MEJORA Medical Journal Awatara adalah sebuah jurnal kesehatan yang berdedikasi untuk mendorong eksplorasi mendalam dalam berbagai aspek ilmu kesehatan dan dinamika masyarakat di era digital. Jurnal ini tidak hanya berfokus pada diseminasi informasi hasil penelitian klasikal tetapi juga mengambil peran sentral dalam memahami bagaimana perubahan teknologi digital mempengaruhi sektor kesehatan dan kesejahteraan masyarakat secara keseluruhan. Artikel-artikel di dalam MEJORA mencakup beragam topik, seperti telemedicine, aplikasi kesehatan digital, manajemen data kesehatan elektronik, pemberdayaan pasien dalam pengambilan keputusan kesehatan, implementasi alat wearable untuk pemantauan kesehatan, serta analisis big data dalam meramalkan tren penyakit. Jurnal ini juga menyoroti isu-isu etika terkait privasi data, keamanan informasi kesehatan, serta kesenjangan akses teknologi di berbagai lapisan masyarakat. MEJORA Medical Journal Awatara terbit secara berkala tiga kali dalam setahun, yaitu pada bulan Januari, April, Juli dan Oktober.

Abstract

All pregnant women have the right to receive high-quality comprehensive prenatal care, especially Antenatal Care (ANC) examinations at least six times (K6) in accordance with the Indonesian Minister of Health Regulation Number 21 of 2021, as crucial in reducing maternal mortality (AKI) and infant mortality (AKB). The achievement of K6 at the Kotakaler Health Center in 2024 shows a figure of 98.87% with an estimate of 101.16%. The purpose of this study is to find out what factors affect the achievement of K6 coverage in the work area of the Kotakaler Health Center, Sumedang Regency. The method is qualitative with a case study approach through in-depth interview techniques to related parties. The results of the study identified high population mobility in urban areas as the main factor that hinders the achievement of K6 coverage, this factor makes it difficult to continuously monitor ANC visits. In addition, the existence of abortion cases also reduces the number of targets that can reach K6. Comprehensive strategies such as indoor and outdoor activities have been implemented. Good knowledge of pregnant women and a positive attitude towards pregnancy examinations support compliance to conduct complete pregnancy examinations, the accessibility of health services has also been supported, complete infrastructure, and the availability of adequate health human resources. However, external challenges such as population mobility still require an adaptive approach.

Abstract

Timely provision of outpatient medical record documents is an essential indicator of hospital service quality because it supports continuity of care, reduces patient waiting time, and improves operational efficiency. Delays in document retrieval may disrupt clinical workflows and affect the overall quality of healthcare services. This study aimed to evaluate the timeliness of outpatient medical record document provision at Hospital X, Cirebon Regency, Indonesia, based on the Indonesian Minimum Service Standards (MSS). A quantitative descriptive research design was employed. The study was conducted in May 2026 using direct observation of 100 outpatient medical record documents selected through consecutive sampling from a population of 11,626 outpatient visits. Data were collected using a structured observation checklist and analyzed descriptively by calculating frequencies, percentages, and measures of central tendency. The findings revealed that only 33.0% of outpatient medical record documents were provided within the required standard of ≤10 minutes, while 67.0% exceeded the standard. The mean document provision time was 10.53 minutes, with a minimum time of 2 minutes and a maximum of 22 minutes. These findings indicate that the outpatient medical record service at Hospital X has not yet fully complied with the Indonesian Minimum Service Standards. Strengthening filing management, optimizing document retrieval procedures, enhancing staff competency, and implementing continuous performance monitoring are recommended to improve the timeliness of medical record document provision and the overall quality of outpatient healthcare services.

Abstract

Comprehensive midwifery care is a holistic, continuous, evidence-based, and woman-centred model of care covering pregnancy, childbirth, the postpartum period, the newborn, and family planning. This study aimed to describe the implementation of comprehensive midwifery care for Mrs. D, a 36-year-old G3P1A1 woman, at UPTD Babakan Public Health Centre, Cirebon Regency, in 2026. A descriptive case study design with a continuity of care approach was used. Data were collected through interviews, physical examinations, observation, and documentation of the maternal and child health book and medical records, and were documented using the SOAP method. Care was provided from 33 weeks of gestation until 41 days postpartum and was analysed by comparing field practice with midwifery care standards. The results showed that antenatal care met the standard of six visits with complete 12T services; labour progressed spontaneously and physiologically within four hours with an estimated blood loss of 60 cc; the newborn was male, weighing 2500 grams, with an Apgar score within normal limits. At the third postpartum visit (day 10), an early-stage localized left breast abscess was identified. The case was managed conservatively in collaboration with the primary health center physician through the administration of oral antibiotics (Amoxicillin 500 mg three times daily for 7 days), health education (IEC) on proper breastfeeding techniques, warm and cold compresses, regular breast emptying, and breast massage. Evaluation on day 41 showed that the breast condition had improved with complete absorption of the abscess, uterine involution was complete, and no further complications were observed. Comprehensive midwifery care utilizing a continuity of care approach proved to facilitate early detection and management of breastfeeding complications during the postpartum period.
