Abstract

Anak-anak yang tinggal di daerah  GAKI  mempunyai risiko yang lebih tinggi untuk mengalami kelaparan dan rendahnya skor IQ.  Tujuan penelitian ini menjelaskan  hubungan status gizi, anemia, status infeksi, dan  asupan  zat  gizi  (energi,  protein,  karbohidrat,  lemak,  iodium,  vitamin  C,  vitamin  A,  Fe,  Zn,  dan Selenium), dengan fungsi kognitif    anak sekolah dasar (SD) yang tinggal di daerah GAKI, dan  mengetahui kandungan iodium pada tanah dan air di wilayah tersebut. Jenis studi observasional dengan desain crosssectional.  Subjek  penelitian  anak  SD  berusia  9-12  tahun  kelas  3,  4,  dan  5  dari  3  SD  di  daerah  endemik GAKI  Kecamatan  Kismantoro,  Kabupaten  Wonogiri,  Jawa  Tengah.  Status  GAKI  diukur  menggunakan metode  palpasi  dan  Urinary  Iodine  Excretion  (UIE),  status  gizi  dengan  indikator  TB/U,  status  anemia menggunakan Hemocue, status infeksi dengan wawancara pada orangtua, asupan zat gizi menggunakan multiple  food  recall  24  jam  (3  hari),  kandungan  iodium  dalam  tanah  dan  air  menggunakan  Inductive Coupled  Plasma-Mass  Spectrometry  (ICP-MS)  dan  fungsi  kognitif  menggunakan  Weschler  Intelligence Score for Children-Revised  (WISC-R).  Hasil menunjukkan  20 subjek (28,9%) mengalami  GAKI, 27 subjek (39,1%)  stunting,  17  subjek  (24,6%)  anemia,  11  subjek  (15,9%)  infeksi  (ISPA  dan  diare)  dan  57  subjek (82,6%)  mengalami  gangguan  fungsi  kognitif.  Analisis  bivariat  menunjukkan  tidak  terdapat  hubungan signifikan antara status  GAKI  dan infeksi dengan fungsi kognitif (p>0,05). Terdapat hubungan signifikan antara status gizi dan anemia dengan fungsi kognitif (p<0,05).  Analisis multivariat asupan zat gizi energi, protein,  karbohidrat,  lemak,  iodium,  vitamin  C,  vitamin  A,  besi,  dan  selenium  memberikan  kontribusi sebesar  20,9%  dari  skor  IQ  total  anak  sekolah.  Hb,  UIE,  dan  asupan  zat  gizi  memberikan  kontribusi sebesar 24,1% dari skor IQ total anak sekolah. Hasil laboratorium menunjukkan  rerata  kandungan  iodiumtanah (2,49 ppm), dan dalam air (2,7ppb) berada di bawah standar. Jadi status gizi dan anemia, asupan zat gizi  berhubungan  dengan  fungsi  kognitif  anak  sekolah.  GAKI  dan  status  infeksi  tidak  berhubungan dengan fungsi kognitif. Secara bersama Hb, UIE dan asupan zat gizi berhubungan dengan fungsi kognitif. Rerata kandungan iodium pada tanah dan air di wilayah penelitian berada di bawah standar.Kata kunci: GAKI, status gizi, anemia, infeksi, fungsi kognitif

Abstract

Background : Previous studies has focused on intakes of individual nutrients and/or foods or food groups to specific effect on health, but there were still not clear about the effect of dieting pattern and food choice to overweight/obesity indicators like BMI-for Age, fat percentage, waist circumference and waist to height ratio and which food group has the most effect to obesity indicators.Objective: This study examined the association of meal pattern to BMI-for Age, fat percentage, waist circumference and waist to height ratio.Methods: This cross sectional study involved 738 individuals aged 15-18 years old adolescents in public school in Kota Yogyakarta. Height and waist circumference were measured by microtoise and metline. Weight and fat percentage were measured by hand-to-foot BIA. BMI-for–age were determined by WHO Antroplus. Dieting pattern was determined by self-reported questionnaire food frequency questionaire which consisted of 91-food item. Data were analyzed by t-test, and linier regression and multiple regression.Results: Overweight and obese were present in 13.41% and 5.43% subjects. In linear regression analysis, legumes, one dish meal, snacks, and sweetened beverages were negatively associated to BMI for age, waist circumference, waist to height ratio and fat pecentage (p<0.05). While in food items analysis, legumes, snacks and sweetened beverages were associated with the increase of waist circumference and fat percentage, while milk and soft drink as protective factor to the increase of fat percentage (-1.70 (-2.40;-1.00); -1.68 (-2.78;-0.58)).Conclusion: In a conclusion, increase in BMI for age and waist circumference are more affected by the intake of high frequency of macronutrients, while increase in fat percentage were associated with sweetened beverages consumption.

Abstract

Urbanization and economic rapid developments in the developing countries cause a nutrition transition. They affect the household dual burden of malnutrition in which there is overnutrition and undernutrition occuring at the same time in a household. This study proposed to calculate the prevalence of household dual burden malnutrition and find its determinants. This cross-sectional study use the Indonesian Family Life Survey (IFLS) 2014 data with total of 6468 families were enrolled as the sample of the study. The household dual burden malnutrition indicator is represented by mother and children’s nutritional status. Descriptive analysis and chi-square test were used in this study. The study found that the prevalence of household dual burden malnutrition in Indonesia is 8.27% in which the Borneo and Eastern Indoesia region has the higest prevalence. Maternal age (p = 0.001), maternal education (p = 0.022), number of children (p = 0.001), and number of household members (p = 0.001) were having significant correlation (p<0.05) with the household dual burden malnutrition in Indonesia. Further study by using longitudinal design is needed to find the predictors of it so the government can formulate an effective and efficient intervenion to prevent adverse effects of household dual burden malnutrition to the community.

Abstract

One of the health problems that happens in elderly is osteoporosis. Its prevalence in Indonesia reached 19.7%. Nutrition is one of the important parts of osteoporosis treatment. Sufficient calcium intake impacts on high bone density. Many factors affect calcium absorption.  Inulin is a prebiotic that helps increase calcium absorption. Bone composition is one of the important factors. Collagen is a bone component besides calcium and phosphate, which improves fracture resistance. The research aims to determine the organoleptic and physicochemical properties of a dairy product named "Kalipang". This research is an observational analytic study done in three stages. The first stage is a dairy product "Kalipang" formulation consisting of four formulas. The second is the organoleptic test of “Kalipang” including test against color, aroma, flavor, texture, and solubility and the last step is physicochemical properties test of products which covers the analysis of total sugar, reducing sugar, sucrose, protein, fat, water, ash, calcium, inulin, and pH. The result showed that from the aspect of flavor and overall, panelists prefer formula 2 than the other formulas.  Based on the color, aroma, and texture, panelists prefer formula 4 than the others. Nutritional content test results showed that formula 1 has the highest level of total sugar, water, and inulin. Formula 2 has the highest level of reducing sugar, fat, and sucrose. Formula 4 has the highest level of protein, ash, and calcium. Formula 4 meets the organoleptic acceptance and has a better nutritional content than the others.

Abstract

Several studies found that the probiotic bacterias such as Lactobacillus acidophilus La5 and Bifidobacterium lactis Bb12 could prevent glycemia and control the blood lipid profiles. Currently, a probiotic product such as yogurt is not preferable by some consumers since the taste is sour. Therefore, this study was conducted to develop milkshakes supplemented by probiotics Lactobacillus acidophilus La5 and Bifidobacterium lactis Bb12 which are expected to be more acceptable and have a beneficial effect with the addition of prebiotics and soluble fiber glucomannan. This study aims to determine the effect of synbiotic shakes on fasting blood glucose levels and lipid profiles in hyperglycemia Wistar rat models. The study design was a pre-posttest controlled group with 48 male Wistar rats divided into 8 groups, namely the healthy control group (I); hyperglycemia control group (II); group III that received metformin; group IV that received synbiotic shake with La5 + FOS; group V that received synbiotic shake La5 + inulin; group VI that received synbiotic shake Bb12 + FOS; group VII that received synbiotic shake Bb12 + inulin; and group VIII that received synbiotic shake La5 + Bb12 + FOS. The dose of the intervention was 3.6 ml/day with an intervention duration of 28 days. The results showed that the differences in blood glucose levels were not significant (p> 0.05) except for groups IV and VI that experienced a significant increase (p <0.05). The differences in lipid profiles showed insignificant changes in LDL levels except in groups IV and V, a significant increase in HDL levels (p <0.05) in group V and VI, and insignificant change in triglycerides except in group VI. It can be concluded that the administration of synbiotic shakes with a variety of prebiotics Lactobacillus acidophilus La5 and Bifidobacterium lactis Bb12 for 4 weeks were able to maintain fasting blood glucose levels and lipid profiles in hyperglycemic rats.

Abstract

Chronic inflammation can lead to insulin resistance in obesity. Increased mRNA expressionof inflammatory markers such as toll-like receptor 4 (TLR4) and interleukin 6 (IL6) werereported both in the tissue and the peripheral blood of obese adolescent and correlatedwith insulin resistance. DNA methylation surrounding TSS region is known to regulate thelevel of a gene expression. The aim of the study was to investigate the methylation statusof TLR4 and IL6 promoter in peripheral blood of obese adolescent and its correlation toinsulin resistance. This was a cross sectional study with observational analytic approached.Fifty adolescents with obesity from Yogyakarta Special Region, aged 15-18 years,z-score value >2 SD, no experience of any acute infections within 2 weeks and signed ininformed consent, were selected in this study. Subjects with diabetic mellitus, incompletedata and DNA concentration of <20 μg/mL were excluded. DNA samples were extractedfrom peripheral blood of the subjects. The bisulfite converted DNA was analyzed formethylation level with direct sequencing. Correlation analysis using Spearman test wasperformed with significance value was set at p<0.05. No methylation in TLR4 promoterwas detected in all subjects. The methylation level of IL6 was transformed to categoricalvariable. Four (8%) subjects with insulin resistance and 2 (4%) without insulin resistancehad methylation on a CpG site. Methylation of IL6 was not statisticaly different betweeninsulin resistance and non-resistance adolescent (p=0.635). The significant correlationwas not found between methylation level in IL6 promoter with insulin resistance (HOMAIR)(r=0.051). Cell specific of methylation mechanism, characteristic of subjects andtypes of samples were suspected causing those results. In conclusion, no correlationbetween methylation levels of TLR4 promoter and IL6 with insulin resistance in obeseadolescent was observed in this study.

Abstract

Cardiovascular disease is the main cause of death worldwide and in Indonesia. Public awareness and acceptance of preventative cardiovascular care are crucial for reducing illness-related morbidity and mortality. As a result, community service is essential for the primary prevention of cardiovascular disease. Between April and July 2019, this community service was undertaken in Ngangkrik Hamlet, Sleman Regency, Yogyakarta Special Region (DIY). Ngangkrik locals participated in this program in three categories: Bapak Bijak (Wise Dads), Ibu Cerdas (Smart Moms), and Pemuda Bijak (Wise Youngsters). Five stages comprised this program: planning, informing, preparation, implementation, and oversight. Lectures/courses, discussions, practice, and roleplaying were used to implement the materials. To examine participants' knowledge, the pretest and posttest approach was utilized. The program raised participants' knowledge and understanding about the prevention and treatment of cardiovascular disease across all categories.

Abstract

Latar Belakang. Gangguan Akibat Kekurangan Iodium (GAKI) masih terjadi di berbagai negara. Ibu hamil, bayi, dan balita adalah kelompok rentan GAKI. Kebutuhan iodium pada ibu hamil meningkat menjadi 250 µg/hari dari 150 µg/hari untuk pertumbuhan dan perkembangan janin. Asupan iodium harian tersebut harus terpenuhi agar tidak menimbulkan dampak buruk terhadap ibu atau janin diantaranya abortus, lahir mati, gangguan tumbuh kembang anak, dan dampak terburuk adalah lahir kretin. Wanita Usia Subur (WUS) merupakan sasaran potensial pendidikan kesehatan mengenai GAKI karena akan melahirkan generasi baru. Tujuan. Untuk mengetahui pengaruh perbedaan metode pendidikan kesehatan terhadap pengetahuan, sikap dan keyakinan WUS mengenai GAKI, efektivitas media intervensi yang digunakan, faktor pendukung, dan penghambat pelaksanaan pendidikan kesehatan. Metode. Jenis penelitian kuantitatif quasi experimental pretest and post test control group design dengan dukungan data kualitatif. Penelitian dilakukan di Desa Wulung Gunung dan Desa Wonolelo, Kecamatan Sawangan, Kabupaten Magelang pada bulan Maret sampai dengan Juni 2018. Kelompok eksperimen diintervensi dengan diskusi menggunakan leaflet sementara kelompok kontrol dengan ceramah menggunakan lembar balik. Sampel dipilih secara simple random sampling. Sebanyak 101 WUS yang sesuai dengan kriteria inklusi terbagi menjadi dua kelompok, kelompok eskperimen (n=54 orang) dan kelompok kontrol (n=47 orang). Data kuantitatif dianalisis dengan t test berpasangan, uji Wilcoxon, t test tidak berpasangan dan Mann Whitney, sementara data kualitatif dianalisis secara tematik. Hasil. Terdapat perbedaan rerata skor pengetahuan, sikap dan keyakinan yang tidak bermakna setelah intervensi antara kelompok diskusi dengan leaflet dan ceramah dengan lembar balik. Faktor pendukung adalah metode dan media intervensi yang dianggap menarik oleh peserta, sementara faktor penghambat adalah tidak semua peserta aktif dalam proses intervensi dan kondisi lingkungan sekitar yang dapat mengganggu konsentrasi peserta. Kesimpulan. Metode diskusi dengan leaflet dan ceramah dengan lembar balik setara dalam meningkatkan pengetahuan, sikap dan keyakinan individu, dan keduanya dapat menjadi alternatif pendidikan kesehatan mengenai GAKI di wilayah perdesaan endemik GAKI.

Abstract

Background The worldwide increase in the prevalence ofcardiovascular diseases in adulthood is related to obesity inchildren and adolescents. Insulin resistance and hyperinsulinemia observed in obese individuals are the precursors of cardiovascular diseases and type 2 diabetes mellitus. Magnesium, through its action on insulin receptors, is proposed to be an important factor in preventing insulin resistance.Objective The aim of this study was to assess the associationbetween magnesium intake and insulin resistance in obeseadolescent girls.Methods This was a cross-sectional study on obese adolescentgirls in Yogyakarta, Indonesia. Insulin resistance was defined as a HOMA-IR index of3.16 or more. HOMA-IR was calculated usingfasting insulin and plasma glucose levels. Magnesium intake andenergy adjusted magnesium intake were measured using a 24-hour food recall method on 6 non-consecutive days.Results Of 7 8 obese adolescent girls included in our study, 56% of them were found to be insulin resistant. Magnesium intake was only 61 o/o of the recommended daily requirement for adolescent girls. There were no significant associations between magnesium intake and either HOMA-IR or hyperinsulinemia.Conclusion Our study does not find an association betweeninsulin resistance and magnesium intake in obese adolescent girls.

Abstract

Background Transcription factor 7-like 2 (TCF7L2) and potassium voltage-gated channel subfamily j member 11 (KCNJ11) gene polymorphisms have been associated with type 2 diabetes mellitus (T2DM) via regulation of insulin production. Ingested nutrients induce glucagon-like peptide-1 (GLP-1), which in turn induces insulin secretion.Objective To evaluate the relationship between TCF7L2 and KCNJ11 gene polymorphism, dietary intake, and physical activity on fasting plasma GLP-1 in normal male adolescents.Methods This observational study with a cross-sectional design included 54 male adolescents selected from high schools in Yogyakarta, Indonesia. Interviews were done to collect data on energy intake and physical activity. The GLP-1 and insulin levels were measured from fasting blood plasma. The TCF7L2 and KCNJ11 gene polymorphisms were analyzed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP).Results Fasting GLP-1 was positively correlated with energy intake (r=0.276; P=0.047), but not with physical activity (r=0.011; P=0.936). The GLP-1 concentration was not associated with TCF7L2 and KCNJ11 gene polymorphisms (all P>0.05). In subjects with an EE genotype (KCNJ11), GLP-1 was not correlated with insulin (r=-0.036; P=0.435). However, in subjects with an EK genotype (KCNJ11), GLP-1 was positively correlated with insulin (r=0.394; P=0.026).Conclusion GLP-1 concentration is positively correlated with body weight. Among male adolescents with a genetic variation in KCNJ11 (EK genotype), there is a significant correlation between GLP-1 and insulin signalling.
