Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

Tiang pancang is the main product of PT Hume Sakti Indonesia. There were the error product of it,the company must be found the solution which can be used to increase quality of this product itself. The writer wanted to plain the improvement of product quality with using USE-PDS concept (Understand, State, Evaluate, Plan, Do, Study, Act) and FMEA analysis. After the writer have done this concept and FMEA got the result where the first DPMO for this company was 10853 per billion of chance with total of failure was 5.680.000 rupiahs. Than chose alternative which the writer can be recommended based on the FMEA table, where recommendation to choose this alternative based on the highest value. Based on FMEA table was gotten the highest priority was from the last production.

Abstract

Many small industries have difficulties to survive or to develop their business. The CSSF (contingent strategic success formula) approach was used to analyze the formula of success strategy combining with internal and external factors of the companies, i.e. to measure, to estimate, and to forecast the condition of environmental companies in this time and the future. Measuring. The internal factors of the companies was aimed to know everything that improved so that follow the external needs condition in the future. In order to succeed in the future, companies need to follow the optimal internal and external condition with turbulence level by use three points of the marketing methods, are to build the product positioning at specific area business, to build a robust differentiation and to build the brand images or emotional relationship with customers.

Abstract

Health represent essential requirement these days. For that on duty in charge have to have yardstick to know efficiency ;level and health service productivity. Measurement of efficiency as productivity index must be done objectively, so that result of measurement can be made input to progress of health of society. Target of research was to make a model assessment of efficiency in order to determination of productivity every health service and determine repair plan to inefficient service. Problem solving used Data model of Envelopment Analysis (DEA) approach. DEA was used to measure productivity and efficiency with input multi and of multi output. With existence of real correct measurement - objective correctness, expected will give instrument to Health department to give satisfaction to wide [of] society. From result of data processing, will be obtained value of technical efficiency for the each Decision Making Unit (DMU) was less efficient, and some repair goals which must reach. Stipulating of peer of group to less efficient DMU will give direction to repair program.

Abstract

The problem of quality is often found in limestone combustion of small and medium central industry in Manyar Regency, Gresik, in which, the quality of limestone is defected and has high variation of CaO. One of many methods to improve it is by applying Robust Design from Taguchi, them, its result is socialized in a central industry as a reference for those who conduct combustion in producing material. This study found four controlled factors that each of them consists of two levels that can influence the quality of limestone: A) raw material (level 1 = yellow limestone; level 2 = white limestone); B) Combustion material (level 1 = wood & rubber; level 2 = wood and gerbage sawmill); C) Combustion length of time (level 1= five days; level 2 = four days); D) Combustion temperature (level 1 = ± 800° C; Level 2: ± 875° C). Meanwhile, the optimal combination factor level to improve the quality of limestone is A2, C1, D1, and B2. If factor of combination level is applied, the number of defected good in each combustion can be predicted as much as 1,41% and the defected variable among combustion is 0.37%.

Abstract

PT XYZ is company of logistics service of PT CI where this company hold responsible to logistics activity of PT CI that is covering distribution and warehousing. In transportation activity PT XYZ conducted armada outsourcing transport from some transporter every day. A problem faced in this time was truck rent expenses for the delivery of order to store. Hypothesis customer early from this research was if it done consolidation delivery of order customer, hence expected will be yielded to decrease cost rent daily truck which enough significant. At this research, consolidation conducted by developing method of saving heuristic. By using data of customer was same order on February, 10 2006, this research can reduce consumer of truck counted 3 truck and yield cost-saving rent equal to 18.87% from expense of early. This research also design software prototype for daily operational activities in determining used truck.

Abstract

The reliability services and power quality of power system nowadays become popular in industry because of cost energy reason, not exception for PT SAI. This company utilize control unit process to produce aluminum foil. The purpose of this research is identifying and analyzing the power quality of PT SAI and then to find the correct and economical solution to extinguish the source of bad power quality. After we identified, we conclude that 65% of load are AC motors, 25% of DC motors, 8% of heater and 2% of lightning. For over voltage and under voltage, refers to IEEE 519 – 1995 standard, PT SAI have 99% of nominal voltage. But for voltage imbalanced, refers to NEMA MG-1-1993 standard, PT SAI have 1,22%. For THD of voltage, PT SAI have 1,98%. But for THD of current, PT SAI have 34,47%. While transformer loading between 0,47% - 29,7%.

Abstract

Coconut tapis fiber is one of the natural fiber for use composites on account of its characteristic such as a high strength to weight ratio, resistance to corrosion, resistance to wear, good finishing surface, no health risk and easy availability. This research was conducted to study tensile strength and flexural strength of coconut tapis fiber reinforced composite with variable of fiber treatment in NaOH for 2, 4, and 6 hours. This research used coconut tapis fiber with 0.04 -1 mm diameter and 10 mm length, resin unsaturated polyester type Yukalac 157 BQTN-EX as its matrix, and metil etil keton peroxide 1% as its hardener. The fiber was treted in NaOH for 2, 4, and 6 hours, afterward rinsed and dried, respectively. The composite was made using press hand lay-up technique with post curing 2 hours at temperature of 620C. Tensile and flexure test specimen were made based on ASTM D3039 and ASTM D 790-03 standard accordingly. The result shows that tensile strength decrease with increasing treatment in NaOH. The highest tensile strength was achieved by 2 hours treatment in NaOH at 58.8 MPa. The highest tensile strain was achieved by 2 hours treatment NaOH at 1.30%. The highest tensile modulus of elasticity was achieved by 6 hours treatment in NaOH at 5.07 GPa. In agreement with tensile strength, flexural strength also decreased with increasing treatment in NaOH. The highest flexural strength was achieved by 2 hours treatment in NaOH at 125.98 MPa, and the highest flexural strain was achieved by 2 hours treatment in NaOH at 1.35%. The highest flexural modulus of elasticity was achieved by 6 hours treatment in NaOH at 13.55GPa.

Abstract

Products which have a good brand awareness will get long-term strategic advantages and one way of creating brand awareness is by promotion activities. This research tries to find out the efficiencies of promotion activities by comparing the number of sales with the promotional expenses. Beside that, a survey assigned to customers to collect informations about the most known promotion media in order to get informations about the most effective promotion media. To assure good qualities of the product promoted, the same survey also run to collect informations about differences between customer’s importances and performances level toward product qualities. In addition, customer price sensitivities also asked to get information about potential loss sales to projected revenue if price rising is stated. This research results that 2nd semester (July-December 2007) is the most efficient, and the most effective promotion media are televisions, magazines, promotion events, and newspapers. This research also notes that friends or families are the prime information source for customers to know about ISP X. In order to improve the recent qualities, here are given some qualities atributes that urgently need according to customers to be fixed. In other side, it also noted that customers are somewhat price-sensitive. However, better offer from competitor still looks more risky than price rising issue itself, though with better qualities present.

Abstract

This Research was conducted in PT X Tbk, Pandaan as a mean to look for combination amount of optimal product. PT X Tbk in the field of plastic production, goods the produced bottle by is assorted of size measure and colour. As for bottle the taken Image of Chamomile and of Nivea, while colour of Image bottle of Chamomile is green and its size measure is 120 ml and 250 ml. For the bottle of white Nivea chromatic and its size measure is 100 ml and 200 ml. From various produced bottle type cause incidence of a problem of that is problem of determination of less precise production volume. This matter is known from often happened product insuffiency or excess causing the loss of opportunity of request so that obtained advantage less optimal. In research to determine optimal product combination with Linear Programming method through two phase, that is first request of forecasting by using method of time series constructively Minitab software among others Moving Average, Single Exponential Smoothing, Double Exponential Smoothing, Winter’S and of Trend Linear, and second stage to conduct calculation of Linear Programming model constructively QS. From result of calculation by using QS hence got optimal solution of product combination between Image bottle of Chamomile 120 ml Green, Image of Chamomile 250 ml Green, Nivea 100 ml Slimdes RW White, and Nivea 200 ml Slimdes RW White which must produce. As for obtained advantage from after applying of Linear Programming method representing result of calculation during 12 period is equal to Rp421,918,752, while obtained advantage before conducted by Linear Programming is Rp410,250,750, so that happened the make-up of advantage equal to 2.84% that is Rp11,668,002.

Planning material control have a purpose to compile a system capable to yield information to conduct correct action (reordering, scheduleng repeat, determining the amount of requirement at the time of is correct). One of the step performed within standard material control that is deermining best raw material ordering method utilize to get the expense of mot message of minimum. Utilize to support plan produce which have been made and fulfill Planned Order Release pursuant to calculation of MRP hence one of the capacities type from company that is labour factor had also have to be reckoned by do existing amount last for fulfilling requirement which have been reckoned. The object of this UD PATRIOT which located in Mojokerto.This company produce various shoes type which in its operation have constrain in determining best ordering method to each every ordered by raw material item as well as standart material control system which good to menimalize production cost. Planning material control by using this MRP II system was proposed to be applied by at company utilize to yield correct action that is determining maount, time, scheduling repeat, and ordering execution requirement of raw material. With applying MRP II can cost effective equal to Rp4,372,413.88 or 35.91% from expense of before using MRP II.
