Abstract

Civil servant compliance and assertiveness to non-smoking areas regulation: a comparative study in Sleman and Kulon Progo RegenciesPurpose:his study aimed to determine the differences of compliance and civil servants on the applica­tion of non-smoking areas in Sleman and Kulon Progo.MethodsThis analytical survey study used a cross-sectional approach. The subjects of this research were civil servants in the regional work units of Kulon Progo Regency as many as 1,072 and Sleman as many as 2,300 employees at 26 SKPD in Sleman and 25 SKPD in Kulon Progo. The sampling technique used multistage random sampling. The research instruments used were question­naires, observations, and interviews to support the research. The analysis used was independent t-test and Mann-Whitney tests, namely to determine whether there was a relationship between independent and dependent variables.ResultsThis study found a differ­ence of knowledge on application of perda KTR with (p = 0.000), attitude with (p = 0.003) and there was difference of socialization to applying of regulation of non-smoking area (p = 0.000). Logistic regression tests showed that the dominant factor of the non-smoking area law was the knowledge.ConclusionThere was a difference of knowledge, attitude, and socialization among civil servants in Kulon Progo and Sleman. There was no significant difference in attitude, smoking status, compliance, assertive­ness, accessibility and social environment among civil servants in Kulon Progo and Sleman districts. The dominant factor affecting the compliance and assertive­ness of civil servants is knowledge.

Abstract

Evaluation of community health center's medical waste management system in Bantul RegencyPurposeThis study aimed to evaluate the Primary health centers medical waste management systems.MethodsThis research was a qualitative study using a case study design in five Puskesmas of Bantul Dis­trict.ResultsThe manage­ment of medical waste in Bantul Regency is a new model using private party as a user (health cooperative) to hire services to the private transporter (CV. Jogya Prima Per­kasa) to carry out the transportation and destruction of medical waste generated by health facilities of Puskesmas, Pustu, Polindes and private health facilities. Con­sideration of Health Cooperation is a private institution belonging to Dinas Kesehatan and has a legal entity con­sidered more flexible to bail out the initially issued funds to pay the financing of transport and extermination services to the transporter.ConclusionPrimary health care medical waste man­agement has followed the regulations. Health center has conducted medical waste management starting from the sorting, collection, pack­aging, storage, and transport. There should be improvements in some aspects, especially the creation of TPS for Puskesmas that do not have them yet, while Puskesmas who already have TPS need to make im­provements ac­cording to the conditions specified.

Abstract

Spatial-temporal distribution of malaria cases based on wind speed and humidity in Kulon Progo Regency of DIYPurposeThe purpose of this study was to determine the correlation of weather variables (humidity and wind flow velocity) in 2005-2015.MethodsThis secondary data study used 1439 cases of malaria and weather data for 11 years. The re­search design was an ecologi­cal study, with analysis using time/series graph and bivariate analysis.ResultsStatistical analysis showed that the vari­ables significantly related to the distribution of malaria cases were wind flow velocity lag 0, wind flow velocity speed lag 1, wind flow velocity lag 2, wind flow velocity lag 3. The humid­ity was not statistically related to the distribution of malaria cases.ConclusionThere was a relation­ship between wind speed up to three months earlier, while the humidity was not related to the spread of malaria cases. Kulon Progo Dis­trict Health Office needs cross-sectoral cooperation with other agencies, and malaria control programs need to pay attention to weather patterns in order to decrease morbidity and mortality due to malaria.

Abstract

Implementation of district level "four children better" family planning policy in MalinauPurposeThis study aimed to analyze the implementation of family planning program policy in Malinau.MethodsA case study was conducted through in-depth interviews with 18 participants.ResultsThe policy of the government in Malinau is ‘four children are better’. Differences in perceptions between stakeholders, providers, and users about family planning affecting social, economic, cultural, beliefs have an impact on contraceptive use in Malinau. The use of contraception is not prohibited for people with medical indications for using contraception, but peo­ple should access the contraception independently in the private sector.ConclusionThe policy of the Ma­linau government to stop the supply of contraceptives to government health facilities since 2012 is an effort by local governments to increase the number of inland and border populations. The unavailability of con­traceptives in government health facilities and the limitations of family planning information, communica­tion and education lead to differences in perceptions between stakeholders, service providers, and users. Therefore, the researcher recommends that the relevant regional apparatus unit does advocacy to legislative and executive boards in Malinau. The local government shall ensure the availability and quality of family planning services for the community on the basis of reproductive health rights.

Abstract

Predictors of severe visual impairment and blindness among diabetic patients in Special Territory of YogyakartaPurposeThe purpose of this study was to identify any predictors of severe visual impairment and blindness among diabetic patients in Daerah Istimewa Yogyakarta.MethodsThis research was a cross-sectional study by analyzing registry data from (JOGED.COM). Data included sociodemographic characteristics, diabetic status, health record, and eye diseases. The analysis was done using chi-square and simple logistic regression tests follow with semi-partial correlation, stratification tests and multilevel analysis with Generalized Estimation Equation (GEE).ResultsAs much as 1093 data were included in this study. The prevalence of severe visual impairment was 12% and blindness was 6.5%. Cataract, diabetic retinopathy and diabetes duration >10 years were statistically significant with severe visual impairment and blindness while glaucoma and hypertension only significant with severe visual impairment. The contribution of cataract in severe visual impairment (4.73%) and blindness (3.11%) was highest among eye diseases. There was modification effect of cataract to severe visual impairment and blindness by diabetes duration. Based on GEE, cataract, diabetic retinopathy and diabetes duration > 10 years was the best model to predict the occurrence of severe visual impairment and blindness.ConclusionsPredictors of severe visual impairment and blindness among diabetic patients in Daerah Istimewa Yogyakarta were the cataract, diabetic retinopathy and diabetes duration >10 years. Modification effect of cataract by diabetes duration only was significant in a model to predict blindness.

Abstract

INTISARILatar Belakang : Malaria merupakan penyakit yang disebabkan oleh Plasmodium spp dan ditularkan melalui gigitan nyamuk Anopheles spp betina. Di Purworejo malaria masih menjadi masalah penyakit menular yang utama, pada tahun 2013-2015 kasus malaria mengalami peningkatan dari 728 kasus (API 0,98‰) pada tahun 2013 menjadi 1.411 kasus (API 1,98‰) tahun 2015.Tahun 2016 hingga bulan Nopember jumlah kasus sebanyak 423 kasus.Tujuan :  Menganalisis faktor-faktor risiko dengan kasus malaria, memetakan distribusi  spasial kasus malaria kaitannya dengan jarak lokasi terhadap habitat perkembangbiakan dan  mengetahui habitat larva nyamuk Anopheles spp di wilayah kerja Puskesmas Winong Kabupaten Purworejo.Metode Penelitian : Penelitian ini merupakan penelitian observasi analitik dengan rancangan penelitian case control atau retrospektive study.Variabel bebas adalah faktor lingkungan dan faktor sosial budaya, sedangkan variabel terikat adalah kejadian (kasus) malaria. Distribusi kasus malaria kaitannya dengan jarak kasus terhadap habitat larva nyamuk Anopheles spp disajikan dalam buffer zone dengan menggunakan Sistem Informasi Geografis (SIG). Hubungan variabel bebas dan terikat dianalisis dengan pendekatan analisis bivariat dengan menggunakan uji Chi square, analisis multivariat dengan uji conditional regresi logistik dan analisis spasial.Hasil : Analisis bivariat dengan uji Chi square menunjukan ada hubungan kejadian malaria dengan keberadaan habitat perkembangbiakan (p=0,02;OR 2,5 ), kondisi dinding rumah (p=0,004; OR 0,29) dan kebiasaan keluar malam hari (p=0,01;OR 3,6 ),sedangkan keberadaan hewan ternak, jarak breeding site, kebiasaan memakai kelambu, penggunaan kawat kasa, pemakaian anti nyamuk dan kebiasaan mengunjungi daerah endemis tidak memiliki hubungan dengan kejadian malaria. Analisis multivariat dengan uji conditional regresi logistik menunjukan bahwa kebiasaan keluar malam hari (p=0,01; adjOR 4,8) merupakan faktor risiko yang paling tinggi berhubungan dengan kejadian malaria. Pada analisis spasial terbentuk 3 cluster kejadian malaria dan hasil buffering menunjukan bahwa kebanyakan kasus berada di dalam area buffer zone dengan jarak <1000 m dari habitat larva nyamuk  Anopheles spp berupa kolam, selokan, genangan air, sawah dan sungai.Kesimpulan : Ada hubungan antara keberadaan habitat perkembangbiakan larva, kondisi dinding rumah dan kebiasaan keluar malam hari dengan kejadian malaria. Terdapat 3 cluster di wilayah kerja Puskesmas Winong dan kebanyakan kasus berada di area buffer zone.

Abstract

Intensity of gadget use among overweighed preschool children in YogyakartaPurposeThe purpose of this paper was to determine the relationship of the gadgets uses intensity among obesity preschool children in Yogyakarta.MethodsThis cross-sectional study was conducted involving all preschool children in playgroup and kindergarten in Yogyakarta who did not attend the full day school program.ResultsPreschool children with high-intensity use of gadgets are 1.3 times more likely to be obese (p= 0.028, RP = 1.25). Preschool children with high-intensity use of gadgets, high education father, high sedentary behavior, adequate social economy, and poor diet are 2.1 times more likely to be obese.ConclusionThe incidence of obesity in preschool children is more common among children with high-intensity use of gadgets.

Abstract

Evaluation of malaria risk factors prevention and control program in Kulon Progo RegencyPurposeThe purpose of this research was to evaluate the program of prevention and control of malaria risk factors in Kulon Progo District in 2016 based on system approach.MethodThis research was evaluation program used mixed methods (sequential explanatory design). Data collected from 7 public health center of active focus malaria and district health office. Respondents were program managers and head of public health center, district program managers, head of control and eradication of communicable diseases, and head of district health offices. The instruments used structured questionnaires and in-depth interview guides. Quantitative data analysis was descriptive and qualitative data used Miles and Huberman model.Results Most of the availability of human resources, funds, facilities, materials, and methods already meet implementation needs except entomologists, allocation of funds activities other than IRS, time of the implementation of larval fish deployment. Most of the processes that started from the planning, implementation, monitoring, and evaluation have gone well except for planning other than IRS, preparation vector control without entomology database, implementation of environmental management, larviciding, and larval fish deployment. Coverage of LLINs, coverage of IRS, control of LLINs efficacy, and test of vector resistance have met the target, except coverage of larviciding and larval fish deployment was not available.ConclusionPrevention and control program of malaria risk factors has implemented but wasn’t based on entomological data. District health offices should recruit entomologist to support vector control activities.

Abstract

Obstacles and opportunities in making a hospital disaster plan: the case study from North SumateraPurposeThis study assessed Kabanjahe District hospital preparedness in coping with earthquake disaster, describes obstacles and opportunities in forming hospital disaster preparedness and availability of hospital disaster plan component.MethodsThis research used qualitative research method with case study research design. This study examines the contemporary phenomenon of Kabanjahe General Hospital preparedness in anticipating the earthquake disaster.ResultsKabanjahe General Hospital is not ready to anticipate earthquake disaster and not yet completed in making hospital disaster plan.ConclusionObstacles to establishing disaster prepared hospitals to include the unavailability of budget for disaster management, lack of hospital personnel who understand K3B, have not participated in training of hospital disaster plan preparation, disaster training and hospital have never conducted disaster simulation. While the opportunity is Kabanjahe General Hospital has been working with other agencies in the fulfillment of facilities and preparation of the plan. The availability of Kabanjahe General Hospital for policy components, disaster risk analysis, communications, financing and evaluation monitoring has not been in accordance with the standards. As for organizational components and facilities already available in quantity but not yet organized to anticipate disaster.

Abstract

Why don't online shop workers enroll in BPJS health insurance? a case study from the City of YogyakartaPurposeThis study aimed to explore why an informal sector group has not decided to become a part of BPJS health by identifying knowledge or health insurance literacy.MethodThis research was a qualitative study with a phenomenology approach. Determination of informants was used by snowball sampling technique. Data collection was conducted with in-depth interviews with 15 people from an online entrepreneurs group who live around the city of Yogyakarta. Data were identified with inductive thematic analysis related to health insurance literacy to determine any relationship pattern of re­search variables.ResultsThe majority of informants have knowledge health insurance as a health care program used to seek treatment when sick. Specifically, informants have difficulties in understanding and explaining the basic terms about BPJS health insurance such as membership type, premium, benefits package, services procedure, the way of registration. The main determinant is caused by personal factors of informants themselves who are not seeking information about BPJS health because busy with works, still young, unmarried, and get ill rarely. Another factor is informants more often rely on getting insurance information from friends or family, while information from the Government and BPJS organization itself is very rare.ConclusionLow health insurance literacy is the main cause why informants decide not to be participants in BPJS health insurance. So it’s necessary to educate people about the BPJS health insurance both from the government, BPJS organization, and especially health providers in health facilities.
