Abstract

Introduction: Chronic Obstructive Pulmonary Disease (COPD) is a heterogeneous lung condition characterized by chronic respiratory  symptoms due to abnormalities of the airways  and/or  alveoli (that cause persistent, often progressive, airflow obstruction. The main environmental exposures leading to COPD are tobacco smoking and the inhalation of toxic particles and gases from household and outdoor air pollution. COPD could lead to worse outcomes from COVID-19. Oxygen therapy is the main treatment method for patients with COPD and hypoxemia. Compared with NIV, HFNC can significantly reduce the incidence of pressure injuries such as facial and nasal bridge rupture. And also HFNC could be a valuable and feasible treatment option for patients with COVID-19 pneumonia, with remarkable clinical advantages. Method: This was a retrospective case report, after analysis of patient clinical data. The patient provided written informed consent to publish their case details and any accompanying images. Results: A 63-year-old man was presented with a complaint of shortness of breath that had been felt for two days before entering the hospital. The first day of treatment, the patient complained of increasing severe shortness of breath accompanied by fever. Later on, the patient was diagnosed with an acute exacerbation of COPD accompanied by COVID-19. During 4 days of hospitalization, the patient was admitted to the ICU because of worsening breathing and was given oxygen therapy with HFNC and other drugs for 8 days. Patient showed improvement and was then discharged after seven days of hospitalization. Conclusion: HFNC can be an alternative therapy in COPD therapy with COVID 19, because it has good tolerance and physiological effects

Abstract

Introduction: Myasthenia gravis (MG) is one of the most common neuromuscular junction disorders with various clinical presentations. Several studies showed cognitive function decline in MG patients which affects cognitive domains in memory, attention, executive function, and verbal. The involvement of the central cholinergic system, known as central cholinergic deficits are thought to manifests as impaired cognitive function in patients with MG. The purpose of this study was to evaluate cognitive function characteristic in patients with myasthenia gravis. Method: This study used a cross-sectional design, involved 33 myasthenia gravis patients in Neurology Outpatient Clinic of Haji Adam Malik Central General Hospital and 33 subjects in healthy control group. Cognitive function tests were performed using the Indonesia version of Montreal Cognitive Assessment (MoCA-Ina). Mann-whitney test was performed to evaluate cognitive performance difference in both groups. Results: : The results were compared between MG and healthy control group. The mean of MoCA-Ina score was significantly lower in MG group compared to healthy control group. The result of this study showed difference in cognitive performance between MG patients and healthy control group (p<0.001). This study showed delayed memory, attention, verbal, abstraction, visuospatial and executive function performance was decline in MG patients. Conclusion: This study concludes significant difference of cognitive performance based on MoCA-Ina scores between myasthenia gravis patients and healthy control group.

Abstract

Introduction: Tension-type headache (TTH) is one of the major public health concerns globally with an incidence of 1.89 billion people. Fatigue is potentially debilitating and affects an individual's quality of life and is known to be associated with the case of TTH. The purpose of this study was To find out the relationship between fatigue and the incidence of tension-type headache in the first semester resident doctors of the Faculty of Medicine Universitas Sumatera Utara. Method: The research design was an observational study with a cross-sectional approach which was undertaken at Haji Adam Malik Hospital and Network Hospital in January to February 2022 with all first semester resident doctors as research subject at Faculty of Medicine, Universitas Sumatera Utara in the period of August 2022. Fatigue was measured through the Fatigue Severity Scale (FSS), TTH frequency was measured using the third of the International Classification of Headache Disorders (ICHD-3) criteria, and TTH pain intensity was measured using numerical rating scale (NRS). Meanwhile, to analyze the relationship between fatigue and the incidence of tension-type headache, the Spearman correlation test was used. Results: : Total subjects who met the inclusion and exclusion criteria was 98.  Regarding to the results of statistical tests, a significant relationship was obtained between fatigue and TTH frequency (r= 0,294, p= 0,003), but there was no relationship between fatigue and the pain intensity (r= 0,143, p= 0,160). Conclusion: There was a significant relationship between fatigue and TTH frequency. However, the relationship was not significant between Fatigue and the pain intensity

Abstract

Introduction: Continuous renal replacement therapy (CRRT) is a type of renal replacement therapy commonly used in the ICU for hemodynamically unstable patients. CRRT can comprehensively achieve various goals such as reducing inflammatory mediators, alleviating fluid overload, and correcting acidosis. Therefore, whether CRRT is necessary and when to initiate CRRT are basic questions that intensivists must consider when treating critical patients in the ICU to achieve favorable outcomes. Method: This retrospective case report analyzed patient clinical data. The patient provided written informed consent to publish their case details and accompanying images. Results: We present the case of a 23-year-old woman with acute pulmonary edema and peripartum cardiomyopathy who experienced cardiac arrest prior to cesarean section. Postoperatively, the patient was admitted to the ICU, and CRRT was initiated to correct various complications. During treatment, the patient experienced clinical improvement and improvement in respiratory and renal function parameters until she was discharged from the ICU without neurological deficits. Conclusion: Early assessment, the decision to use CRRT, and comprehensive therapy are crucial for the successful treatment of post-cardiac arrest patients with pregnancy-related acute pulmonary edema and peripartum cardiomyopathy in the ICU.

Abstract

Introduction: Left ventricular diastolic dysfunction is common in patients with hypertension. Inadequate management can cause complications such as hypertensive heart disease. Electrocardiography as a tool that is easy to find in health services is known to be able to detect left ventricular diastolic dysfunction. The purpose of this study was to examine the relationship between P wave peak time in leads II and V1 by electrocardiography with left ventricular diastolic function. Method: This study was a cross-sectional study that was conducted by collecting 111 patients with a history of hypertension at RSUP HAM from October 2021 to November 2022. The P wave peak time was measured in leads II and V1 on the ECG. Diastolic function was assessed using echocardiography. A bivariate test was performed to assess the correlation between the two variables. Furthermore, ROC analysis was performed to assess the P wave peak time as a predictor of left ventricular diastolic function. Results: : Total subjects were 111 hypertensive patients consisting of 49 (31.4%) hypertensive patients with normal diastolic function and 62 (68.6%) hypertensive patients with diastolic dysfunction. In bivariate analysis, a significant difference was found between the P wave peak time in lead II of hypertensive patients with normal diastolic function and hypertensive patients with diastolic dysfunction (65.47± 2.5 vs 68.9 ± 2.7;p = 0.001). Based on the ROC analysis, it was found that the P wave peak time could predict left ventricular diastolic dysfunction in hypertensive patients with AUC = 0.81 in lead II. Conclusion: The P wave peak time in lead II can be a predictor of diastolic dysfunction in hypertensive patients.

Abstract

Introduction: COVID-19 is a pandemic situation involving more than 100 countries with a total case globally that has reached more than 700 million cases. Data from WHO reports that about > 7% of patients experience myocardial injury, and 22% of them are patients with critical illness conditions. COVID-19 is caused by the SARS‑CoV‑2 virus (Severe Acute Respiratory Syndrome Coronavirus 2). SARS-CoV-2 infection occurs when the surface spike protein of the virus binds to the angiotensin-converting enzyme 2 (ACE2) receptor. ACE2 is a receptor that can bind directly to viral surface spike proteins which is expressed in the lungs as well as in intestinal epithelium, kidneys, vascular epithelium and heart. Cases: There are three confirmed cases of COVID-19 reported. Case one is about Inferior ST-Elevation ACS. Case two is Non-ST Elevation ACS with differential diagnose Unstable Angina Pectoris and lastly case number three is about Ventricular Tachycardia. Conclusion: Cardiovascular Disease now causes the most deaths in low-and middle-income countries. Many COVID-19 patients have comorbid cardiovascular disease, or have experienced an acute injury to the heart during the course of their illness
