Abstract

Trigger finger is an inflammation of the tendon or connective tissue between bones and muscles, which results in a painful sensation when the affected individual bends or extends the finger. In this case, the condition was identified at a primary health care facility in Malang City. A thorough medical history was collected to identify risk factors, followed by a home visit to further detect these risk factors. This case highlights the role and application of family medicine, given the complex interplay of biopsychosocial variables that necessitates comprehensive and holistic treatment.

Abstract

Schizophrenia is a chronic mental disorder characterized by episodes of psychosis, including hallucinations, delusions, and disorganized thinking with longterm of antipsychotic medications that can significant effect complication such as the development of extrapyramidal syndrome (EPS). This case aims to explore the interplay between the neurological side effects of antipsychotic medications, specifically EPS, and the impact of family dynamics on the course of schizophrenia. Understanding this relationship is essential for developing comprehensive treatment plans that address both the medical and psychosocial needs of patients with schizophrenia, thereby improving their quality of life.

Abstract

A 56-year-old woman presented with the primary complaint of urgency hypertension and suspected heart failure. The patient has a history of obesity and a sedentary lifestyle, both of which are key risk factors for the development of hypertension and cardiovascular diseases. Her medical history revealed infrequent physical activity, a high-calorie diet, and a family history of hypertension and heart disease. Physical examination and supporting tests indicated markedly elevated blood pressure, signs of heart failure, and changes in the electrocardiogram suggesting possible left ventricular hypertrophy. Genetic factors, obesity, and a sedentary lifestyle played a significant role in the exacerbation of her condition. Management involving blood pressure control, heart failure therapy, and lifestyle modifications is crucial to reduce the risk of further cardiovascular complications. This article aims to discuss the relationship between obesity, sedentary behavior, genetic predisposition, and the development of hypertension and heart failure in older women. Both medical and non-medical histories were explored to determine the involved risk factors, followed by two home visits to further assess and identify additional risk factors. This case report highlights the role of the family medicine approach, where the interaction of biopsychosocial variables necessitates holistic and comprehensive therapeutic.

Abstract

Chronic Kidney Disease (CKD) is a significant global health issue, with hypertension being one of its major risk factors. This article explores a comprehensive Family Medicine-based approach to managing a 54-year-old male patient with CKD accompanied by hypertension. Through a holistic strategy, emphasis is placed on blood pressure control, lifestyle modifications, and strengthening the family's role in supporting the patient’s adherence to therapy. This approach aims to enhance the patient's quality of life, prevent further complications, and optimize remaining kidney function. The study underscores the importance of interprofessional collaboration and effective communication between the patient, family, and healthcare providers for successful CKD management within a primary care context.

Abstract

Benign Paroxysmal Positional Vertigo (BPPV) and isolated systolic hypertension are common conditions in the elderly population. This article discusses the case of an 80-year-old female with a combination of these conditions, where advanced age serves as a significant risk factor. A multidisciplinary approach was employed, involving therapy for BPPV management and tailored lifestyle modifications and pharmacological therapy for systolic hypertension. The study highlights the importance of comprehensive assessment of age-related risk factors, integrated symptom management, and the family’s role in supporting patient care. The findings suggest that individualized and collaborative care strategies can improve the quality of life in elderly patients.

Abstract

Background: The @telmed.fkub account is a health promotion program of the Medical Faculty of Brawijaya University that provides health information services to the public. This program conducts regular evaluations to determine the effectiveness of services to the community.  Objective: To evaluate this program in determining the user's satisfaction with appearance, usability, service, and willingness to recommend. Then, the level of satisfaction is linked to user demographic data, whether there is a relationship or not.  Methods: This research uses a cross-sectional observational research method where data is collected by a Google Form questionnaire distributed randomly to Instagram account followers @telmed.fkub. The demographic variables were age, gender, education level, and educational background. The satisfaction level was categorized into five subtypes: appearance, service, usability, and willingness to recommend. These satisfaction levels were then measured on a five 5-level Likert scale. The Chi-Square or Fisher's Exact tests analyzed the collected data to determine the p-value.  Results: The total number of respondents was 99. Most respondents are satisfied with the @telmed.fkub akun and program (96.97%). It was also shown that there was a significant relationship between the genders of @telmed.fkub users in telehealth display satisfaction, and there were no significant differences between age groups, education levels, and backgrounds education of @telmed.fkub users in telehealth satisfaction.  Conclusion: There is a significant relationship between gender and appearance satisfaction levels of @telmed.fkub users.

Abstract

Bacterial vaginosis is a reproductive system infection that causes a variety of symptoms. This condition can be caused by a variety of risk factors, including sexually transmitted infection from her husband. This case was detected in a Malang City primary health care. An in-depth history is collected to identify risk factors, followed by a home visit to detect risk factors. We shall explore the role and use of family medicine in cases of bacterial vaginosis in this case report. Because of the complicated interplay of biopsychosocial variables, comprehensive and holistic treatment is required.

Abstract

Acute gastritis, a prevalent gastrointestinal condition, is often affected by lifestyle and psychosocial factors, requiring a comprehensive approach in primary care. This case report highlights the presentation, contributing factors, and comprehensive management of acute gastritis in a young adult, underscoring the essential role of family medicine principles in this context. We present the case of a 30-year-old female translator suffering from acute epigastric pain, clinically diagnosed as acute gastritis. Her condition worsened due to irregular eating habits, excessive intake of spicy foods and coffee, late sleeping hours, and work-related stress. The patient's symptoms are consistent with known risk factors for gastritis, such as eating habits, caffeine consumption, and mental stress. Although pharmacological intervention alleviated symptoms, the reemergence of symptoms following dietary indiscretion highlighted the significance of non-pharmacological approaches. The Family APGAR score indicated that the family was functioning at a moderate level, suggesting areas where communication and support could be improved. Our intervention focused on patient-centered education, emphasizing lifestyle modification, stress management, and promoting family support. This case demonstrates that acute gastritis in young adults frequently arises from alterable lifestyle factors. Effective management extends beyond simply prescribing medicine. It also includes teaching them about their condition, encouraging them to adopt healthy habits, helping them manage stress, and providing support from family and friends, all of which are important aspects of family medicine.

Abstract

Chronic tonsillitis, a frequent recurring inflammation of the pharynx, profoundly affects quality of life. Tonsils, which are important immune organs, are constantly exposed to germs. A long-lasting infection can cause chronic inflammation and lead to structural changes in the body, which is why family medicine needs to take a comprehensive approach. A 35-year-old male exhibited recurrent pharyngitis, T4 tonsil hypertrophy, and persistent snoring. This shows how much his condition affects his body, since he has had many episodes that have affected his daily life. A microbial infection, typically bacterial or viral, causes recurrent tonsillitis, accompanied by an immune response that isn't functioning properly in the tonsillar crypts. This causes long-term swelling and blockage of the airways, which can make it hard to breathe and snore. Lifestyle factors that make you more likely to get sick, like your diet and smoking history, make this worse. A comprehensive family medicine evaluation, encompassing biomedical, psychosocial, and environmental factors, informs individualized management that targets infectious agents, immune function, and physiological health. This method, which focuses on infectious causes, immune responses, and physical effects, is crucial for achieving good results and maintaining health in individuals with chronic tonsillitis.

Abstract

During the Covid-19 pandemic, learning activities had to be done online, increasing audiovisual technology usage. Although there are many benefits of PLD usage, these devices also indirectly harm hearing. This study aims to determine the effect of students' knowledge, attitudes, and behaviour in PLD usage on the speech frequency hearing threshold. It was observational-analytic research through filling out questionnaires. The sample was the medical students who took online learning during the pandemic. The respondents were selected by consecutive sampling and according to the inclusion criteria. Bivariate analysis was performed using the chi-square test. The variables measured were knowledge, attitudes, the behaviour of PLD usage, and speech frequency hearing threshold. The results showed a relationship between knowledge in PLD usage and speech frequency hearing threshold (p-value < 0.05). In contrast, attitudes and behaviour did not find a relationship with the speech frequency hearing threshold (p-value > 0.05). This study concludes that there were incidences of Hearing loss in college students in the second year of the pandemic for various reasons, including PLD usage. There was an influence between knowledge regarding excessive PLD usage that can cause hearing loss. There was no effect on attitudes and behaviour of PLD usage on speech frequency hearing thresholds.   Abstrak Di masa pandemi Covid-19, kegiatan pembelajaran harus dilakukan secara daring sehingga penggunaan teknologi audiovisual meningkat. Meski banyak manfaat penggunaan PLD, perangkat ini juga secara tidak langsung membahayakan pendengaran. Penelitian ini bertujuan untuk mengetahui pengaruh pengetahuan, sikap, dan perilaku siswa dalam penggunaan PLD terhadap ambang dengar frekuensi bicara. Jenis penelitian ini adalah penelitian observasional-analitik melalui pengisian kuesioner. Sampelnya adalah mahasiswa kedokteran yang mengikuti pembelajaran daring selama pandemi. Responden dipilih dengan cara consecutive sampling dan sesuai dengan kriteria inklusi. Analisis bivariat dilakukan dengan menggunakan uji Chi-Square. Variabel yang diukur adalah pengetahuan, sikap, perilaku penggunaan PLD, dan ambang dengar frekuensi bicara. Hasil penelitian menunjukkan adanya hubungan antara pengetahuan penggunaan PLD dengan ambang dengar frekuensi bicara (p-value < 0,05). Sebaliknya, sikap dan perilaku tidak didapatkan hubungan dengan ambang dengar frekuensi bicara (p-value > 0,05). Penelitian ini menyimpulkan bahwa terdapat kejadian gangguan pendengaran pada mahasiswa pada tahun kedua pandemi karena berbagai sebab, termasuk penggunaan PLD. Terdapat pengaruh antara pengetahuan tentang penggunaan PLD berlebihan yang dapat menyebabkan gangguan pendengaran. Tidak ada pengaruh sikap dan perilaku penggunaan PLD terhadap ambang dengar frekuensi bicara.
