Abstract

Mammalian target of rapamycin (mTOR) is a PI3K (phosphatidylinositol-3-kinase)-related kinase, via two distinct forms – mTOR complex 1 (mTORC1) and mTORC2 coordinates various extracellular and intracellular signals like growth factors, nutrients, amino acids, energy levels in order to achieve optimum cell growth. mTORC1 regulates protein synthesis, lipid biogenesis, oxygen homeostasis, proliferation, autophagy; mTORC2 plays role in cell survival. Since this pathway controls major cellular processes, its dysregulation is associated with pathological conditions like cancer, obesity, type 2 diabetes mellitus. Rapamycin, a naturally occurring mTOR inhibitor, along with synthetically derived compounds of rapamycin – rapamycin analogs (rapalogs), produced by chemical modification of parent drug have achieved clinical success in certain tumor subtypes. Rapalogs have shown their own limitations due to development of resistance and activation of feedback pathways, focusing on the need for efficient mTOR inhibition. In this regard, mTORC1/mTORC2 (mTOR Kinase) inhibitors and dual mTOR/PI3K inhibitors have shown promising results in various in-vitro tumor models, and are currently in phase I/II of clinical trials. With the better understanding of dynamics of mTOR signaling pathway and its function in tumor microenvironment, mTOR inhibitors may provide a good alternative against advanced and refractory cancers. Keywords: Rapamycin, mTOR, Tuberous sclerosis complex, PI3K, AKT, Rapalogs, Everolimus.

Abstract

Background: Percutaneous nephrolithotomy (PCNL) has become the treatment of choice for staghorn stones, replacing open stone surgery. However, in a developing country, open stone surgery is still the treatment of choice due to its nature for not requiring endourology devices, it can be performed by a general surgeon, and it is financially acceptable. Objective: This study aims to compare the cost-related outcome between PCNL and open renal stone surgery in patients with staghorn stones.Method: Data were collected retrospectively, taken from patients’ medical records who underwent PCNL and open stone surgery in three hospitals from 2016-2017. Seventy-eight patients were divided equally into PCNL group and open surgery group. Data of the hospital length of stay, the volume of blood loss during surgery, stone-free rate, and post-operative additional procedures were recorded.Outcome Measurements and Statistical Analysis: The data were analyzed using SPSS version 23.0 for Windows. Results: The hospital length of stay was shorter in PCNL compared to open stone surgery (3 vs. 4 days, p<0.001). The post-operative stone-free rate was lower in PCNL compared to open stone surgery (48.7 vs. 92.3%, p<0.001). More post-operative additional procedures were found in PCNL compared to open stone surgery (p<0.001). There was no significant difference in the amount of blood loss during surgery in both groups (p=0.102).Conclusions: PCNL is a minimally invasive procedure that is effective and safe for treating staghorn stones. Nevertheless, in places where PCNL is unavailable, open stone surgery can still be a choice for treatment.

Abstract

The research was performed by the authors to study the effect of exam stress on the respiratory system of 1-4 year students. The experiment was carried out in the longitudinal regime for two years with the participation of the same students. Healthy students participated in the experiment voluntarily. We surveyed 89 1-4 year students, of which were 30 boys and 59 were young females aged 18-22 years. The survey was conducted during the intersessional period and on the day of the exam. To study the functional state of external respiration in the examined students, a pneumotachometric type spirometer with a SPIRO-SPECTR calibration syringe was used. The measurements were carried out from 9 to noon since during this period of the day the studied parameters are the most stable. The experiment took into account gender characteristics. Two age groups were formed: 18-19 years old (1-2 years of education) and 20-22 years old (3-4 years of education). Analysis of the research results showed that in the time course of learning from 1 to 4 years, students' adaptive capacity of the respiratory system increases, resulting in increased efficiency and effectiveness of regulatory mechanisms. Keywords: Longitudinal studies, Psycho-emotional stress, Examination stress, Functionality, Respiratory system.

Abstract

Smoking behavior is one of the risky health behaviors adopted by adolescents in Indonesia. The behavior was commonly recognized as a masculine among male adolescents. The label of man identity called masculinity influences this condition. This study investigated the association of masculinity as ideology, masculinity norms, and gender role conflict and its correlation with smoking behavior in male adolescents who lived in Surabaya. This study used a descriptive correlative method and involved 314 adolescent males aged 15-18 years old in Surabaya as participants. We employed univariate analysis to identify predictors, including the ideology of masculinity (MNRI-SF), masculinity norms (CMNI-46), and gender role conflict (GRCS-I); and a dependent variable: male adolescents’ smoking behavior. The data were collected electronically, following waived written consents. Bivariate analyses were conducted to investigate each of the predictor's questionnaires and dependent variables using chi-square with the level of significance of p<0.05. Adolescents as participants in this study have low levels of ideological masculinity, high norms of masculinity and gender role conflict in the moderate category. The results indicated that ideology masculinity, masculinity norms, and gender role conflicts are positively associated with smoking behavior (p=0,001, p=0,029, and p=0,001 respectively). The findings of this study suggested that smoking behavior was constructed as an element to the idea of masculinity in the perspective of male adolescents in Surabaya, despite the available warning to prevent the circulation of tobacco products to adolescents. Thus, this study recommends the need for reinvention around tobacco-products circulation as not to involve the youth. Keywords: Adolescent, Gender role conflict, Masculinity, Smoking behavior.

Abstract

In this study novel unsaturated compounds could be prepared with the required skip-conjugated (Z,Z)-double bonds configuration using  a traditional Wittig reaction. This reaction depends on a coupling of aldehydes with phosphonium salt. Must products do not appear to have been prepared before, and these products were diagnosed by using NMR spectrum, infrared (IR) and mass spectrometer techniques. It is concluded that these products could widely be used as a fatty acid and a useful intermediates in the synthesis of pheromones instead of insecticides, and several chemical reactions. Keywords: Witting reaction, Unsaturated fatty acid, Skip-conjugated alkenes, Pheromone.

Abstract

Ototoxicity is an adverse drug reaction of Amikacin and involves mainly bilateral high-frequency sensorineural hearing loss and tinnitus. This case report is essential to identify, resolve and create awareness of the predisposing factors which can lead to ototoxicity. It elaborates the rare adverse effect caused due to Amikacin induced ototoxicity and the treatment strategies adopted at a secondary care hospital in an 8 year old male child. Treatment for Urinary Tract Infection (UTI) included Amikacin for 1 day following which the next day he developed blocked ears and diminished hearing as an adverse reaction to the aminoglycoside antibiotic. Management emphasis is on prevention, as most hearing loss is irreversible. After 3 days of treatment, the patient’s condition improved and was discharged with advice to continue liquid paraffin 3drops-0-3drops in both ears for 3 days. Appropriate measures must be put into place to manage aminoglycoside-induced cochleotoxicity effectively, in order to maximize the affected individual’s quality of life.

Abstract

The paper discusses the results of testing the bactericidal and bacteriostatic effect of various concentrations of pefloxacin and enrofloxacin on European foulbrood pathogens in contrast to oxytetracycline. The work presents the therapeutic efficacy of pefloxacin in treating bee colonies affected by European foulbrood disease. The study tested different methods of introducing medicine in the field experiment. European foulbrood disease revealed at the studied apiaries is caused by Melisococcus plutonium, Bacillus alvei, and Enterococcus faecalis; Brevibacillus laterosporus. In Nurimanovsky and Baimaksky districts (Republic of Bashkortostan, Russia), Melisococcus plutonium, which is the primary causative agent of EFB, has not been detected. The minimum bactericidal concentration of pefloxacin is 0.001% for Enterococcus faecalis, while the minimum level for Brevibacillus laterosporus and Bacillus alvei is 0.01%. Oxytetracycline exceeds the index 10fold, and it is 0.01 % for Enterococcus faecalis and 0.1 % for Brevibacillus laterosporus and Bacillus alvei. The minimum bactericidal concentration of pefloxacin is 0.01% for Melisococcus plutonium, which is 100 times lower than the minimum concentration of oxytetracycline (1.0%). High therapeutic efficacy is achieved (93.8...95.9%) in treating bee colonies affected by European foulbrood disease with pefloxacin by adding the antibiotic to the feed syrup (1:1) at a dose of 0.01%. The value is significantly higher than the effect of oxytetracycline applied at a dose of 0.05% (87.4...88.2%).Keywords: Apis mellifera mellifera L, European foulbrood disease, Bactericidal activity, Fluoroquinolones, pefloxacin.
