Abstract

Introduction: Emergency is an unexpected and often dangerous situation requiring immediate action, accurate, and correct action to save lives. Delays in conducting emergency assistance often occur in children, adults and the elderly, where the consequences of the delay are permanent disability and even death. Emergency for children becomes an important concern because children are not used to expressing uncomfortable conditions in an emergency condition. Emergency conditions which often occur in children are febrile convulsion, choking and cardiac arrest. The purpose of this study was to identify the differences in parents’ knowledge before and after given education about febrile convulsion, choking, and cardiac resuscitation.  Method: This research employed Pre-Experiment with One Group Pre-test Post-test design. The sample of this research was 70 parents who have children under five years. The sampling technique was used total sampling technique. Univariate analysis was used to determine the demographic data of respondents. Bivariate analysis was used to determine the differences in parents’ knowledge before and after given the education.  Results: The research revealed that (Febrile convulsion P 0,000 <a = 0.05), (Choking P 0,000 <a = 0, 05), and (Cardiac arrest P 0.000 <a = 0.05). Conclusion: There is an effect of giving education toward parents' knowledge in handling children with emergency conditions. It is recommended that further activities and training on emergencies should be carried out steadily since emergencies can occur anywhere. Abstract

The health state of children in Russia has a number of unfavorable trends, i.e. chronic diseases increase, and infectious diseases increase. Objective: To obtain an objective characteristic of the overall sick rate in children of the regional center and the role of urgent pathology in its level and dynamics. Methods: The study base was ‘Children's City Clinical Hospital #8’, serving the entire child population of the Metallurgical District, Chelyabinsk. The number of children for the period from 2008 to 2017 raised from 23.4 thousand to 26.5 thousand children. Results: The total disease appealability decreased from 2195.0 per 1,000 children to 1,733.2 in 2017. At the same time, the total urgent morbidity, according to the emergency department, ambulance and hospital wards increased to 1,025.3 cases per 1 000 children Conclusion: A disease appeal ability decrease is counterbalanced by an increase in urgent morbidity, which means the absence of a real improvement in children's health indicators.Keywords: Sick rate, Child population, Urgent morbidity, Disease appeal ability, State of health.

Abstract

Introduction: The lack of nursing quality indirectly can be seen in patients dissatisfactory. Today society demands service that focuses on patients need, and the correct approach will be determined by the selection of the nursing model. To increase the quality of service in nursing, require a redesigned model through implementing integrated healthcare services. Methods: This is a quasi experimental study to implement an integrated nursing model using an intervention group and a control model. A total of 154 samples were chosen using consecutive sampling technique and equally distributed to each group. T-test were used to analyze the patients satisfaction level of the nursing service. Results: The result shows in the intervention group, there were 60 people (78%) that are satisfied, while in the control group, there is only four-person (5%). The increase might be caused by the nursing that is more consistent, cooperative, and responsive. The nurse understand problem identification and actively collabore in search of solution in the integrated model. Constructive integrated structure model about responsible is essential in increasing professional nurse performance. Conclusion: In conclusion, there are significant differences; higher patient satisfaction level on the model implementing integrated model than without implementing integrated model.Keywords: Integrated model, Service Focus on Patients.

Abstract

Alternaria alternate was isolated from orange juice, using potato dextrose agar (PDA) media, incubated for 5 days at 27 co, the evaluating anti fungal effects were seven different concentration (1.25, 2.5, 5, 10, 15, 20, 25) mg / ml of the extract alcohol: (William pear extract = WPE) and (Propolis extract = PE), the inhibitory efficacy of each extract against the fungus was increased by increasing its concentration. Also observed that the inhibitory activity of (PE) was greater than (WPE).The separation process of (PE) showed a presense: Flavone, 3-hydroxy flavone and Quercetin and absense these compounds in (WPE).Keywords: Propolis, Wiliam pear, A. alternate, TLC.

Abstract

ABSTRACTBackground: Preeclampsia characterised by new-onset hypertension with systolic blood pressure ≥ 140 mmHg or diastolic blood pressure ≥ 90 mmHg, measured on two occasions at least four hours apart, and proteinuria of > 0.3 g per 24 hours or ≥ 1+ proteinuria, detected by urine dipstick after 20 weeks of pregnancy, or in the absence of proteinuria. Objectived: This study aimed to determine effect of EVOO as a strong exogenous antioxidant to decrease TNF-α, HSP-90 and ET-1 level in preeclampsia rat model. Method: This research consisted of five groups; negative control, positive control (preeclampsia rat model), doses 1, 2, and 3 that were preeclampsia rats given EVOO in 3 different doses (0.5 mL/day, 1 mL/day and 2 mL/day respectively). After sacrificed, placentas were collected to determine HSP-90, TNF-α and  ET-1 level. Result: Result of this study showed that there was a reduction of HSP90 (p=0,00), TNF-α (p=0.011), ET-1 (p=0.02). Conclusion: Administration EVOO decreased HSP-90, TNF-α and ET-1 Level in preeclampsia rat modelKeywords: Preeclampsia, EVOO, HSP-90, TNF-α, ET-1,  Antioxidant, Oxidative stress.

Abstract

Netherton syndrome (NS) is a rare severe autosomal recessive disorder of cornification characterized by a triad of symptoms: congenital ichthyosiform erythroderma, trichorrhexis invaginata and an atopic diathesis with elevated immunoglobulin E (IgE). Skin surface recovered in the background of IDA correction with iron preparations and vitamin and mineral complexes. Family history: patient has a 55-year-old uncle with ichthyosis vulgaris and her 4-year-old daughter suffers atopic dermatitis. The patient was treated with systemic steroids (prednisolone solution 90mg per day IV days 1-5; 60 mg per day, days 6-11; 30 mg per day, days 12-15) with marked improvement of the skin lesions in 1, 5 weeks. At the time of discharge from the hospital almost complete regression of eruptions and desquamation was observed, there were some residual effects in the form of post-inflammatory erythema. We present a case of a 29-year-old woman with Netherton syndrome, primarily diagnosed with atopic dermatitis. The iron-deficiency anemia concurrent with gastro-intestinal tract disorder was also observed. Topical corticosteroid therapy showed no clinical improvement. Combined in-depth clinical assessment and genetic testing resulted in NS diagnosis with subsequent successful systemic therapy-intestinal tract. Trichoscopy examination of the scalp hair demonstrated trichorrhexis nodosa, one of the typical NS signs. Initial parameters. HGB: 95.0g/l (normal range 120-140g/l), RBC: 3.451012/l; (normal range 3.7-4.71012/l), Hypochromia: ++ Color indicator: 0.64 (normal range 0.85-1.15), Eosinophils 13%; (normal range 0-5) and serum IgE level of 156 IU/ml (normal range 20-100 IU/ml). Quality blood indicators after treatment are within normal limits or close: HGB level has increased, eosinophils number has decreased, which indicates an almost complete inflammatory process cessation in the body. The main change vital for the body, which the physiological blood indicators depend on, is an increase in iron content (almost 1.5 times). Indicators of toxic substances, such as uric acid, decreased. Each of the patients with Netherton's syndrome requires a strictly individual approach. Emphasis on the advisability of prescribing systemic glucocorticosteroids is also essential, as well as the dosage of aromatic retinoids. In this case, the dosage and prescription of drugs of this type depends on the disease severity, in particular on the pathology’s manifestations on the skin surface.  Genetically caused disease (Netherton syndrome) proceeded against the background of acquired disease (anemia). Our strategy has shown effectiveness, caused by systemic steroids.Key words: Netherton syndrome, Trichorrhexis invaginata, Ichthyosiform erythroderma, SPINK 5, Iron-deficiency anemia.

Abstract

Background: The systemic lupus erythematosus (SLE) is an autoimmune disease with various clinical manifestations. SLE indicated by the presence of autoantibodies to the nucleus components and immune complex deposits. Prevalence ratio of SLE is higher in women than in men, but male patients seem to have a higher frequency of symptoms and more aggressive disease. So, this case study aims to evaluate A seventeen-year-old boy patient with lupus nephritis as a complication of Systemic Lupus Erythematosus (SLE) based on laboratory evaluation. Case Description: A seventeen-year-old male patients referred to the emergency department in Sanglah General Hospital with swelling on both eyelids and legs for two weeks before admission. The patient has high blood pressure, discolouration of urine and significant weight gain about 14 kilograms in two weeks due to swelling. Several examinations performed and the results were indicating anaemia, proteinuria, hematuria, hypoalbuminemia, high BUN and creatinine level, hyperkalemia, hyperphosphatemia and high titer of ANA-IF and anti-ds-DNA level. The patient diagnosed with lupus nephritis and treated with antihypertension and diuretic. Conclusion. The most common complication of SLE is lupus nephritis, and its prevalence varies significantly by region, race and tribe. Although overall improvements in the treatment of SLE patients have increased in kidney survival rates, the prognosis remains unsatisfactory. Early detection and diagnosis should be done so that the patient’s condition worsens not occur because the late diagnosis of LN can increase the risk of End-Stage Renal Disease.Keywords: Autoimmune Disease, Lupus Nephritis, Systemic Lupus Erythematosus.

Abstract

Background: Liver toxicity often results from drug administration, such as antituberculosis. The prevalence of liver toxicity in tuberculosis patients remained high in the last decade. Liver toxicity induced by isoniazid (INH) and rifampicin (RIF) combination might have different features and mechanisms with liver toxicity induced by other substances or drugs (acetaminophen, carbontetrachloride, d-galactosamine, ethanol or dimethylnitrosamine). The goal of this study was to investigate the toxic dose and duration  of  administration of combined isoniazid and rifampicin that contributed to the hepatic injury in rats. Methods: A combination of INH-RIF contained 100 mg of INH and 100 mg of RIF.  The induction of INH-RIF was given for 28 consecutive days. Twenty rats were divided into five groups: control, group 1 (dose 50 mg), group 2 (dose 100 mg), group 3 (dose 200 mg), group 4 (dose 400 mg),and group 5 (dose 800 mg). At the end of induction, serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were measured, and liver histopathology was evaluated. Data were analyzed with one-way Anova test.  Results: Serum AST and ALT levels were significantly higher in groups 1 and 2 compared to control (p<0.001). Post-hoc analysis revealed that 50 mg and 100 mg dose significantly increased the serum AST and ALT. Conclusion: There were significant differences in serum AST and ALT, as well as histopathological scores, among rats induced by INH and RIF combination.Keywords: Isoniazid; Rifampicin; Liver; Animal.

Abstract

Background: Intraabdominal adhesion after surgery causes pain, infertility, and chronic abdominal pain. Intraabdominal adhesion also causes longer surgery time and increases intraoperative complications. This study aimed to prove that the use of polypropylene suture causes less intraabdominal adhesion and less inflammation compared to polyglycolic acid suture.  Methods: This was a randomized posttest-only control group study. Thirty-six mature male Wistar rats weighing 200-250 g were anesthetized with ketamine 50 mg/kg intramuscularly. It was incised on the inner abdominal wall layer by layer until peritoneum was seen. Wistar rats in Group A were sutured with polypropylene 4-0 with a half-centimeter apart. Rats in Group B were sutured with polyglycolic acid 4-0 suture. Peritoneum in both groups was not closed, and the abdomen was closed layer by layer, and then the lesion was enclosed with bandage. Relaparotomy was performed on day 14 and 30 to evaluate the presence of adhesion and inflammatory cells, MMP-1, TGF-ß, and TNF-α. Results: There was a significant difference between polypropylene-based sutures compared polyglycolic acid in terms of the presence of intraabdominal adhesion on 30 days after laparotomy (p=0.016). The infiltration of neutrophil, macrophage, and lymphocytes were significantly lower in polypropylene groups compared to polyglycolic acid (p<0.001). Conclusion: Polypropylene-based sutures resulted in less adhesion and inflammation compared to polyglycolic acid sutures in Wistar rats underwent laparatomy.Keywords: Laparotomy, Lymphocyte, Macrophage, Neutrophil, MMP-1.

Pollution consider the most dangerous that faced by human and living organisms on the earth,  oxygen and water is the necessary sources  for our life,  air and water pollution effects on human, animals and plants .Ecology study the relations between living things and its habitat forming certain ecosystem. Ecosystems explain any relation types between living organism and the environment. There are many problems in earth affected continue of life, ozone depletion permit  to entry of  U.V. rays from sun to earth,  and cause increase  dangerous  for human, animals, microorganisms, and plants. Desertification and deforestation in lands acts for decreasing of vegetation area.
