Journal of Health Technology Assessment in Midwifery provides an international forum for the publication, dissemination and discussion of advances in evidence, controversies and current research, current knowledge and promotes continuing education through publication of systematic and other reviews and updates across broad range of clinical and interdisciplinary topics including: 1. midwifery; 2. maternity and children health; 3. primary care for women and newborn; 4. public health; 5. health care policy; 6. health system, and global health. Abstract

The involvement of men in family planning has an important role in making decisions related to the use of contraception. This study aimed to determine the effect of husband’s support on the use of IUDs and Implant contraception among Indonesian couples. This is an analytic survey with a cross-sectional approach. The sample was selected by using the Slovin formula, resulted 398 respondents. Sampling was carried out using accidental sampling. Data collection was carried out using a questionnaire. Validity and reliability tests were carried out using Pearson's product-moment and Cronbach's alpha. Results analysis of the study was carried out by multiple logistic regression. 251 (63.1%) husbands were supporting the use of IUDs and Implant contraception as much as 147 (36.9%) others did not support the use of both contraceptives. The results of multiple logistic regression analysis are OR = 7,713; p = 0,0001 Husband's support influences the use of IUDs and Implant contraception.

Abstract

The recruitment process of nurses, professions supplementary to medicine and paramedical staffs had been a lengthy and labourers work for the Ministry of Health and Indigenous Medical Services (MoHIMS) of Sri Lanka. The process affected the regular recruitment of mentioned staff categories. Flaws in this system contributed negatively to the country's shortage of human resources for health (HRH) as well as efforts to improve the healthcare service delivery. The aim of the study was to identify the gaps in the traditional paper-based recruitment system of the MoHIMS and to design a suitable intervention to improve the process and to assess the improvements brought about by the intervention. A service improvement project with full participation of stakeholders was planned by the Human Resource Coordination Division (HRCoD) of the MoHIMS. Qualitative methods were used to identify the gaps in the existing recruitment system and to gather the ideas for improvement. A computer based online recruitment system was implemented with the collaboration of the Department of Computer Science Engineering, Faculty of Engineering, University of Moratuwa. The perceived satisfaction of the stakeholders was reassessed by qualitative methods. Long lag between advertising and recruitment for training was revealed. Deficiencies such as, lack of transparency, negatively affected routine office processes, inability to validate examination results during application process and frustrated applicants leaving the training programs due to lengthy selection processes were also among the main gaps identified. The implemented online recruitment system was recognised by the stakeholders for its improved timeliness, transparency and minimal interference to the routine office functions. Service improvement projects to be successful, should be implemented with full stakeholder participation. Computer based online recruitment systems can be successfully implemented in public sector recruitments to improve the efficiency and transparency

Abstract

The recent Ghana Demographic and Health Survey 2014, estimated that 30 % of currently married women have an unmet need for family planning services, with 17% having an unmet need for spacing and 13% having an unmet need for limiting. The objective of the study was to review the unmet need of family planning in order to make appropriate recommendation to improve family planning use in the Northern Region of Ghana. A conceptual framework which outlines the factors that interplay to determine the use of family planning services in Northern Region of Ghana was designed and used for the study. The data were sought from unpublished documents from the Regional Health Directorate of Northern Ghana, websites of Ghana Health Service, online international publications and University of Leeds Library. The unmet needs of family planning in the region was found to result from a number of factors including inaccessibility to family planning services, non-availability of some type of contraceptive methods in the communities, religious beliefs that contradict the use of family planning, traditional and cultural belief system which promotes high fertility preferences, high illiteracy level, non-approval of family planning by men who are the decision makers, misconception about the use of contraception and high poverty in the region. The identified possible strategies for tackling the factors responsible for unmet need of family planning in the Northern Region, community-based family planning services, family planning health education outreach, peer education and religious-based education were found to be considerably effective, feasible and sustainable.

Abstract

Dysmenorrhea is one of discomfort experienced by adolescent during menstruation. A Survey of 10 young women at University of Muhammadiyah Lamongan found 90% had dysmenorrhea. To reduce pain, they take pain relievers. However, the use of drugs can cause side effects, especially if long-term use can lead to addiction or dependence. Research design using Quasy Eksperiment (pretest-posttest). The study was conducted on adolescents who are experiencing dysmenorrhea. They were divided into two groups namely control and treament groups. The study began by assessing pain levels in both groups. The treatment group was given warm compresses but the control group was not given any treatment, after that reassess the level of pain. The study analyzes changes in pain levels and compares pain change in the two groups. The results showed that in the control group most of the adolescents did not experience pain changes during dysmenorrhea (86.7%), whereas in the treatment group most of the adolescents experienced a decrease in pain (93.3%). Data analysis using the Mann Whitney test showed p=0.000 where p0.05 so it can be concluded that there are differences in dysmenorrhea before and after treatment between the warm compress group and the control group. The results of this study are expected to be used as consideration in developing plans to reduce discomfort in the form of menstrual pain in a non-pharmacological manner.

Abstract

Introduction: Gestational Diabetes Mellitus (GDM) is one of the most common complications of pregnancy. Women with gestational diabetes have a higher risk of serious health outcomes for mother and baby such as preeclampsia, premature birth and the long term development of type 2 diabetes. This study was conducted to present a review of available research in several countries about GDM management during and after pregnancy. Method: Several databases including PubMed, ScienceDirect and EBSco were searched for relevant articles published between January 2009 and January 2019. Result: Of the 1186 initial articles identified, this study analyzed 7 relevant articles that met the inclusion criteria. This study showed that management for GDM includes medical nutrition therapy, exercise, monitoring of blood glucose, and insulin therapy if blood glucose is not achieved with that treatment. Exclusive breast feeding for at least three months has been shown reducing the risk of childhood obesity of children, particularly in those born to obese and mothers with GDM Conclusion: There is a need to increase awareness of long-term consequences on gestational diabetes, both in patients and in healthcare professionals. Counseling is needed for dietary intervention and physical activity for all postpartum women with a history of GDM to stay healthy or to improve future health

Abstract

Evidence-based is a very effective strategy to improve the quality of midwifery services. One of the most frequently problems faced is the gap between midwifery practices that are in accordance with the procedures (based on evidence-based) with clinical care practices that occur in the clinic. The purpose of this study was to explore in-depth the obstacles to the implementation of evidence-based normal childbirth, which are the position of delivery, monitoring and documenting delivery by using partographs in the Independent Practice of Midwives of Gunungkidul District, Yogyakarta.This research was a qualitative research with a phenomenological descriptive approach. The participants of this study were midwives who had the Independent Midwife Practice (PMB) consisting of 7 informants. Participant selection in this study used a purposive sampling technique with convenience sampling and identification of obstacles used was the Consolidated Framework for Implementation Research (CFIR). Data collection used was semi-structured interviews and observations. Data analysis used was a thematic analysis.The interview and observation of delivery assistance is done at the Independent Midwife Practice Center (PMB). The position of delivery that was often used by patients was lithotomy and left slant, and the application of evidence-based monitoring and documentation using partograph was still not optimal. This was because there were several obstacles in its application, which were obstacles from patients, families, midwives, and obstacles from the organization. The impact of these obstacles was that it can reduce the quality of obstetric care in patients according to evidence-based. The evaluation of clinical practice based on evidence-based by the organization and the existence of feedback from patients to midwives are expected to improve the quality of service to patients.

Abstract

Adolescent pregnancy is defined as a pregnancy between the ages of 13-19 years, adolescent pregnancy occurs around 25% of women worldwide, but this number continues to decline due to educational support, contraception and prevention strategies for adolescent pregnancy. Evidence shows that there were programme interventions to reduce adolescent pregnancy such as encouraging people to participate in comprehensive prevention of adolescent pregnancy programs and health campaign to avoid unsafe sexual relationships. This scoping review was aimed to map out available evidence related to the parents’ involvement and barriers of programme intervention to reduce adolescent pregnancy. This scoping review followed the framework of Arksey and O'Malley, which consists of identifying the review questions; look for relevant evidence; selecting relevant studies; maping the data; and medicate, conclude and report results. PRISMA flowchart was used to display the evidence search flow. The results of the review indicated that there were 12 included articles from the search process. This review emerged 3 themes: efforts to reduce adolescent pregnancy rates, support for adolescent reproductive health services, barriers to reproductive health services for adolescents. It is important encouragage people to participate in comprehensive adolescent pregnancy prevention programs in including provision of information about the dangers of adolescents’ pregnancy, reproductive health, and maintain positive relationship with the family.
