Abstract

Background: Medication administration errors are a patient safety issue that may result in injury, prolonged hospitalization, increased healthcare costs, and death. Barcode Medication Administration (BCMA) is a barcode-based verification technology used to confirm patient identity, medication, dose, time, and route before medication administration. Objective: This article aims to review scientific evidence regarding the role of BCMA in preventing medication administration errors and supporting patient safety in hospitals. Methods: A literature review was conducted using PubMed and ScienceDirect on April 26–27, 2024. The keywords included barcode medication administration, patient safety, medication error, and hospital. Articles were considered eligible if they were published between 2015 and 2024, available in full text, open access, conducted in hospital settings, and addressed BCMA or medication administration safety. Of 662 identified articles, five met the criteria and were synthesized narratively. Results: The synthesis showed that BCMA has the potential to support patient safety by improving patient and medication verification, strengthening adherence to the rights of medication administration, and reducing several types of medication administration errors. However, its benefits are influenced by infrastructure readiness, barcode quality, hardware and software issues, workflow compatibility, user training, and the risk of non-compliance or workarounds. Conclusion: BCMA is a technology that has the potential to help prevent medication errors and improve patient safety in hospitals. However, its successful implementation depends not only on the availability of the technology, but also on organizational readiness, nurse training, system integration, and user compliance in consistently following scanning procedures.

Abstract

Background: Burnout is a condition of chronic physical, emotional, and mental exhaustion commonly experienced by healthcare workers due to prolonged work-related stress. This condition has become a serious concern in primary healthcare services, particularly in community health centers, because it may affect service quality and patient safety. Tampaksiring 2 Public Health Center faces increased workload after the pandemic, limited healthcare personnel, and high service demands, which elevate the risk of burnout among nurses. This study aimed to describe the incidence of burnout and the implementation of nursing care for nurses experiencing burnout at Tampaksiring 2 Public Health Center. Methods: A descriptive case study was conducted at Puskesmas Tampaksiring 2 from March to May 2025 involving 11 nurses. Data were collected through in-depth interviews, observation, and completion of the Maslach Burnout Inventory (MBI), supplemented by workload documentation review. Nursing care was delivered systematically based on SDKI, SLKI, and SIKI standards. Results: A total of 54.5% of nurses showed high burnout levels, characterized by severe emotional exhaustion, depersonalization, and reduced personal accomplishment. The primary nursing diagnosis was fatigue (keletihan). Energy management and psychosocial support interventions resulted in significant improvements in MBI scores after four weeks. Conclusion: Burnout among nurses at Puskesmas Tampaksiring 2 is at an alarming level, requiring structured nursing interventions and managerial policy measures.

Abstract

Background: Hospitalization in children often causes anxiety due to the unfamiliar hospital environment, frightening medical procedures, and separation from a comfortable environment. This anxiety condition can hinder the treatment process and reduce the child's cooperation during medical procedures. One intervention that can be done by nurses to reduce anxiety in children is distraction techniques through puzzle play therapy. Playing puzzles can help children divert attention from fear and provide positive cognitive and emotional stimulation. Objective: To determine the policy of actions that prevent puzzle play distractions in reducing children's anxiety due to hospitalization in the Chandra Room of the Dr. J. A. Latumeten Class II Hospital Ambon. Methods: This study used a case study approach on 2 preschool-aged children who experienced anxiety during hospitalization. The intervention was carried out by providing puzzle play therapy during the treatment period and observations were made on changes in the children's anxiety levels before and after the intervention. Results: The study showed that after being given puzzle play therapy, the children's anxiety levels decreased, which was indicated by reduced crying behavior, tension, and increased cooperation during nursing procedures. Conclusion: This study shows that puzzle play distraction nursing actions are effective in helping reduce anxiety in preschool children who are hospitalized. This intervention can be used as a non-pharmacological approach in pediatric nursing practice in hospitals.
