Abstract

Objective: Nicotine, a mayor addictive constituent of tobacco plays a critical role in smoking addiction.  Nicotine is primarily metabolized to cotinine. The objective of the research was to determine the variation of Nicotine metabolism in adult men in the Jakarta Clinical Laboratory. Methods: The research was done to 27 men aged 19-45 years old, which consisted of 6 smokers and 21 nonsmokers. Nicotine and cotinine of the plasma concentration 2 hours after chewing one piece of nicotine gum were determined by Liquid Chromatography-Mass Spectrometry (LC-MS) method. Results: The cotinine/nicotine ratio of the plasma concentration was calculated as an index of nicotine metabolism. Scatter plot cotinine/nicotine ratio of showed distribution heterogeneous (not homogeneous).Conclusion: It could be concluded that variation nicotine metabolism could be categorized to light nicotine metabolism (<0, 22), medium (0, 22 - 4, 19), and heavy (> 4, 19).Keyword: Nicotine metabolism, Cotinine/nicotine ratio.

Abstract

Erectile dysfunction (ED) is a  common problem in the medical field. In contrast to worldwide data, the prevalence of ED and its risk facftors in Indonesia is still unknown. The objective of this study was to determined the characteristics of male erectile dysfunction patients in the andrology unit of Dr. Soetomo hospital, Surabaya, Indonesia. A descriptive cross-sectional design was implemented in this study. There were 83 male patients at URJ Andrology RSUD dr. Soetomo Surabaya. The highest prevalence of erectile dysfunction patients in the age group was age 41-50 years old (22.9%). The degree of IIEF-5 was the most in severe degree  (39.8%), while the EHS score was the most in score 2 (38.6%). The highest internal risk factors were vasculogenic (90.4%), the external risk factors was the lifesyle factor (56.6%). In Surabaya, Indonesia, the incidence of erectile dysfunction has increased in the 40 year age group with severe degree, which mostly caused by vascular disorder combined with lifestyle factors. Multidisciplinary treatment and healthy life are needed for erectile dysfunction management to improve the quality of life.Keywords: Erectile dysfunction, Patient characteristic, Surabaya, Indonesia.

Abstract

A new chelating imidazolylazo groups were synthesis. New ligand of azo dye 2-[2ــ- (benzimidazolyl) azo]-5 -dimethyl amino benzoicacid (BIABA) have been synthesized by diazotization in alkaline alcoholic solution. Complexes of this ligand were prepared with a series of metallic ions Cu (II), Co (III), Ni (II). These complexes were examined using UV-Vis spectroscopy, the surface nature, morphology and elemental composition of individual novel (BIABA) ligand particles were characterized using Field Emission Scanning Electron Microscope (FE-SEM) coupled with an Energy Dispersive X-ray system (EDX). The structures of the synthesized dyes were determined by analysis of their Mass spectrum, Fourier-transform infrared (FTIR), 1H NMR. Abstract

Introduction: Early menarche is one of the major of concern in reproductive health, and affects a large number of women throughout their reproductive life from early adolescent. Early menarche is an important indicator of sexual and reproductive health. The aim of this research was to explain the relation between nutritional status and lifestyle with early menarche case in-adolescent in Surabaya. Method: The research design used was cross sectional. The study population consisted of 232 early-adolescent. The study sample totaled 145 early adolescents based on Purposive sampling with a study period of 2 months. The independent variables were nutritional status and lifestyle, while the dependent variable was early menarch. The data collection method used was a questionnaire sheet filled out by the adolescents and BMI measurement. The statistical analysis used Spearman rho (α ≤ 0, 05). Results: The results showed that nutritional status and lifestyle were positively related to early menarche in early-adolescents (p = 0.000). Discussion: Overweight nutritional status or high BMI and unhealthy lifestyle will tend to experience early menarche. Early menarche can lead to a premature menapouse as well, the risk of heart and bone disorders.Keywords: Nutritional status, Lifestyle, Early menarche, Early-adolescent.

Abstract

The aim of the work is to analyze and systematize data regarding the efficacy and safety of anti-tumor necrosis factor-α biosimilars, as well as their registration and patenting. Studies were conducted using a databases on the Internet: Ukrainian patent office, the European patent office, the US patent office, the Food and drug administration, European Medicines Agency, State enterprise “The State Expert Center” of Ukraine. The breakthrough in the treatment of rheumatoid arthritis is the discovery and administration of anti-TNF-α agents, such as infliximab, adalimumab, etanercept, certolizumab pegol, golimumab. Numerous randomized clinical trials and meta-analysis data indicate high efficacy and acceptable safety of anti-TNF-α biosimilars for the treatment of rheumatoid arthritis. The development and registration of anti-TNF-α biosimilars reduces the costs of health care and patients for biological therapy, increases patient access to treatment with the latest drugs, and contributes to the further development of biotechnology. Thus, effective anti-tumor necrosis factor-α biosimilars may be more accessible to the general population with rheumatoid arthritis. Despite certain hazards and risks associated with possible violations in the implementation of biological products in real practice, the industry of highly effective biosimilars brings progress both for the development of science and for practical healthcare.Keywords: infliximab, adalimumab, etanercept, certolizumab pegol, golimumab, biosimilar, efficacy, patent.
Bacterial Isolates Associated with Pelvic Inflammatory Diseases among Women of Reproductive Age in Babylon Province Moayad N. Mosleh Al-Kafajy

Abstract

Background: Pelvic inflammatory disease (PID) is a common infection and inflammation of the reproductive tract that is lining (endometritis) and fallopian tubes (salpingitis), is a recurrent condition among young women .PID and its complications, such as infertility, ectopic pregnancy, and chronic pelvic pain, are preventable by screening asymptomatic patients for sexually transmitted infections (STIs). This study aimed at identifying the microorganisms isolated, as well as the evaluation of frequency distribution for Chlamydia trachomatis, Neisseria gonorrhea, and others aerobic and anaerobic bacteria which may be associated with PID. Materials and Methods: Endocervical swabs totalling 43 were aseptically collected from women of reproductive age with pelvic inflammatory disease (PID), attending in two hospitals of Babylon Province: Province: Babylon Maternity and Pediatrics Hospital, and Al-Mahaweel General Hospital; as well as the samples taken from private clinics, during the period from January to October 2018. Results: Out of the 43 samples analysed, 30 women (69.7%) yielded pure cultures of bacterial isolates, among these 30 women, 22 (73.3%) had mixed cultures while no bacterial growths were recorded from the remaining 30.3 % samples. Organisms encountered were Escherichia coli the most encountered frequency (37.2%) followed by Staphylococcus aureus with (27.9%). Enterococcus faecalis (23.2%), Gardnerella vaginalis (16.2%), and Candida spp (11.6%) were less common, while Chlamydia trachomatis, Klebsiella pneumonia, and Neisseria gonorrhoe had the lowest occurrence frequency of  (6.9%) , (6.9 %), and (3.2 %). Conclusions: Pelvic inflammatory disease is a major health problem in developing countries of the world; the micro-organisms frequently responsible for acute PID were genital tract Escherichia coli. Thus, measures should be taken regarding early diagnosis in order to provide treatment before the complications develop of PID.Keywords: Acute pelvic inflammatory disease (PID), Female patient, Clinical presentation, Microbial isolates.

Abstract

Objective: The main goal is to address the role of mesenchymal stem cells derived from bone marrow (BM-MSCs) and adipose tissue (AD-MSCs) in mitigation of cyclosporine A (CsA)-induced nephropathy in rats. Methods: Isolation and propagation of MSCs from both sources were performed and the cells were recognized by the microscopic appearance and estimation of specific surface antigens (CD90, CD105 and CD45) using flow cytometry. In this study 40 adult male rats were distributed into 4 groups; (1) control group, (2) CsA group, (3) BM-MSCs group and (4) AD-MSCs group. Two month after stem cell infusion, urea and creatinine serum levels were estimated. Also, serum levels of MCP-1, NF-κB, nephrin and NAG activity were evaluated. Also, histopathology of kidney sections was carried out. Results: Microscopic photo documentation and CD surface markers proved that the isolated cells have typical characteristics of MSCs. The outcomes of the in vivo study indicate the beneficial effect of BM-MSCs and AD-MSCs in kidney restoration as manifested by the significant blunting in serum urea, creatinine, MCP- 1, NF-κB levels and the significant inhibition of serum NAG paralleled by the significant increase of serum nephrin level and GFR. These findings were also appreciated by the histopathological observation of kidney tissue sections as the cell therapy elicited marked betterment in the structural organization of kidney tissue. Conclusion: This study provides great evidence favoring the significant role of MSCs, particularly isolated from AD, in retrieving experimental nephropathy induced by cyclosporine. The efficiency of MSCs could be ascribed to their immunomodulatory effect, anti-inflammatory and anti-apoptotic property.Keywords: Mesenchymal stem cells, Nephropathy, Inflammation, Apoptosis, Rats.

Abstract

Pharmaceutical Care by Pharmacist at Public Health Centers in Pandeglang is not considerably optimal. This can be seen from the increasing of the using anti-hypertensive drugs from year to year at Public Health Center, as the biggest cause of death in Pandeglang Regency categorized to non-communicable diseases. This requires pharmacists to optimize their roles further by increasing their knowledge and capability through training. This study aimed to assess knowledge level and ability of Pharmacists in Public Health Centers in Pandeglang Regency and to find out whether a significant influence could be emerged before and after training. This research was a pre-experimental study with the design of one group given pretest and posttest. Training was held twice. The first training was held in Pandeglang Regency and about Communication Skill and the second one was held in Bandung City and about Anti-Hypertensive Therapy. The results of this study indicated that in the first training the number of respondents who was not good or respondents classified as poor knowledge was 42.86% before training, the number declined after training to 7.14% and the increasing on the number of respondents classified as good enough or fair respondents, from 57.14% before training to 92.86% after training. The average score was 66.57 after training higher than the score before training was 55.36. Moreover, the second training indicated that the number of respondents who classified as poor knowledge before training was 85.71%, the number of declined after training to 14.29% and the increasing on the number of respondents classified as knowledgeable or good respondents, from 0% before training to 14.29% after training. The average score after training was 68.9 higher than the score before training was 42.5. In conclusion, both the first training and the second training can increase Pharmacist knowledge and ability to give Pharmaceutical Care about anti-hypertension.Keywords: Anti-hypertension, Counseling Training, Knowledge Level, Pharmacists.
Clinico-Epidemiological Study of Thalassemia in Wasit Governorate Atta AH Mousa Al-Sarray

Abstract

Background: Globally, thalassemia is the most common genetic hemoglobin disorder. In the developing world, the majority of patients die before the age of 20 years. Objective: To determine the prevalence of thalassemia and different Clinico-epidemiological factors associated with thalassemic patients in Wassit governorate Subjects and Methods: A cross sectional study conducted at thalassemia center in Al-Kut hospital for Gynecology obstetrics and pediatric in Wasit governorate, included 425 patients, the data collection continued for the period of five months stating on 1st of March 2019 to 30th of July 2019. Results: The prevalence of thalassemia was 34.45% of the total patient attending Al-Kut Hospital for Gynecology Obstetrics and pediatrics during the period of the study with a mean ±SD age of patients 14.71±8.5years. Most cases from beta thalassemia, and major genotype regarding the genotype of thalassemia and clinical characteristics highly significant association were found between genotype and age of onset, age at first transfusion, frequency of blood transfusion, and regular blood transfusion. Regarding the complications of thalassemia higher percentage of study sample were iron overload, splenomegaly , protrusion of facial bones and teeth, (89.9%, 83.1%, 55.5%) respectively, while the lower percentage were visual impairments, hepatitis B, AIDS (5.6 %, 0.9%, 0.2%)   respectively. Conclusion: The prevalence of thalassemia was 34.45% of the total patient attending Al-Kut Hospital for Gynecology Obstetrics and pediatrics during the period of the study Most cases from beta thalassemia, and major genotype significant association were found between genotype and age of  onset, age at first transfusion, frequency of blood transfusion, and regular blood transfusion  Recommendations: We recommend a comprehensive medical examination on the identification of pre-marital hemoglobin electrophoresis to enhance genetic counseling and to help reduce the transmission of these genetic diseases among generations, and use of the system-centered risk stratification model in order to individualize patient treatment for each.Keywords: Thalassemia, Hemoglobinopathies, Iraq, Prevalence, Wasit.
Correlation of Myeloperoxidase Activity with Malondealdehyde Levels in Iraqi Patients with Coronary Artery Diseases Saba Z. Hussein

Abstract

Background and Objective: Coronary Artery Disease (CAD) is one of the largest cause of mortality worldwide. The majority of CAD is caused by atherosclerosis, which is the main pathophysiological process underlying of CAD. The purpose of the current study was to investigate the role of myeloperoxidase (MPO) activity in coronary artery disease besides to its relationship with malondealdehyde levels, Athrogenic Index of Plasma (AIP) value and its association with the risk of future evolution of CAD in sera of Iraqi patients. Methods: Two main groups were included in this study; 60 patients (35male&25female) with coronary artery disease, and 40 (23male& 17female) apparently healthy controls comparable for age were enrolled in the current study. The blood samples were obtained for measurements of MPO activity, lipid profile, MDA levels and AIP value; for all participants. Results: The obtained results showed that there was a significant increases in serum MPO activity and MDA levels in coronary artery disease (46.35 ± 5.13 U/L) and (6.44 ± 1.32 nmol/ml) as compared with the control (29.12 ± 4.88 U/L) and (2.75 ± 0.43 nmol/ml), respectively. Meanwhile a highly significant increase in AIP value in CAD patients group in comparison to control group (0.36 ± 0.08, 0.076 ± 0.04),  respectively. The activity of MPO was significantly correlated with MDA levels (r=0.751, p=0.000) and AIP value (r=0.762, p=0.000). Conclusion: According to the obtained results, MPO activity, levels of MDA and AIP value were elevated in patients with CAD. It can be concluded that higher MPO activity and its correlation with high MDA levels and AIP value may be associated with complications of CAD in the Iraqi patients.Keywords: Coronary Artery Diseases, Myelopreoxidase, Lipid peroxidation, Atherogenic Index of Plasma.
