Jurnal Ilmu Kedokteran Keluarga is a journal managed by the Department of Family Medicine, Faculty of Medicine, Brawijaya University who has a mission to improves the health and well-being of individuals, families, and communities through leadership in education and clinical care. Jurnal Ilmu Kedokteran Keluarga is published regularly twice a year. Jurnal Ilmu Kedokteran Keluarga contains research articles on basic medical, clinical medicine, epidemiology, prevention, technology, social medicine, medicine in primary health settings related to family medicine. We accept all types of research articles, case reports, systematic reviews, meta-analyses, literature reviews, and letters to editors. The article review process will involve two reviewers in a double blind manner. Jurnal Ilmu Kedokteran Keluarga provides open access articles that can be accessed publicly and freely.

Abstract

Allergic rhinitis (AR) is an allergic response that causes varied symptoms in the nose by exposure to allergens. A patient with a family history of atopy has a higher risk of AR. Various trigger factors can trigger the allergic response. Symptoms disturb daily life activity, decrease productivity, and cause many complications if left untreated. This case was found in primary health care in Malang city. Complete history-taking was done to identify the risk factors, and then some home visit was done to identify the trigger factors. The patient was a woman with a history of atopy. Trigger factors found came from the environment, but there is a possibility that the trigger factor is psychological pressure. This case report will discuss the role of family medicine and its application in allergic rhinitis. Comprehensive and holistic management is needed because of the complex relationship between biopsychosocial factors with AR. ABSTRAK Rhinitis alergi merupakan suatu respon alergi yang menyebabkan sekelompok gejala yang bervariasi pada hidung, karena adanya paparan dari alergen. Penderita dengan riwayat keluarga atopi lebih berisiko memiliki rhinitis alergi. Terdapat berbagai macam faktor pencetus yang dapat mencetuskan respon alergi tersebut. Rhinitis alergi dapat menyebabkan gangguan pada aktivitas harian dan menurunkan produktivitas karena gejala yang ditimbulkannya, dan menyebabkan berbagai komplikasi bila dibiarkan. Kasus ini ditemukan dari fasilitas kesehatan layanan primer di Kota Malang. Anamnesis dilakukan secara mendalam untuk mengidentifikasi faktor risiko, kemudian dilakukan kunjungan rumah dan tempat kerja pasien untuk mengidentifikasi faktor pencetus. Penderita pada kasus ini merupakan wanita dengan riwayat atopi. Faktor pencetus berasal dari lingkungan, tetapi juga terdapat kemungkinan bahwa pencetusnya berupa tekanan psikologis. Pada laporan kasus ini akan dibahas bagaimana peran dan penerapan kedokteran keluarga pada kasus rhinitis alergi. Penanganan komprehensif dan holistik diperlukan karena keterkaitan faktor biopsikososial yang kompleks pada rhinitis alergi. Abstract

Background: Many factors influence Children's health, including the mother's employment status and exclusive breastfeeding.  Purpose: To present the case of children with OMSA (Acute Suppurative Otitis Media) and several risk factors for recurrent upper respiratory tract infection (URI), not getting exclusive breastfeeding, and working mothers.  Case: A boy, one-year-old and four months, came to the family doctor's clinic with his mother. They were complaining of discharge from his right ear. The fluid has come out from the right ear since seven days ago and does not smell. He had a recurrent cold previously. He only gets exclusive breastfeeding for two months, and the mother was busy with work. On examination, there was a central perforation on the right ear tympanic membrane. He was treated with antibiotics and decongestants, then turned to an ENT specialist. After a home visit at two weeks of therapy, the tympanic membrane was closed.  Conclusion: The importance of controlling risk factors to prevent recurring ASOM. ABSTRAK Latar Belakang: Kesehatan anak dipengaruhi oleh banyak faktor, salah satunya adalah status pekerjaan ibu dan pemberian ASI eksklusif. Tujuan: Untuk menyajikan kasus anak dengan OMSA (Otitis Media Supuratif Akut) dengan faktor resiko ISPA berulang, tidak mendapatkan ASI eksklusif, dan ibu yang bekerja. Kasus: Seorang anak laki-laki berusia 1 tahun 4 bulan datang dengan ibunya ke klinik dokter keluarga dengan keluhan keluar cairan dari telinga kanan. Keluar cairan dari telinga kanan ini sejak 7 hari yang lalu dan tidak berbau. Sebelumnya pasien mengalami pilek yang sering kekambuhan. Pasien hanya mendapatkan ASI eksklusif selama 2 bulan, dan Ibu sibuk bekerja. Pada pemeriksaan didapatkan perforasi sentral pada membrane timpani telinga kanan. Pasien diterapi dengan antibiotic dan decongestan, kemudian dirujuk ke dr Spesialis THT. Setelah dilakukan homevisit pada 2 minggu terapi, didapatkan membrane timpani sudah menutup. Abstract

Stroke is the most common cause of death and the third leading cause of disability globally. The incidence is increasing and causing various problems including disease complications, high health costs, and low quality of life. This case report discusses the diagnosis and management as a whole using a family medicine approach to the elderly who experience post-stroke immobilization and its complications. Abstrak Stroke adalah penyebab kematian paling umum dan penyebab kecacatan terbanyak ketiga secara global. Insidennya makin meningkat dan menyebabkan berbagai masalah meliputi komplikasi penyakit, biaya kesehatan yang tinggi, dan kualitas hidup yang rendah. Laporan kasus ini membahas diagnosis dan tatalaksana secara menyeluruh dengan pendekatan ilmu kedokteran keluarga pada lansia yang mengalami imobilisasi post stroke beserta komplikasinya. Abstract

During the Covid-19 pandemic, learning activities had to be done online, increasing audiovisual technology usage. Although there are many benefits of PLD usage, these devices also indirectly harm hearing. This study aims to determine the effect of students' knowledge, attitudes, and behaviour in PLD usage on the speech frequency hearing threshold. It was observational-analytic research through filling out questionnaires. The sample was the medical students who took online learning during the pandemic. The respondents were selected by consecutive sampling and according to the inclusion criteria. Bivariate analysis was performed using the chi-square test. The variables measured were knowledge, attitudes, the behaviour of PLD usage, and speech frequency hearing threshold. The results showed a relationship between knowledge in PLD usage and speech frequency hearing threshold (p-value < 0.05). In contrast, attitudes and behaviour did not find a relationship with the speech frequency hearing threshold (p-value > 0.05). This study concludes that there were incidences of Hearing loss in college students in the second year of the pandemic for various reasons, including PLD usage. There was an influence between knowledge regarding excessive PLD usage that can cause hearing loss. There was no effect on attitudes and behaviour of PLD usage on speech frequency hearing thresholds.   Abstrak Di masa pandemi Covid-19, kegiatan pembelajaran harus dilakukan secara daring sehingga penggunaan teknologi audiovisual meningkat. Meski banyak manfaat penggunaan PLD, perangkat ini juga secara tidak langsung membahayakan pendengaran. Penelitian ini bertujuan untuk mengetahui pengaruh pengetahuan, sikap, dan perilaku siswa dalam penggunaan PLD terhadap ambang dengar frekuensi bicara. Jenis penelitian ini adalah penelitian observasional-analitik melalui pengisian kuesioner. Sampelnya adalah mahasiswa kedokteran yang mengikuti pembelajaran daring selama pandemi. Responden dipilih dengan cara consecutive sampling dan sesuai dengan kriteria inklusi. Analisis bivariat dilakukan dengan menggunakan uji Chi-Square. Variabel yang diukur adalah pengetahuan, sikap, perilaku penggunaan PLD, dan ambang dengar frekuensi bicara. Hasil penelitian menunjukkan adanya hubungan antara pengetahuan penggunaan PLD dengan ambang dengar frekuensi bicara (p-value < 0,05). Sebaliknya, sikap dan perilaku tidak didapatkan hubungan dengan ambang dengar frekuensi bicara (p-value > 0,05). Penelitian ini menyimpulkan bahwa terdapat kejadian gangguan pendengaran pada mahasiswa pada tahun kedua pandemi karena berbagai sebab, termasuk penggunaan PLD. Terdapat pengaruh antara pengetahuan tentang penggunaan PLD berlebihan yang dapat menyebabkan gangguan pendengaran. Tidak ada pengaruh sikap dan perilaku penggunaan PLD terhadap ambang dengar frekuensi bicara. Abstract

Carpal Tunnel Syndrome (CTS) is a neuropathic entrapment due to compression of the median nerve that passes through the carpal tunnel at the wrist. The most common etiology of CTS is a history of repetitive wrist movements such as sewing. This case report discusses the diagnosis and management of CTS using a family medicine approach. Abstrak Carpal Tunnel Syndrome (CTS) merupakan jeratan neuropati akibat kompresi pada nervus medianus yang melewati carpal tunnel di pergelangan tangan. Etiologi paling sering pada CTS yaitu riwayat gerakan pergelangan tangan yang bersifat repetitif seperti menjahit. Laporan kasus ini membahas diagnosis dan tatalaksana CTS secara menyeluruh dengan pendekatan ilmu kedokteran keluarga.
