory hallucinations. The initial assessment includes identification of mental health history, observation of hallucinatory behavior, and precipitating factors. The nursing diagnosis was determined as sensory perception disorder: hallucinations. The treatment plan involves collaboration with the mental health team, use of drug therapy prescribed by the doctor, as well as nursing interventions such as education about stress management, distraction techniques, and increased coping. Evaluations are carried out periodically to monitor the patient's response to the interventions provided. The conclusion of this study is that nursing care for patients with sensory perception disorders requires a comprehensive approach that includes collaboration between nurses, mental health teams and families in order to help improve the quality of life of patients with sensory perception disorders, especially in managing the symptoms of hallucinations.

The aim of this research is to analyze and design nursing care for two patients, who are at risk of violent behavior. Observations, interviews, and medical record documentation studies are data collection methods. Based on the research results, it is known that both patients have a high risk of violent behavior, especially related to a history of previous violence and underlying mental disorders. Initial assessment includes identification of potential triggers for violence, evaluation of level of coping, and assessment of social support. Risk of violent behavior is the patient's second nursing diagnosis. Treatment plans are implemented with a focus on violence prevention, involving collaboration with a multidisciplinary team, including medical personnel, and psychiatrists. Nursing interventions involve therapeutic approaches to improve coping, interpersonal skills training, and close monitoring for signs of violence. Periodic evaluations are conducted to assess the effectiveness of the treatment plan and identify changes in patient behavior. Discharge planning involves coordination with family and mental health services in the community to ensure continuity of care after discharge. The conclusion of this research is that nursing care for patients at risk of violent behavior requires a careful, proactive and collaborative approach. Implementing integrated violence prevention strategies and close monitoring can help reduce the risk of violent incidents.
