Abstract

Background: Early-onset and early-stage knee osteoarthritis (KOA) is increasingly reported in overweight young adults and is associated with pain, functional limitations, and reduced quality of life. This umbrella review aims to map the domains of Sports Medicine-Oriented Holistic Rehabilitation (SMOHR) interventions, defined as exercise-based therapy combined with at least one additional domain, and to summarize the reported effects and certainty of evidence for pain, physical function, psychosocial outcomes, and quality of life. Methods: A PRISMA 2020-guided search of PubMed, Scopus, Web of Science, and SPORTDiscus identified English-language systematic reviews with meta-analyses and network meta-analyses published between 2021 and 2025. Two reviewers independently screened abstracts and full texts; disagreements were resolved through discussion and review by a third reviewer. Review quality was assessed using AMSTAR-2, and GRADE ratings were summarized according to those reported in the included reviews without repeating assessments. Results: Fifteen meta-analyses were included. Exercise-based rehabilitation was consistently associated with improvements in pain and physical function, although effect estimates varied depending on the comparison and outcome measured. Network meta-analyses also showed favorable comparative performance of knee braces and hydrotherapy for WOMAC-related outcomes; however, subgroup estimates specific to young adults with overweight were rarely reported. Self-management and psychosocial interventions showed more variable effects, with some evidence of benefits for physical activity, pain, and kinesiophobia in specific contexts. Conclusion: SMOHR may be a useful framework for organizing multiple conservative treatments for KOA, but heterogeneity and limited evidence based on age and BMI limit conclusions for young adults with overweight and early-stage disease. stages.

Abstract

Background: The prevalence of diabetes continues to rise worldwide, and diabetic foot ulcers remain a serious complication caused by repeated pressure on the tissue of the sole of the foot due to neuropathy. This study aimed to compare the immediate changes in the plantar pressure time integral (PTI) following the use of Plastazote shoe insoles versus thermoplastic polyurethane (TPU) shoe insoles in adults with type 2 diabetes and peripheral neuropathy. Methods: This study was a randomized, two-group pre-post design study. A total of 24 subjects with type 2 diabetes mellitus were divided into two groups of Plastazote insole group (n = 12) and the TPU insole group (n = 12). PTI was measured using a dynamic plantar pressure measurement system that divided the foot into five regions (heel, midfoot, forefoot, big toe, and toes) before and after insole use. Analyses were performed within groups (pre-post comparison) and between groups (ΔPTI comparison). Safety and user perception were evaluated using a pain VAS during insole use. Results: TPU resulted in a greater reduction in PTI compared to Plastazote in the rear and middle sections of the foot (p ≤ 0.043) as well as in total PTI (p = 0.019), while Plastazote increased PTI in the front section of the foot and total PTI (p ≤ 0.047; p < 0.001). The between-group difference favored TPU (p ≤ 0.024), with no effect on the big toe or other toes (p ≥ 0.721). Conclusion: In patients with type 2 diabetes, the highest plantar pressure is found in the heel and forefoot. The distribution of plantar pressure in type 2 diabetes is influenced more by dynamic factors and neuropathy than by static foot position. Therefore, screening for diabetic foot complications should incorporate static, dynamic, and sensorimotor assessments.

Abstract

Background: Breast cancer is the most common type of malignancy affecting women and principal cause of cancer-associated deaths. Chemotherapy often induces side effects such as fatigue, neuropathy, and muscle weakness, which contribute to impaired balance and increased fall risk. This study aimed to investigate the impact of low-intensity cycle ergometer exercise on balance performance in breast cancer women underwent chemotherapy. Methods: Randomized controlled trial with pretest–posttest design was conducted. Twenty-two eligible participants (aged 18–59 years, stage II–III breast cancer) were randomly assigned to treatment group (low-intensity cycling plus breathing exercises) or control group (breathing exercises only) for eight weeks. Balance was assessed using the Single Leg Stance (SLS) test, Short Physical Performance Battery (SPPB), and Activity-Specific Balance Confidence (ABC) scale. Data were analyzed using paired and independent t-tests or their non-parametric equivalents with a significance level of p < 0.05. Results: The treatment group showed significant improvements in SLS (p < 0.001, d = 3.67), SPPB (p = 0.003, d = 0.475), and ABC scores (p = 0.006, d = 0.934). No significant differences were observed in the control group. Conclusion: Low-intensity cycle ergometer exercise effectively enhances static and dynamic balance, functional performance, and confidence in women with breast cancer during chemotherapy, suggesting its suitability as a safe and beneficial component of rehabilitation protocols.

Abstract

Background: Sedentary behaviour in adolescents is associated with lower cardiorespiratory fitness and higher cardiometabolic risk. Simple functional exercises may represent a practical and accessible way of improving aerobic fitness in school settings. This study aimed to examine the effects of a sit-to-stand plus heel-raise (STS-HR) exercise program on maximal oxygen consumption (VO₂max) in inactive male adolescents. Methods: This single-blind randomized controlled trial was conducted in a high school in Surabaya, Indonesia from July to November 2025. Inactive males aged 15-17 years were randomly assigned to either a control group that did not participate in organized exercise or an intervention group that underwent 8-week supervised STS-HR program three times per week. Predicted VO₂max was assessed before and after the intervention using the Chester Step Test by blinded assessors. Results: Thirty-seven of 39 randomized participants completed the trial. The STS-HR group had a significant increase in predicted VO₂max (36.14 ± 8.53 to 46.52 ± 7.90 mL·kg-1·min-1; p < 0.001) whereas the control group did not (41.48 ± 8.24 to 41.33 ± 9.23 mL·kg-1·min-1; p = 0.911). The between-group difference in change in VO₂max was 10.53 mL·kg-1·min-1 (95% confidence interval: 6.89 to 14.17; p < 0.001). No adverse events, exercise-related discomfort, or injuries were reported during the intervention. Conclusion: An 8-week supervised STS-HR program led to a significant increase in predicted VO₂max in inactive male adolescents. These results provide evidence that simple functional exercises may be an effective and low-cost feasible school-based strategy to improve cardiorespiratory fitness in inactive youth.

Abstract

Background: Total hip arthroplasty (THA) is a frequently performed orthopedic procedure to relieve pain and restore function in patients with degenerative or traumatic changes of the hip joint. Obesity has been identified as a potential modifier of surgical outcomes, but the literature is conflicting on the effects of obesity on postoperative functional recovery compared to non-obese populations. This study aimed to investigate the influence of preoperative body mass index (BMI) on long-term functional outcomes following primary THA, with a five-year follow-up conducted in South Sulawesi, Indonesia. Methods: A retrospective cohort study was conducted involving 30 patients who underwent primary THA at Dr. Wahidin Sudirohusodo Central General Hospital, Makassar, in 2023. Functional outcomes were assessed using the change in Harris hip score (ΔHHS), calculated from preoperative and six-month postoperative evaluations. Statistical analyses included the Shapiro-Wilk test for normality, Spearman’s rank correlation for non-parametric associations, Kruskal-Wallis and Mann-Whitney U tests for group comparisons, and multivariate linear regression to identify independent predictors of functional improvement. Results: Samples consisting of 60% female, mean age 56.8±11.0 years; 66.7% obese. Strong negative correlation between BMI and delta HHS (r=-0.949, p<0.001). Significant differences across BMI categories (p<0.001), with obese II showing the poorest outcomes. BMI was the significant primary predictor (-3.417, p<0.001), while age and gender were not. Conclusion: Higher BMI significantly correlates with poorer functional outcomes post-THA. Weight management after THA is crucial for optimizing the patient’s functional outcomes.
