Abstract

Background: Stroke is still becoming one of the most common non-communicable diseases in Indonesia. Sample Registration System survey in 2016 showed that in Indonesia stroke has become the most common cause of death at all age, about 19.9%. 4 Research conducted by Novbakht found that age, sex, BMI, type of stroke, family history of stroke, hyperlipidaemia, diabetes, and severity of stroke was related and increased the mortality rate. 6 Academic Hospital Universitas Gadjah Mada had not conducted this type of research, so that researcher wanted to know the one-year mortality rate after stroke, factors that related to mortality, and to improve the quality of stroke treatment at Academic Hospital Universitas Gadjah Mada.Objective: To know one-year mortality rate in acute stroke patients in Academic Hospital Universitas Gadjah Mada by looking for the acute stroke characteristic, outcome of stroke patients, and risk factors that cause mortality in stroke patients.Method: This is an analytic research using retrospective cohort study through data collection of acute stroke patients (patients with <5 days stroke onset) during July 2019 until June 2022 then we followed up the condition in the first year after stroke by using medical records, direct examination, or via telephone or video call. Mortality rates were assessed one year after stroke and the predictors of death were evaluated using the cox proportional hazard model.Result: One-year mortality after stroke in our study were related to age (>67 years old) (OR=2,29; 95% CI 1,18-4,46; p=0,015) and the severity of stroke was related to higher NIHSS score (NIHSS score during admission 16-42, (OR=38,49; 95% CI 8,92-166,0; p<0,001)) and ICU admission ((OR=8,01; 95% CI 4,03-15,90; p<0,001)).  Conclusion: An association was found between stroke severity and one-year outcomes of acute stroke patients with old age (>67 years), high NIHSS scores, and ICU admissions

Abstract

BACKGROUND : Trauma becomes the main health problem worldwide due to its high mortality rate. Traffic accidents are a trauma with a high incidence and death rate in Indonesia, specifically in Yogyakarta. Factors causing trauma-related death are the lack of hospital infrastructure, the lack of quality human resources, the lack of standardization of trauma management, and the limited number of hospitals providing comprehensive trauma services. This research aims to identify the characteristics of trauma patients who died while receiving medical treatment at UGM Academic Hospital and to analyze the mortality of patients according to the trimodal distribution to prepare this hospital to become a trauma center hospital.METHOD : This quantitative descriptive research used a cross-sectional method. Data were taken from medical records of patients admitted to the ER of UGM Academic Hospital in 2018-2023 based on predetermined inclusion and exclusion criteria. The research was conducted from March to November 2023.RESULT : This research involved 58 patients consisting of 42 (72.4%) male and 16 (27.6%) female patients. The most common cause of death was brain injury (63%), followed by spinal cord injury (13.7%), multi-organ failure and sepsis (8.6%), cardiac and respiratory arrest (6.8%), and comorbidities patients (6.8%).CONCLUSION : The majority of trauma patients who died at the UGM Academic Hospital are male and aged 60-80 years with the main cause of death of brain injury. The patient dies at the late death stage based on trimodal distribution of death time.

Abstract

Background: ERACS (Enhanced Recovery After Caesarian Surgery) is a fast programrecovery after a Caesarean section in the form of a series of care starting from preoperativepreparation, intraoperative and post-operative care until the patient is discharged. InIndonesia itself, the implementation of the ERACS protocol began to be implemented in2019. UGM Academic Hospital, the ERACS method is still a new service from the MaternalPerinatal Installation and will start running in November 2022. This research aims todetermine the difference in maternal mobilization time post Sectio Caesaria with the ERACSmethod and mobilization of mothers post non-eracs SC at UGM Academic Hospital in 2023.This research is a quantitative research using a quasi-experimental method with anonequivalent posttest control group design. The research was conducted at the UGMAcademic Hospital in April - September 2023. The sampling technique was accidentalsampling. The research results showed that the average mobilization of post-Sc patientsusing the ERACS method was 13.1 hours and the average mobilization of non-ERACS post-Scpatients was 33.7 hours. From the results of the analysis of 54 respondents using theIndependent Sample Test, it shows a value of 0.000. So H0 is rejected and Ha is accepted.Conclusion: there is an effect of giving the ERACS method by accelerating the mobilizationof women giving birth post sc.Keywords: Sectio Caesaria, Maternity, Mobilization, Rapid Mobilization, ERACS

Abstract

Tuberculosis (TB) is a public health problem that is a global challenge and is the third leading cause of death in Indonesia and the first leading cause of death due to infectious diseases. Based on data from the Ministry of Health of the Republic of Indonesia, there has been a decrease in the number of TB cases found and treated. As one of the strategies to control risk factors for TB disease transmission, RSA UGM provides a special clinic for TB patients whose rooms meet national standards. However, several times researchers found that there were still TB patients who controlled outside the TB clinic schedule so that they mingled with non-TB patients, which resulted in increasing the risk of TB disease transmission to patients visiting the UGM Hospital. The World Health Organization (WHO) declared Covid-19 a pandemic on 9 March 2020. This has caused public fear to visit health facilities. This study aims to determine the level of compliance of TB patient visits during the Covid-19 pandemic at the UGM RSA TB Clinic. This descriptive observational study used secondary data, with a total sampling technique and analyses using univariate analysis. Of the 40 samples that met the inclusion and exclusion criteria, 92.5% of patients were in the productive age range (18-65 years), mostly male (70%). The proportion of patients with pulmonary TB was 62.5% and the remaining 37.5% were extra- pulmonary TB patients. There was an increase in adherence of TB patients to control at the TB clinic and on schedule from month 1 to the following months.

Abstract

AbstractBackground: Hormonal therapy is one treatment option for prostate cancer. There are two main categories of hormone therapy: surgical therapy and non-surgical therapy.  Orchiectomy is the surgical method of hormone therapy; medication supplementation is the non-surgical method. Many kinds of indicators are used to track the advancement of patients with prostate cancer receiving hormone therapy. tPSA levels are one of the markers that are regularly evaluated.Methods: In this retrospective study, PSA levels in patients with prostate adenocarcinoma cancer were evaluated before and after bilateral orchiectomy.  Use the search terms "prostate cancer" and "orchiectomy" to find research subject data in the UGM Academic Hospital's medical records installation between 2020 and 2023. The study's findings will be shown in a graphic and statistical analysis while protecting the subjects' anonymity.Results: Most of the participants (n=18, 60.0%) have pre-orchiectomy tPSA level of more than 30 ng/mL, while only almost a quarter (n=7, 23.3%) of participants have post-orchiectomy tPSA level of more than 30 ng/mL. The two research groups (p<0.0001) before and after the orchiectomy procedure, significantly differ between the two research groups based on Wilcoxon paired sample test analysis.Conclusion: TPSA levels are considerably decreased by bilateral orchiectomy.
